The Home Health Clinical Support Specialist is an integral part of the Home Health interdisciplinary team and as such is responsible for the excellent delivery of care through triage calls after hours and on holidays. Must be available to work afterhours, weekends, and holidays.
Responsibilities
* Responsible to receive calls from patients and/or family members and respond appropriately and assess problems focusing on the delivery of high quality, patient-focused, compassionate care.
* Assess patient status and intervene as indicated by the patient's condition and established protocols.
* Responsible for timely and accurate documentation of calls received within the electronic medical records system including the processing of workflow associated with the clinical record.
* Knowledge of basic triage protocols and best practices to guide and address the needs of patients in a crisis situation.
* Responsible for coordinating with the agency on-call nurses to deliver high quality nursing care and schedule nursing assessments as required in a timely manner to meet the needs of the patients and families.
* Responsible to ensure appropriate education regarding all updates/processes in the electronic medical record, relative state and federal regulations, documentation processes and needs, etc. by attending mandatory educational offerings and in-services.
* Facilitates orientation of new personnel as assigned.
* Exhibits exemplary and timely communication skills when assessing or educating patients/caregivers, performing telephone triage, or collaborating with fellow healthcare professionals.
* Serves as a consistent example of dedication to patient advocacy, customer service, integrity, and superlative nursing practice.
* Knowledge of and adherence to all policies, procedures, and LHC Standards of Behavior.
* Responsible to complete all new and ongoing education to maintain competency for this role.
* Maintains productivity standards as assigned.
* Other duties as assigned.
Education and Experience
* Must be a licensed Registered Nurse in good standing with no restrictions in all states of practice. Compact licensure preferred.
* Must have two (2) years' experience as a Registered Nurse with at least one year of direct patient care in a hospice, home health or oncology setting.

Document the rationale clearly for all nurse reviewer decisions using appropriate criteria and refers cases correctly.
Evaluate Nurse Reviewer referrals and Provider reconsideration. Referral Coordinators approve cases that meet criteria or use experience and nursing judgment, when appropriate, to overturn Nurse Reviewer referrals and provider reconsideration. Referral Coordinators review cases that fail these screens to the appropriate Peer Consultant.
Obtain timely specialty-to-specialty review of referred cases, and complete and process the review in a timely manner.
Coordinate in-house MD review by specialty via web and phone with an appropriate Peer Review Consultant.
Consult with Medical Directors on issues related to Peer Consultant review decisions.

Perform video-based nursing assessments and when required, conduct off-site nursing assessments, and participates in candidate scoring session to determine program eligibility.
Accurately and efficiently perform pre-admission, level change and continued placement reviews for applicants/clients related to the Independent Care Waiver (ICWP) program.
Accurately render appropriate level of care decisions based on the information received using the Department of Community Health (DCH) ICWP Policies; and clearly and concisely documents the rationale for all nurse reviewer decisions in accordance with applicable time frame requirements of the contract.
Prepares denial letters to applicants using appropriate business format.
Perform accurate data entry of clinical review and decision information into Prior Authorization (PA) system.
Completes required review activity within contractual deadlines within established time frames.
Responds to provider inquiries through “Contact Us” mechanism.
Refer cases appropriately to the physician consultant for review when indicated.
Conduct re-evaluations and review of appeals utilizing stated criteria as outlined in the ICWP Manual, completes summary information required for appeals and fair hearings in a timely manner, and attends/participates in hearings when required.
Assists providers with resolution of claims edit issues as needed.
Participates in provider education as required.
Identifies and document suspected cases of fraud and abuse to either the Manager Waiver Programs and/or Director of Utilization Review Services.

Accurately and efficiently perform pre-admission, level change and continued placement reviews for applicants/clients related to the Independent Care Waiver (ICWP) program.
Accurately render appropriate level of care decisions based on the information received using the Department of Community Health (DCH) ICWP Policies; and clearly and concisely documents the rationale for all nurse reviewer decisions in accordance with applicable time frame requirements of the contract.
Prepares denial letters to applicants using appropriate business format.
Perform accurate data entry of clinical review and decision information into Prior Authorization (PA) system.

Responds to provider inquiries through “Contact Us” mechanism.
Refer cases appropriately to the physician consultant for review when indicated.
Conduct re-evaluations and review of appeals utilizing stated criteria as outlined in the ICWP Manual, completes summary information required for appeals and fair hearings in a timely manner, and attends/participates in hearings when required.
Assists providers with resolution of claims edit issues as needed.
Participates in provider education as required.
Identifies and document suspected cases of fraud and abuse to either the Manager Waiver Programs and/or Director of Utilization Review Services.
