Responsible for the intake and logistics of facilitating outside hospital requests for patient transfer into the University of Virginia Medical Center. MINIMUM REQUIREMENTS

Education: High School Graduate or Equivalent is required. Graduation from a.certified.Paramedic or EMT-Intermediate education program is required. Associate's degree in EMT- Paramedic or a Bachelor's degree in Communications Emergency Services or Health Sciences is preferred.

Experience: 3 years of experience at the Intermediate or Paramedic level with a high-volume career or volunteer EMS system or.3 years of experience within the hospital setting.

Licensure: Must maintain current Virginia EMT-P Virginia EMT-I National. Registry NRP or National Registry NREMT-I certification(s). Must maintain current AHA Health. Care Provider BLS certification and certifications.in ACLS and PALS. Certifications in PHTLS ATLS and/or AMLS preferred.

PHYSICAL DEMANDS

Sitting for prolonged periods, frequently utilizing a computer and phone system, typing, and multi-tasking; Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description:

Medical Coordinators are critical to managing the discharge of homeless patients from hospitals and overseeing their discharge care plan. This position will help the Recuperative Care team provide high quality medical follow up coordination and social care.

This position is eligible for a $1,500 sign on bonus ($500 at 30 days of employment, $500 at 3 months of employment, $500 at 6 months of employment).

Tasks/Responsibilities

  • Performing initial intake to ensure proper client discharge from hospital with all appropriate needs for the continuation of care, entering and documenting all findings.
  • Conducting morning, afternoon, and evening checks for all assigned clients to observe well being, ensuring there is no change in medical conditions since hospital discharge.
  • Linking clients to medical services when necessary (insurance, PCP, specialists, referrals)
  • Coordinating home health visits and follow-up MD appointments.
  • Educating clients on illnesses, medications, preventative measures, and overall general health
  • Entering client information into KIPU to maintain client records
  • Exiting clients and preparing client reports for billing
  • Performing administrative tasks such as answering phones and filing paperwork
  • Collaborating with service providers to bring new resources for programs
  • Transporting clients to and from medical appointments if needed to advocate for client needs.

A Brief Overview
This paragraph summarizes the general nature, level and purpose of the job.

Under direction of a physician and/or resident physician, the focus of the job is to enhance the efficiency and improve the throughput of patients while maintaining and improving patient satisfaction. The FLOW COORDINATOR will accomplish this in the following manner:
1. Record a physician-directed patient history of present illness (HPI), past medical history (PMH), family and/or social history and physical examination (PE), including pertinent positives and negatives.
2. Record procedures and the results of laboratory tests and radiographic images.
3. Improve the efficiency of patient care by assisting the physician with routine matters.
4. Serve as an ambassador/liaison between the physician, the patient and the ED staff.

What you will do

At the physician's discretion, the FLOW COORDINATOR shall accompany the supervising physician during a patient's history taking and examination. The physician will personally elicit the patient's HPI, which will be recorded by the FLOW COORDINATOR. The FLOW COORDINATOR will record onto the ED electronic medical record (EMR) the details of the patient's visit as directed by the physician. The physician may elect to personally document any part of any patient's record, or to complete it in its entirety. After the FLOW COORDINATOR and the physician leave a patient's room, the physician will review the documentation entries recorded by the FLOW COORDINATOR during the encounter, and will make any and all necessary amendments to the patient's medical record so that it is accurate and complete. The physician retains exclusive responsibility for the documentation of all patient care information recorded by the FLOW COORDINATOR during each patient encounter and at all other times. The FLOW COORDINATOR can assist the physician with researching and recording a patient's PMH by assembling and reviewing nurse's notes, EMS reports, referral notes from other health care facilities, and hospital records (e.g., prior admission notes, discharge summaries, operative/procedure notes and ED records). At end of the physician documentation, beneath the signature lines, the FLOW COORDINATOR will electronically Esign the record in addition to the physician's signature. Secondary Duties (Occasional)
1. The FLOW COORDINATOR may advise the physician when test results are available, when a patient is ready for disposition (i.e., all tests are resulted), when a critical value has been flagged/reported, when a patient is ready for a procedure (i.e., the appropriate cart is in the room and ready for use), and when there has been a delay in completion of ordered tests (e.g., a urine has not been obtained, blood has not yet been drawn, etc.).
2. The FLOW COORDINATOR may perform a "patient ambassador" function on behalf of the physician, checking to see if patients are comfortable, have had adequate pain relief, etc. The FLOW COORDINATOR may obtain blankets and pillows for patients, as well as nourishment (following approval by the physician or the patient's nurse). The FLOW COORDINATOR will inform the physician if the patient has, pain, nausea or any other condition or request that requires attention.
3. The FLOW COORDINATOR may serve as a liaison between the physician and nurses, clinical associates and Health Unit Coordinators (HUCs) by forwarding messages between the parties (e.g., a request by the physician to prepare a patient for suturing). The FLOW COORDINATOR may not communicate messages regarding direct patient care (e.g., medication or IV orders).
4. The FLOW COORDINATOR may also perform the above functions in collaboration or in support of the resident working with the physician.
5. The physician will personally enter all orders for patient care utilizing EPIC. FLOW COORDINATORs will not enter such orders.
6. All medication and nursing orders for patient care are to be personally communicated to a nurse directly by the physician, and not by a FLOW COORDINATOR.
7. The FLOW COORDINATOR may, at the Physician's direction, assist with the discharge process by selecting and preparing the discharge instruction and follow-up FLOW COORDINATORP or Specialist Referral in EPIC. The FLOW COORDINATOR may not prepare any prescriptions.
8. The physician will sign each patient's medical record when it is complete and after he/she has reviewed and, if necessary, amended the document for accuracy.

High School Diploma and/or GED equivalent



Experience Qualifications


Three (3) years of overall experience. Preferred previous experience as medical scribe in an Emergency Department or EMT/Paramedics or experience in a healthcare educational program.



Required Knowledge, Skills and Abilities


Successfully complete Stanford's Physician Coordinator Training and Orientation Program. Attend any required Stanford new-employee orientation sessions, complete all HIPAA/Compliance training and background/exclusion checks required, receive any vaccinations required, and sign all applicable confidentiality agreements and forms. Must possess significant medical terminology knowledge and medical thought process background. Demonstrated business communication skills, including the ability to write and type clearly and legibly (with solid knowledge of grammar and spelling). Ability to demonstrate computer knowledge and understanding of Microsoft Office applications (Word, Excel, PowerPoint, Outlook). Have the ability to interact pleasantly, cooperatively, and respectfully with patients, families, physicians and Emergency Department staff. Possess the ability to work effectively and maintain poise under stressful conditions. Ability to demonstrate customer service skills. Knowledge and understanding of an electronic medical record system and/or EPIC. Upper level knowledge of medical terminology.
