Essential Functions and Responsibilities:

Prepares treatment room(s) for patient by following procedures and protocols (clean table surface and change pillowcase).
Prepares patients for physical therapy treatment by welcoming, comforting, providing and/or assisting patient into physical therapy gym/rooms.
Assists with patient care by applying/removing heat packs; cold packs; assisting patients off of traction table; helping patients onto exercise equipment, monitoring motion; tracking walking time and distance; assists with guidance of prescribed exercises and strengthening techniques.
Educates patients by demonstrating proper use of equipment and exercise routines.
Provides assistance with positioning equipment for therapist access; positioning patient on equipment.
Consulting with patients to learn about their physical condition; Accessing and interpreting initial evaluation and progress note documentation; Providing input to the physical therapist in the modification of treatment plans using a variety of treatment techniques based on age, specific cultural and rehabilitative needs in compliance with the Physical Therapy Practice Act.
Develop rapport and trust with your patients to encourage and motivate them to do their best while undergoing physical therapy.
Exercise independent judgement based on experience and collaboration with a physical therapist to provide individualized effective patient care; Communicate and coordinate with the physical therapist to ensure a consistent level of care; Assisting clients in exercises to minimize injury and promote fitness; Modifying exercises as needed for patients.
Teaches, supervises and counsels the patient and family regarding the recovery process, challenges to expect, home exercises injury prevention, ergonomics, ways to promote physical health and other care needs as appropriate to the patient's condition; Utilizes educational material as appropriate.
Maintains safe and clean working environment by complying with procedures, rules and regulations by assisting with laundry, refilling paper needs in all areas; monitor/clean restrooms; sweep/vacuum floors; remove trash from areas.
Maintains physical therapy supplies inventory by checking stock and refilling needed supplies.
Ensures operation of physical therapy equipment by completing preventive maintenance/cleaning requirements; following manufacturer's instruction.
Adhere to applicable policies and procedures of company, Federal as well as Michigan state regulations.
Provides thorough, timely, professional and consistent patient care documentation through the electronic medical record (EMR); Daily notes per company policies; Documentation demonstrates minimal to no grammatical errors; Documentation clearly indicates medical necessity and clinical progression towards completion of care.

Essential Functions and Responsibilities:

Exercise independent judgement based on experience and collaboration with a physical therapist to provide individualized effective patient care; Communicate and coordinate with the physical therapist to ensure a consistent level of care; Assisting clients in exercises to minimize injury and promote fitness; Modifying exercises as needed for patients.

Maintains safe and clean working environment by complying with procedures, rules and regulations by assisting with laundry, refilling paper needs in all areas; monitor/clean restrooms; sweep/vacuum floors; remove trash from areas.

What You'll Do
Deliver direct physical therapy services in patients' homes under the supervision of a licensed Physical Therapist
