PRINCIPAL RESPONSIBILITIES AND DUTIES
CLINIC OPERATIONS:
Manages the operations of the clinic, including costs, processes, staffing, and quality standards.
Provides leadership, coaching, and development plans for all direct reports.
Partners with internal Human Resources, Quality, and Technical Services departments.
Collaborates with or functions as the Home Therapies Program Manager to oversee the facility's Home Therapies Program.
Maintains integrity of medical and operations records and complies with all data collections and auditing activities.
Accountable for completion of the Annual Standing Order Review and Internal Classification of Disease (ICD) coding.
Manages clinic financials including efficient utilization of supplies or equipment and regular profits and loss review.
Responsible for all required network reporting and on-site state or federal surveys.
PATIENT CARE:
Coordinates all aspects of patient care with the appropriate staff members, from admission through discharge of the patient.
Ensure education of the patient regarding quality measures, transplant options, modality awareness, and access care.
Acts as a resource for the patient and family to address concerns and questions.
Accountable for timely completion of patient care assessments and care plans.
Manages timely patient schedules to ensure facility efficiency and develops action plans for missed treatments.
Plans, coordinates, and validates patient eligibility for treatment.
Reports changes in patient status regarding any clinical, insurance, or travel and transportation issues.
STAFF:
Participates in the selection and hiring of new team members and ensures completion of new hiring orientation and mandatory trainings.
Responsible for all patient care employees completing appropriate training courses and that licensures/certifications are current.
Provides support for all clinical staff members at regular intervals and encourages professional growth.
Maintains current knowledge regarding company benefits, policies, procedures, and processes.
Completes employee evaluations and establishes annual goals.
Documents any disciplinary actions and confers with the clinic Director of Operations and Human Resources when needed.
Manages staff scheduling and payroll.
PHYSICIANS:
Facilitates the application process for physician privileges and compliance with Fresenius Medical Care Medical Staff By-Laws.
Responsible for strong physician relationships and ensures regular and effective communication.
Participates in Governing Body, an interdisciplinary team for each region including MSWs, Dietitians, MDs, DOs etc. that governs policies.
EDUCATION AND REQUIRED CREDENTIALS:
Bachelor's Degree or an equivalent combination of education and experience.
Graduate of an accredited School of Nursing (RN).
Current appropriate state licensure.
EXPERIENCE AND SKILLS:
Required:
6+ years business operations experience in a healthcare facility.
12 months experience in clinical nursing.
6 months chronic or acute dialysis nursing experience.
Successfully pass the Ishihara Color Blind Test.
Preferred but not required:
3+ years supervisory or project/program management experience.
Med/surg or ICU/CCU experience.
PHYSICAL DEMANDS AND WORKING CONDITIONS:
Day-to-day work includes desk and personal computer work and interaction with patients, facility staff, and physicians
Position may require travel between assigned facilities and various locations within the community and/or travel to regional, business unit, and corporate meetings may be required.

The RN Case Manager will be responsible for the coordination of care and drive patient progression to establish a discharge plan. This includes functioning as an interdisciplinary team member, creating, implementing, and developing all aspects of discharge planning to establish appropriate, timely, and effective transitions throughout the care continuum.
The Care Management Model:
Our Care Management Triad Team Model is a collaboration between the following:
RN Case Manager - Manages patient care, drives patient progression, and establishes a discharge plan.
Social Worker - Resolves psycho-social barriers and supports discharge needs.
Utilization Management - Reviews patient status for appropriateness and anticipated payer coverage.
CARE MANAGEMENT:
Identify patients who have post-acute care, placement, and complex discharge planning needs based on a comprehensive assessment that includes physical, as well as psycho-social factors/needs.
Anticipate, initiate, and establish a discharge plan for patients with post-acute care needs, collaborating with the physician, nurse, and other health care providers, the patient, their family/primary caregiver(s), third-party payers, and employer following established clinical guidelines, standards, and pathways.
Review the admission assessment and collaborate with the primary nurse and other health care providers to ensure a multidisciplinary plan of care is in place to meet identified patient care needs and desired outcomes.
Identify system issues that serve as barriers to care. Participates in the development and implementation of strategies to remove barriers and facilitate patient progression.

Education & Requirements
An active RN license in DE or Compact State is required.
BSN required.
RNs with at least 2 years of medical-surgical nursing experience in an acute care hospital are encouraged to apply.
A minimum of 1 year of Care Management experience is strongly preferred.
BLS preferred.
Case Management Certification (CMC) is required within 18 months of eligibility.
Hourly Pay Range: $41.28 - $66.05This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. Christiana Care Hospital is hiring a RN Clinical Documentation Specialist for improving the overall quality and completeness of clinical documentation (CDI) within the inpatient medical record.
Primary Function:
Responsible to ensure that health care services are administered with quality, cost efficiency, and within compliance, along with improving and ensuring the overall quality, cost efficiency and completeness of documentation within the inpatient medical record. Ensure accuracy of documented diagnosis and support clinical indicators. Facilitate modifications to clinical documentation through extensive concurrent interaction with Physician Advisor, physicians, coding staff and other members of the healthcare team to support the appropriate reimbursement and clinical severity are captured compliantly for the level of service rendered to all patients.
Scope, Purpose, And Frequency of Contacts:
Daily contact with Clinical Documentation Manager, HIMS/coding staff, Physician Advisor, and intradepartmental personnel. Frequent contact with physicians, physician's office staff, clinical, and ancillary departmental staff. Frequent contact with HIM Coding Management
Principal Duties & Responsibilities:
Concurrent review of inpatient medical records throughout the patient's hospitalization. Analyzes clinical status of patient, current treatment, past medical history and identifies potential gaps in physician documentation.
Communicate with physicians or other providers to validate diagnoses, clinical indicators and appropriate prompts for documentation, if necessary, either verbally or written.
Demonstrates proficient knowledge of HIMS standards of coding and applies to ongoing evaluation of the medical record documentation.
Educates physicians and other key healthcare providers regarding best practice clinical documentation and the need for accurate and complete documentation in the health record.
Reviews and clarify clinical issues in the health record with the coding professionals that would support an accurate DRG assignment, severity of illness, and/or risk of mortality.
Monitors patients progress consults with primary physician or designee and other members of the care delivery team at agreed-upon intervals, or more frequently, as needed.
Participates in performance improvement activities regarding documentation improvement
Maintains confidentiality of patients/members and staff information.
Attends coding/financial/CDI educational programs and regulatory educational programs and updates as necessary to maintain a proficient knowledge base.
Reviews all PSI encounters for completeness and accuracy. Review outcome with Physician Advisors, and Medical Directors, as needed.
Attends daily huddles with HIMS to address any clinical concerns
Computes a working DRG to identify the anticipated length of stay. Shared with the care team for LOS goals.
Uses Vizient data to identify documentation opportunities and targeted physician education.
Performs assigned work safely, adhering to established departmental safety rules and practices; reports to manager, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
Performs other related duties as required
Education and Experience Requirements:
DE RN licensed or Compact State.
Minimum of 3 years recent experience as a Registered Nurse in acute care, adult care setting preferred.
RN experience in Cardiac, Critical Care, Med-Surg, and Oncology experience highly desirable.
Clinical Documentation, Case Management, Performance Improvement, or Inpatient Coding experience are highly desirable.
Certified Clinical Documentation Specialist (CCDS) is required in a year after eligibility.
Knowledge, Skill, And Ability:
Extensive knowledge of medical terminology, anatomy, physiology, pharmacology, and disease processes.
Knowledge of nursing principles, practices, and processes.
Knowledge of ICD-10 nomenclature, UHDDS and general coding principles. Knowledge of CMS, JCAHO and external regulatory and quality requirements.
Knowledge and ability to comply with the Health Insurance Portability and Accountability Act of 1996 (HIPAA)
Knowledge of Performance Improvement Principles.

Overview:Performs computerized axial tomography procedures in accordance with the established professional and departmental standards and procedures.Responsibilities:Reviews consultation request, checking for special instructions and consults with the Radiologist as necessary. Obtains patient history for correlation with scan. Ensures that billing codes match the exam that was done. Begin and ends procedures and document scans all relevant paperwork. Completely follows through with urgent add-on requests. Pleasantly greets patients, confirming their identification and introducing yourself. This individual is a clinically competent, certified advanced practitioner, who serves in the comprehensive care of the medical oncology and hematology patient throughout the continuum of care. Under the scope of an advanced practice clinician and approve privileges within Beebe Healthcare, the individual is expected to perform comprehensive patient assessments, follow-up care, diagnosis, side effect and infusion reaction management, survivorship care services, and patient education/counseling in collaboration with the medical oncology physicians. Follow(s) established advanced practice standards, procedures, and guidelines/protocols to give direction to patients, families, nursing and other staff when appropriate.
Responsibilities:
Performs comprehensive assessments for new and established oncology and hematology patients.
Management of complex oncology and hematology patients to include side effect management, diagnosis, and treatment.
Demonstrates critical thinking, prioritization of duties, and time management skills based on patient assessment and interpretation of test results.
Serves as the primary clinical support for the infusion center when assigned. Duties include management of infusion reactions, patient care needs presented by nursing and pharmacy, coverage of triage issues and assessments, and critical lab and radiology results.
Formulate and implement survivorship care plans for all curable cancer patients.
Identify and initiate referrals to palliative care.
Collaborate with the palliative care team for management of patient's ongoing care.
Communicates positively and effectively with patients, families, physicians and the multidisciplinary oncology team.
Coordinates patient care with PCP and specialty providers when appropriate.
Provides detailed education and counseling to patients and families regarding diagnosis, goals of therapy, expectations, and treatment related side effects and management.
Develop a comprehensive plan of care in collaboration with the attending oncologist to include selection of appropriate pharmacologic agents, referrals, and end of life care.
Inpatient oncology and hematology acute care coverage to include rapid assessment and treatment of acutely ill oncology/hematology patients.
Collaborate with medical oncologist and inpatient disciplines to determine the most appropriate level of care.
Assist in the admission and discharge of patients receiving inpatient chemotherapy in collaboration with the attending oncology physician.
Administration of intrathecal chemotherapy in collaboration with interventional radiology.
Provide after-hours call coverage as part of a rotating schedule to include phone triage, clinical decision-making, and urgent inpatient/outpatient concerns in conjunction with on-call oncologist.

Education/Experience
Graduate from an accredited Physician Assistant or Nurse Practitioner program.
