AVAILABILITY TO BE CONSIDERED FOR POSITION: - OPEN AVAILABILITY Shifts 11pm-7am 3pm-11pm 7am-3pm We are currently hiring part-time Support Aides for our Generations I and Crossroads programs. Reporting directly to the Residential Supervisor, the Support Aide provides trauma-informed support to residents and their children and assists in maintaining a welcoming, safe, and supportive community environment, while ensuring that residential services are of the highest quality standards according to the City of Philadelphia and the Commonwealth of Pennsylvania regulatory and funding source guidelines, as well as state licensing requirements. The Generations I program is a congregate care foster care program that serves young women up to age 21 who are pregnant and/or parenting. The Crossroads program supports young adults between the ages of 18-24 to stabilize while acquiring individualized skill development and support to transition successfully into adulthood. This position works one of three shifts (the programs are staffed 24 hrs/day). Specific responsibilities vary based on the shift. Essential Functions Include: Leadership Support an environment that fosters growth and development for all residents and encourages residents to actively participate as a partner in the development of goals and planning of services. Demonstrate ethical and professional interactions with other staff and residents and hold others (staff and residents) accountable when unethical and/or unprofessional interactions do occur. Interact calmly and appropriately in crisis situations and manage positive outcomes for the safety of the residents. Demonstrate positive teamwork in working with other Support Aides and members of the Residential Programs team. Direct Service Ensure the residents and their children are supervised and have adequate meals and activities. Transport residents to appointments, check inventory for supplies, check/purchase requests, and food shopping when required. Ensure medication administration policies and procedures are followed. Support an environment that fosters growth and development for all residents. Documentation & Communication Complete all documentation in a timely fashion and in required formats, including but not limited to: therapeutic and non-therapeutic interactions, unusual incidents, routine activities, progress notes, health & safety checklist, point sheets, medication administration, etc. Complete and submit a daily end of shift report. Ensure residents are provided with written appointment reminders daily. Communicate pertinent information daily with the Support Aide staff regarding doctors appointments, school attendance, transportation, court dates, behavioral therapy appointments and group therapy participation, recreational activities, and any needs and/or concerns that a resident or staff member has presented. Monitoring Monitor and track residents to ensure regular participation in the program, including chores, maintaining cleanliness of rooms, etc. Monitor residents school attendance as applicable. Basic Skill Sets Ability to clearly and positively communicate and interact with all Generation I staff and residents. Ability to give clear and concise oral and written instructions. Proficiency in the use of computers and pertinent software programs a must (i.e., Microsoft Office). Qualifications Education: minimum high-school diploma or GED required. Associates Degree or college degree from four-year, accredited institution a plus. Major in human services or related field preferred. At least 21 years-old (required) Experience: minimum of two years working with adolescents in a child welfare or behavioral health setting or related experience required. Experience with young women, infants, and toddlers preferred. Special Skills: Familiar with Philadelphia child welfare system (i.e., the Department of Human Services, Community Umbrella Agencies), as well as with and intersecting systems (i.e., the court system, probation, behavioral health). Certification in medication administration a plus. Physical Capabilities & Work Environment Ability to walk up and down stairs. Ability to lift at least 20 lbs. Ability to operate a computer. Current valid drivers license and the willingness to drive as needed. Summary: NCS's Wellness &Resiliency program/Outpatient Services Program provides outpatient services that help children and adolescents with depression, anxiety, stress, anger, self-esteem, ADHD, ADHD, bereavement or other emotional, social and behavioral difficulties. Following a comprehensive assessment and consultation, WRP/Outpatient Clinicians identify the child's needs and make recommendations for treatment. Treatment may consist of one or more of the following: individual therapy, family therapy, group therapy, and/or medication management. Generally, sessions are about one hour weekly, though depending on the child's needs, the pace may vary. Evidence-based treatments such as Trauma Focused Cognitive Behavioral Therapy, Cognitive Behavioral Therapy, Milieu Therapy are available. Internship must be able to provide individual therapy and group therapy under the direction of a supervisor. Specific Duties Communication Maintain therapy appointments with children and their families on site and in their home. This also includes following up on cancellations and no-shows. Maintain correspondence with families who do not show for sessions or are unable to be reached via telephone through letters. Obtain signatures from children and their caregivers on all paperwork requiring them on specified dates as the paperwork is prepared. Maintain consistent correspondence with supervisor regarding continuing treatment. Notify Supervisor of any emergency situations within 30 minutes. Follow agency emergency protocol and complete incident report within 24 hours. Attend supervision as scheduled. Consult with psychiatrist and evaluators as directed and provide clinical updates as needed / requested. Clinicians are expected to check mailboxes, email, and voicemail daily. Communicate with WRP case manager when additional support to meet paperwork requirements is necessary at the discretion of the supervisor. Direct Care Provide Group therapy for WRP/Outpatient program as directed. Provide Individual therapy for WRP program as directed. Provide crisis intervention and action in response to acute client behavior, including oversight of CRC transportation and accompaniment, if necessary, for WRP consumers as directed. Provide risk assessments, when necessary, for WRP/outpatient consumers as directed. Ensure the treatment plans are up to date and measurable. Comply with CBH guidelines and authorizations. Complete assessment for children in Wellness & Resiliency Program (WRP) Collaborating with Behavioral Health Technicians (BHT), assisting with psychoeducation, providing feedback, and supervision Assisting with the implementation of psychoeducation Assisting with CRISIS situations, completing Incident Reports, and follow up. Document all telephone contacts with children, their families, and outside agencies via clinical progress note. Follow up with families who cannot be reached via telephone through communication letters. Schedule to meet with children's guardians to complete all treatment plans either on site or in homes. Set up and maintain a weekly therapy schedule for his/her caseload as assigned by the Director. Maintain charts via clinical documentation for all assigned children. Have supervision weekly as scheduled Stay current on educational task per syllabus. Additional Duties Attend all agencies specific trainings and additional professional trainings as assigned. Complete chart reviews as assigned. Complete discharge paperwork and aftercare plans as assigned. Provide administrative and case management duties as assigned. Maintain professional and agency ethics. Follow all agency policies and procedures, Work in accordance with federal, state, and local regulatory standards. Education requirements: Education: Full time enrollment in a master's degree from an accredited university in a clinical mental health discipline, such as psychology, counseling, clinical social work, and marriage and family therapy or related field. A Behavior Consultant (BC) provides intensive therapeutic services to a child in the home, community or school setting. Under the supervision of the Clinical Manager, the Behavior Consultant leads the team to provide clinical direction of services for each child on their caseload, revises and updates the Individual Treatment Plan (ITP), and provides oversight of the provision of the ITP. The BC should consult with adults across settings in that child's life and create appropriate and reasonable goals. Essential Functions of this Position Include: Leadership Coordinates the overall clinical direction of treatment for the children assigned to caseload, in collaboration with family and other treatment professionals. Is a resource to the child, family, treatment team, school and Behavior Health Technicians in crisis and problem situations to develop a crisis plan and resolution. Planning Conducts meetings to plan anticipated clinical needs of consumer. Develops treatment plan based on psychiatric evaluation, per agency and regulatory standards. Completes functional behavioral assessments for clients. Monitoring Ensures that treatment goals, objectives and interventions are appropriate and fit the particular needs of the child and family. Monitors consumer progress on treatment goals and reports this information through treatment plan updates at least every 180 days. Updates treatment plan and crisis plan when significant incidents occur (e.g. Hospitalizations, call to Mobile Crisis, suicidal ideation/threats, etc.) Conducts classroom observations for assigned students. Direct Service Provides child-centered, strengths-based individual psychotherapy weekly. Provides all services that are authorized by the funding source. Provides billable case management in the form of family and school contact to provide updates on treatment, attendance at meetings, phone calls, etc. Communication Maintains regular weekly contact with family, school and other agencies as identified (up to 1 hour per month) regarding client progress on the treatment plan. Clearly and effectively communicates program goals and directives to staff, families, school district personnel, agency staff and other providers of service. Attends treatment team meetings, IEP meetings and ITM meetings. Documentation Documents all therapy sessions, case management activities, attendance at meetings, incidents, etc. within 72 hours of service provision per agency policy and procedure. Creates treatment plan based on functional behavioral assessment and regulatory standards. Documents all billable services, including all case management and collaboration activities. Completes incident reports within 24 hours when significant events occur. Networking Collaborates with schools, families, funding source representatives and internal/external resources as appropriate. Collaborates with treatment team, including Care Coordinator, to refer families to appropriate services to meet needs identified from Social Determinants Scale. Requirements Education: Minimum Requirement - Master's Degree from an accredited university in a clinical mental health discipline AND completed clinical or MH practicum Board Certified Behavior Analyst (BCBA) certification or behavioral analysis certification Graduate degree in Applied Behavioral Analysis (ABA) Licensed BSC Experience: At least one years full-time paid experience in the mental health field. Excellent working knowledge of clinical issues relating to children, adolescents and families.     Job Description Under the supervision of the Foster Care Director, The Resource Parent Support Worker (RPSW); works directly with the Resource Parent to help them achieve and support the well-being, stability and permanency of the child or youth in care. This includes working directly with the resource caregiver to help them successfully meet the needs of the child or youth. The RPSW must provide face-to-face support to the resource caregiver a minimum of one time per month, with more frequent and intense contacts for new placements and during periods of need. When a child or youth is first placed in the home, the RPSW must provide initial, in-person contact with the Resource Caregiver within three business days if they were not present at the actual placement. This cannot be a phone contact. Specific responsibilities of the RPSW include providing direct, in-home support to Resource Caregiver focused on: Ensuring the safety and well-being of the child or youth. As needed, by supporting the Resource Caregiver to stabilize the child or youth in the home and school setting by helping the Resource Caregiver understand from a developmental perspective child or youth behaviors as they may relate to their trauma history. Supporting Resource Caregivers to effectively communicate with any education, medical, dental or other service providers working with the child or youth. Ensuring Resource Caregivers are in compliance with medication administration and documentation. ESSENTIAL FUNCTIONS OF THIS POSITION Communication Maintains appropriate monthly visitation with Resource Families. Advocates for the needs of the consumer to obtain needed resources and supports. Clearly and effectively, communicates all pertinent information to involved parties. Participates in regular weekly supervision with the Foster care Supervisor Communicate with the child's CUA worker, Child Advocate Social Worker and Attorney when legal attention is warranted; Promoting and teaching transferable self- advocacy skills to Resource Parents The RPSW must communicate all safety concerns to the Foster care Supervisor within immediately. The RPSW is responsible for supporting and bridging communication barriers between Resource Parents and Community Umbrella Agency (CUA)/Department of Human Services (DHS) staff. Timely communication between the CUA, Provider and caregiver is essential to promoting the safety; permanency and wellbeing of children and youth. All parties share a mutual responsibility to proactively communicate with one another and respond to emergencies, incidents, requests for information and to schedule meetings. If the subcontractor or the RPSW are not receiving the information they need to appropriately care for children and youth in their care, the chain of command at the CUA is to be utilized. The RPSW communicates with the CUA as well as supports communication between the Caregiver and the CUA by: -(HOWEVER NOT A CUA WORKER) Attending CUA case manager Quality visits occurring in the Resource Home, as invited by the CUA or as requested by the RPSW. Attending teaming meetings, as invited. Attending Family Court, as invited and not in lieu of the CUA's appearance. Attending JPO facilitated Teamings or meetings prior to Court and Court proceedings as needed. Initiating communication with the CUA when the current array of services a child or youth is receiving needs to re-evaluate by the CUA Case Manager based on the child or youth's functioning and status in the placement setting. Obtaining documents from the CUA that may be needed by the Resource Caregiver and providing them to the caregiver, such as consents for medical, dental, or behavioral health care; school documents etc. Obtaining results of medical, dental or treatment appointments the child receives while in the Resource Home and providing them to the CUA. Documentation Maintain documentation of supports and interventions provided to the resource Caregivers. Adheres to the 3700 Foster family regulations Ensures the home is a safe environment for children placed Within 48 hours of placing a child -RPSW must meet Face to face with the resource parents to review all medical/dental information RPSW must develop a Transportation Plan Within five days the worker must develop and implement an Individual Crisis plan in collaboration with the resource parent and the CUA case manager in order to meet the needs of the child Within 30 days of placement, develop and implement a Resource Parent Support Plan specific to each child placed in the home. Within five days the RSPW must develop and implement Respite Plan Review medication logs with Resource parents Review ANSEL CASEY with Resource parent Maintain case record for the resource parents Complete bi-weekly home visit/home safety documentation RPSW is responsible for ensuring respites are planned in advance,, RPSW will work with our Admission Coordinator to schedule respites Planning Attends all required training and pursues other training for professional development. Manages time effectively in order to complete direct service requirements and required documentation. Works collaboratively with each CUA and resources parents to ensure agency policies and procedures are adhered. Monitoring Maintains the appropriate number of training hours as required by local, state and federal regulations. Monitor safety plans Megan's law safety plans if needed Direct Service Coaches the resource parent on home based strategies to address mental health, Physical health needs of the child. To maintain a high continuity of care between Northern's Resource Families and the children they care for, this individual is responsible for coordinating with CUA providers, in order to obtain outstanding records, documentation and case management supports needed for children placed in their homes. The Care Coordinator is expected to work closely with other disciplines to create the best treatment program for the designated cases and must customize service to meet the needs of the individual consumer. Additionally the Care Coordinator assures that the consumer's needs are coordinated, among other programs, agency departments, outside agencies and funding source. Communication Provide supports to families as an adjunct to the clinical treatment. Spend a considerable amount of time assessing & evaluating the Social Determinants of Health (SDOH) related to each family. The Social Determinant of Health scale which we prefer to be utilized to add additional quantitative data is: OneCare Vermont: Self-Sufficiency Outcomes Matrix Be well-versed not only in supports and resources available throughout Philadelphia but also within the local community surrounding the child's school and home.
