*Primary Responsibilities:*
* Daily follow-up to monitor all resident health concerns and regular in person follow up at assigned sites.
* Administration of medications and vaccinations as requested by the health care provider.
* Complete nursing assessments, diabetic foot assessments, AIMs and ensure Health Care Appraisals (HCAs) are complete.
* Coordination of care to ensure continuity of care including but not limited to, follow through with verbal and written orders or medication changes, monitoring for drug interactions and side effects, administering injections per physician order, hospital admission and discharge follow up, and follow up on other scheduled appointments related to individual's care.
* Routinely reviews medication counts, and medication orders in accordance with level of health care needs.
* Assists with coordination of resident psychiatric, medical, dental, and optical appointments as needed.
* Is an integral member of the care team and participates in interdisciplinary team meetings when scheduled. Proactively works with an interdisciplinary team to overcome barriers to care such as but not limited to, treatment or medication refusal.
* Performs DMA and medical skills hands-on training when applicable.
* Participate in medical on-call rotation.
* Assists with development and revisions of medical policies and procedures.
* Assists in the destruction of controlled substances as outlined by DEA guidelines.
* Responsible for triaging medical concerns.
* Audits the use of EHR and EMAR programs.
* Educates staff and individuals served regarding infection prevention, physical health, medication management, and medical skills including use of medical equipment.
*Education and Qualifications:*
* Valid LPN license in the state of licensure with a minimum of 2 years' previous experience.
* Maintains own mandatory training and certification requirements.
* Must pass criminal background check
* Must possess a valid Driver's License.
* Proficient in speaking, reading and writing the English language.
* Approved by state, federal and government entities to work within BSLS programs.
* Ability to use office equipment, including BSLS information technology systems.

GENERAL PURPOSE

ESSENTIAL DUTIES OF THE POSITION
Works closely with the Unit Managers, Charge Nurses, Staff Development Coordinators, practitioners, and Unit Staff to ensure proper principles of wound prevention.
Implements treatment plans on residents following appropriate wound care practices.
Assesses skin condition using standardized assessment tools. Obtains physician orders for treatment modalities as determined and implements treatment modalities as assigned.
Ensures reports are updated and entered accurately in medical record, documenting care provided to residents. Reviews previous documentation to assure consistency.
Completes weekly rounds, taking photos and ensuring treatment plans are followed.
Provides the needs of the resident upon recommendation and orders of the attending physician and reports to Registered Nurse (RN), Nursing Supervisor, or physician, the resident's condition and reaction to medication, treatments or significant incidents.
LPN is also responsible for including but not limited to timely documentation, assistance with admissions, discharges, transfers, returns, medication, reporting of untoward events, updating care plans, notifications to RN's, supervisors, physicians, families, etc. LPN will follow established procedures; utilizes resources as needed, e.g. staff development, policies, adjust work assignments in response to unanticipated absences, changes in resident load or nursing needs, or similar situations. Qualifications
Education

Experience
Must have proven supervisory and leadership skills and be able to handle stressful situations.
Three to five years working in geriatric, long-term or home care nursing preferred. Experience with Alzheimer's disease and other memory impairments is a plus.

-Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness.
-Initiates or assists with emergency response measures.
-Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.
-Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.
-Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.
-Ensures patient awareness related to transplant and treatment modality options.
-Performs all other duties as assigned by Supervisor.
PHYSICAL DEMANDS AND WORKING CONDITIONS:

-The position provides direct patient care that regularly involves heavy lifting, moving of patients and assisting with ambulation. Equipment aids and/or coworkers may provide assistance.
