We are seeking a Bilingual Medical Case Manager fluent in Spanish and English to join our community-based organization! Identifies and advocates for clients engaged in care within one or more of Philadelphia FIGHT's Community Health Centers sites related to shelter/housing, utility assistance, food assistance, clothing banks, medication assistance programs, substance use and/or behavioral services, etc.
Assesses un- and under-insured clients' eligibility for Pennsylvania public benefits (e.g., Medicaid, MAWD, CHIP, SPBP, etc.).
Maintain regular contact with clients, guided by program requirements, client needs, and care goals.
Conduct biopsychosocial assessments and develop individualized, goal-oriented, and strengths-based service care plans at intake; reviews and updates quarterly in collaboration with the care team.
Provides benefits assistance and enrollment for clients via COMPASS.

Provides trauma-informed therapeutic support, including crisis interventions in collaboration with behavioral health and medical providers.

Identifies and advocates for population-specific support services for those who need gender-affirming care, immigrants, clients who are blind, deaf/hard of hearing, have cognitive abilities, etc.
Supports clients at medical appointments or during home visits as needed to ensure continuity of care.
Participates in on-call rotation during regular office hours.
Serves as a mandated reporter for minor patients in cases where harm is perceived or evident.
Supports department operations by providing cross-coverage for Health Access Services staff (e.g., Medical Case Managers, Benefits Access Navigators, Assistant Case Managers, receptionists, etc.).
Abides by all compliance standards and operational activities, including but not limited to, completing training, attending all required staff meetings, supervision meetings, participating in events, etc.
Assists with special projects, organizational events, staff coverage, etc., when needed.

Deliver culturally responsive education on HIV/STI prevention, PrEP, medication adherence, and harm reduction
Respond to PrEP inquiries with medically accurate, affirming, and sex-positive information
Patient Engagement & Navigation
Conduct initial PrEP consultations and assess patient needs and eligibility
Maintain consistent contact via phone, text, patient portals, and in-person visits
Identify and help address barriers to PrEP access, including insurance, medication access, and social determinants of health
Educate patients on starting, stopping, and re-initiating PrEP, and how to stay connected to care
Case Management & Retention
Support clinical teams by ensuring:
Quarterly provider visits and required lab work are completed
Medication refills and assistance program renewals are submitted on time
Patients receive appointment reminders and follow-up support
Conduct Rapid HIV testing as part of PrEP care
Link patients to PrEP following STI diagnoses or Post-Exposure Prophylaxis (PEP)
Coordinate PrEP medication distribution and manage prescription tracking
Connect patients to additional services such as mental health care, substance use treatment, housing support, or HIV care when needed
Data, Reporting & Program Growth
Document all patient interactions accurately in the electronic medical record within 48 hours
Collaborate with leadership to analyze PrEP data, identify trends, and improve retention strategies
Support reporting on adherence, retention, and key performance indicators to strengthen program impact

Bachelor's degree in healthcare, public health, social work, or related field
1-2 years of experience in HIV prevention, case management, or patient counseling
Knowledge of PrEP and combination HIV prevention strategies
Strong organizational, communication, and interpersonal skills
Comfort working independently and collaboratively in a fast-paced clinical environment
Proficiency with electronic health records and Microsoft Word, Teams, and Excel
A demonstrated commitment to LGBTQ communities and the mission of Mazzoni Center

Medical Case Manager

Assess the needs of individuals living with HIV and assist with linking them to necessary services/resources related to their medical status and psychosocial needs. Provide necessary monitoring, follow-up, and coordination of services.
Communication SkillsGood oral and written communication skills are required for this position. The person in this position must be able to communicate effectively with the clients, staff, and others; and contribute to a positive environment. Key ResponsibilitiesClient Care
Ryan White funded MCMs must maintain a caseload of 40-45 clients
Provide a minimum of 400 units of service per month (1 unit = 15 minutes).
Assess the biopsychosocial needs of clients based on funding and program requirements.
Maintain minimum contact with clients based on program, client need, and service care plan.
Identify appropriate resources to address client needs and assist client with linkage to those resources.
Coordinate and monitor ongoing services.
Assist client with identifying, building and maintaining external support systems
Documentation
Maintain accurate written documentation of all client contacts (using DAP format) in CareWare and/or athena Net. Service units/visit encounters and case notes should be input within 24 hours of contact.
Maintain accurate and orderly client files with hard copy of client-related forms, applications, correspondence, etc.
Protect client information and records at all times in compliance with HIPAA and agency Confidentiality Policy.
Other
Participate in weekly supervision with Case Management Services Managers, Care Coordination Manager, Assistant Director of Care Services, or Director of Care Services.
Participate in agency meetings, staff in-services, group supervision, and external trainings as required.
Assist with food bank deliveries.
Perform other program related duties as assigned by Assistant Director of Care Services or Director of Care Services.
Qualifications and Skills
Bachelor's Degree in Social Services (Psychology, Social Work, Nursing, Public Health).
Knowledge of HIV/AIDS and community resources.
LGBTQ cultural competency.
Competency in working with diverse populations.
Computer literate.
Bi-lingual preferred
Physical Requirements:

The ability to efficiently move around a city location on a regular basis.
The ability to communicate using written and verbal or signed language.
The ability to lift objects greater than 35 pounds.
The ability to use a computer for up to 6 hours per day.
The ability to work in an environment with a moderate noise level, in an open cubicle setting.
Must have all necessary vaccinations pursuant to Mazzoni Center policies.
