Qualification Requirements All Temporary/Relief Respiratory Care Practitioners assigned to Correctional Facility must have the minimum experience outlined below: 1. Twelve (12) continuous months of experience within the last three (3) years performing services similar in scope to those defined herein in a public or private institution.
2. Possess and maintain while performing services for the Correctional Facility a current and valid license issued by the Respiratory Care Board to practice respiratory care in the State of California and credential issued by the National Board of Respiratory Care.
3. Graduated from an approved respiratory therapy program accredited by the Committee on Accreditation for Respiratory Care or its predecessor, the Joint Review Committee for Respiratory Therapy Education.
4. Each Temporary/Relief Respiratory Care Practitioner shall be in compliance with 8 CCR 5199 standards for both current N95 and Powered Air Purifying Respirator (PAPR) medical clearance and annual general respiratory protection training which is to include a request to repeat PLHCP clearance if they develop 1) marked immunocomprompromise or pregnancy which might require great respiratory protection than N95s provide or 2) a health condition which might limit N95 or PAPR use and completion of the annual 8 CCR 5199 Appendix G questionnaire.
5. Temporary/Relief Respiratory Care Practitioner providing services to the Correctional Facility must be proficient in the English language and be able to communicate effectively with the Correctional Facility. All Temporary/Relief Respiratory Care Practitioner must be able to speak, understand oral and written communication and write effectively in the English language. Any Temporary/Relief Respiratory Care Practitioner who fails to meet the minimum qualifications shall not be permitted to perform services.
Service Delivery Requirements
Under the direction of the Institution CEO/CME or designee, the Temporary/Relief Respiratory Care
Practitioner shall provide services as permitted within the scope of practice for Respiratory Care Practitioner services in accordance with Institution/Facility policies and procedures. The Respiratory Care Practitioner must be able to work in conditions that require all of the following essential functions:
a. Perform care consistent with generally accepted evidence based practice;
b. Set up equipment and provide instructions for use of equipment for each patient/youth per established policy and procedure;
c. Perform complete medical evaluations of patient either during regularly scheduled clinic hours or upon request for consult by either the CEO/CME or designee and make appropriate recommendations for treatment to the referring physician, CEO/CME or designee;
d. Maintain all charting on a daily basis. Any chart remaining delinquent for more than ten (10) days could result in termination of this Temporary/Relief Respiratory Care Practitioner;
e. Aid in the development and updating of policy and procedures related to respiratory therapy;
f. Provide periodic assessment of equipment needs and recommendations;

Provides respiratory care to patients in alternate sites in accordance with AdaptHealth's policies and procedures. Respiratory care will be preventative, rehabilitative, and palliative in nature. The RT will utilize all the resources available within the agency and community to accomplish care objectives. Essential Functions and Job Responsibilities:
Utilizes various sources of information to attain greater competence about his or her position, including attending educational events (including attending optional in-services) and asking questions.
Utilizes acquired knowledge to increase his or her competencies.
Consistently demonstrates ability to adequately complete all documentation and charting procedures in compliance with company policy and procedures.
Maintains complete and accurate patient files by updating all documents per company policy and procedures.

Retain knowledge of and consistently adhere to procedures for the use of Personal Protective Equipment (PPE), infection control and hazardous materials handling.
Works to promote AdaptHealth by new program development, operational backup, personal visits, coordination of educational activities, etc.
Assume on-call responsibilities during non-business hours in accordance with company policy.
Uses clinical expertise in evaluating vent patients records once a ventilator set up has been completed by the branch Respiratory Therapist.
Ensures accuracy of prescriptions and plan of care was followed and documented. Also reviews delivery tickets, home inspection, ventilator check, and patient equipment competencies are complete and documented.
Maintains proficient knowledge of ventilator patients including compliance software, new technology, units, and supplies supported by Adapthealth.
Ability to demonstrate and instruct on use of vent units and supplies. Ability to make decisions for patients based on compliance data and assessment.
Communicates with team, physicians and referral sources and other patient agencies ensuring prompt attention to patient care issues.
Maintains working knowledge of Medicare/Medicaid and other third-party payer guidelines related to ventilation.
Electronically documents patient care activity, intervention provided and all communication regarding the patient. Documentation is accurate, complete and follows company standards.
Appropriate steps taken to ensure recommendations and orders sent are acknowledged and followed up in a timely manner.
Responsible for accuracy, clarity, and timeliness of verbal and written communications as it relates to role.
Responsible for documentation that supports data collection to track and trend outcomes.
Assists in establishing clinical documentation when needed for third party reimbursement or justification.
Uses knowledge in working with referral sources to educate about best practice standards.

Education, Training and Licenses Required:

Experience Required:

Job Duties:
Monitor and adjust ventilator settings for long-term patients.
Perform weaning trials and assist with liberation from mechanical ventilation.
Troubleshoot ventilator alarms and complications.
Administer oxygen therapy (high-flow, CPAP/BiPAP, traditional oxygen delivery).
Perform tracheostomy care, suctioning, and airway clearance techniques.
Conduct ABG (arterial blood gas) analysis, pulmonary function tests (PFTs), and bedside spirometry.
Monitor pulse oximetry, capnography, and other respiratory parameters.
Develop and implement respiratory care plans in collaboration with physicians.
Educate patients and families on chronic respiratory conditions, inhaler techniques, and home care needs.
Respond to rapid responses and codes (e.g., Code Blue, respiratory distress).
Provide advanced airway management as needed.

Part-time Description
The Respiratory Care Practitioner (aka Respiratory Therapist or RT) with Montefino does more than provide essential direct medical care to their patients. Our RTs have smaller patient caseloads, in some cases working with a single patient at the assigned facility on a given shift. This allows RTs to apply their skills in more direct and intentional ways to contribute toward positive patient outcomes. RTs build meaningful relationships with the patient's loved ones, their care team, and most importantly, the patient to foster positive impacts on their daily lives.
Montefino RTs follow established standards and procedures, provide supervision, care, support, and training to individuals with Developmental Disabilities. The position provides critical assistance to individuals with ADL's (activities of daily living). Effective RTs are organized, analyze situations accurately, take effective action, diligently follow directions, and communicate clearly and effectively. They possess knowledge of the industry/field, applicable state/federal laws (e.g. Title 17 & 22), and associated organizations and agencies (e.g. Regional Centers, Dept. of Social Services, Dept. of Development Services, State Council on Developmental Services, etc.). Successful candidates are enthusiastic in their efforts to expand upon their technical knowledge (e.g. dual Diagnosis, Individual Program Plans, emergency protocols, etc.), advocate for their patients, and are consummate professionals.
RTs maintain a safe, clean, and therapeutic environment that ensures respect, dignity, protects privacy, rights, and physical/emotional well-being to all residents. The RT possesses skills in organization, time management and analytical problem solving. Clear and effective written and verbal communication, interpersonal skills, multi-tasking, critical thinking and active listening skills and essential to success.
Essential Duties
Include but are not limited to:
Set up and operate devices such as mechanical ventilators, therapeutic gas administration apparatus, environmental control systems, and aerosol generators, following specified parameters of treatment.
Provide emergency care, including artificial respiration, external cardiac massage and assistance with cardiopulmonary resuscitation.
Determine requirements for treatment, such as type, method and duration of therapy, precautions to be taken, and medication and dosages, compatible with physicians' orders.
Monitor patient's physiological responses to therapy, such as vital signs, arterial blood gases, and blood chemistry changes, and consult with physician if adverse reactions occur.
Read prescription, measure arterial blood gases, and review patient information to assess patient condition.
Work as part of a team of physicians, nurses, and other health care professionals to manage patient care.
Enforce safety rules and ensure careful adherence to physicians' orders.
Maintain charts that contain patients' pertinent identification and therapy information.
Inspect, clean, test and maintain respiratory therapy equipment to ensure equipment is functioning safely and efficiently, ordering repairs when necessary.
Educate residents and their families about their conditions and teach appropriate disease management techniques, such as breathing exercises and the use of medications and respiratory equipment.
Explain treatment procedures to patients to gain cooperation and allay fears.
Perform bronchopulmonary drainage and assist or instruct patients in performance of breathing exercises.
Demonstrate respiratory care procedures to trainees and other health care personnel.
Make emergency visits to resolve equipment problems.
Provide supervision and guidance for residents during skills building activities.
Develop and implement individual treatment plans for residents.
Assist with daily living activities, including preparing meals and assisting with personal hygiene.
Develop a rapport with residents and cultivate a safe and supportive relationship.
Update resident case notes on a daily basis.
Ensure an appropriate staff-to-client ratio at all times.

Requirements
High School Diploma, GED, or equivalent.
Possess (and maintain) a current Respiratory License/Credentials from an accredited body. Preferred types include Registered Respiratory Therapist (RRT); Certified Respiratory Therapist (CRT); CA Respiratory Care Practitioner License (RCP)
1+ years of experience as a Respiratory Therapist or similar role is desired, but not required.
Possess a current driver license with a clean driving record and the ability to operate a motor vehicle safely.
Possess a service-oriented mindset with genuine passion to provide care for others.
Possess knowledge of the field and best practices. This includes, but is not limited to, concepts such as: Methods of oxygen delivery and possible side effects; Laws regulating the field/patient populations and regulatory entities; Proper sanitation and other care procedures, etc.

Non-exempt employees will be paid overtime in accordance with state and federal overtime requirements. One and one-half times (1.5) the regular rate of pay for all hours worked in excess of eight (8) hours in any workday; One and one-half times (1.5) the regular rate of pay for all hours worked in excess of 40 hours in the same workweek.

Under physician supervision, in accordance with policies and procedures approved by the Medical Director, is expected to assume primary clinical responsibility for all respiratory care modalities and to exercise considerable independent clinical judgment in the respiratory care of patients. Must be capable of serving as a technical resource person to the physician regarding current practices in respiratory care and to the hospital staff regarding effective and safe methods for administering respiratory care. Works under the general supervision of an administrative supervisor or the direction of a technical lead to deliver respiratory care services to patients in all areas of the hospital. Must be able to adapt to the particular area of duty assignment and to perform special duties required by the assignment. Standards of performance for the area to which the individual is assigned must be maintained.
Special duties may include provision of basic and advanced respiratory care to neonatal and pediatric patients in the intensive care unit and during transport, which frequently requires making rapid decisions and assisting physicians to respond to status changes in neonatal and pediatric patients.
Special duties may include performance of a variety of diagnostic tests of the cardiopulmonary system and to determine the effect of disease and treatment on that system.
Essential Responsibilities:
* Selects Reviews, Obtains and Interprets Data: Reads and evaluates physicians orders, clarifying as needed. Accurately transcribes physician orders to electronic charting system or departmental patient master. Reviews patient information including history, lab data, radiologic studies, progress notes, respiratory and cardiovascular monitoring data and performs assessment/data collection in systematic manner.
* Systematically assesses patients overall cardiopulmonary status by inspection, palpation, percussion, auscultation, and patient interview.
* Interprets data to formulate goal-directed plan of care and determines requirements for treatment such as type and duration of therapy, precautions to be observed, equipment required, and medication and dosages.
* Determines modifications to physician orders, which optimize patient, care and makes recommendations to physician. Recommends procedures to obtain additional data including radiographs, bronchoscopy, Gram stain and C&S, spirometry, blood gas analysis, lung mechanics, work of breathing, EKG, noninvasive monitoring.
* Performs Diagnostic Modalities: Performs procedures and interprets results: ABG sampling and analysis, co-oximetry, spirometry, pressure-volume and flow-volume loops, cuff pressure, sputum induction. May perform EKGs. May place Holter monitor. May perform pulmonary function, and sleep diagnostic tests.
* May perform Exercise Stress Tests. May perform indirect calorimetry.
* Selects Assembles and Checks Equipment for Proper Function, Operation and Cleanliness: Selects, assembles and checks equipment for cleanliness, proper function and safe operation. Ensures proper function and ready availability of respiratory related life support equipment. Troubleshoots and performs minor repairs of equipment. May perform Quality control procedures for blood analysis devices.
* May perform Quality control, calibrate, maintain and troubleshoot pulmonary function diagnostic equipment.
* Participates in routine departmental programs of cleaning, disinfecting and sterilizing of respiratory care and other related equipment.
* Participates in routine departmental preventive maintenance program and performs basic repairs.

* Monitors patients physiological responses to therapy, recognizes patient adverse reaction and stops or modifies or recommends modification to procedure based on patient response.
* Documents all required information in the medical record according to departmental policy and procedure. Initiates pressure-cycled/volume-cycled/high frequency ventilation. Weans/modifies mechanical ventilation according to assessment and physician orders.
* Assists with or performs extubation.
* Conducts patient education and disease management. Performs cardiopulmonary resuscitation according to resuscitation guidelines. May assist with or perform intubation. May assist the physician when performing bronchoscopy, thoracentesis, tracheostomy, cardioversion, and intubation.
* May participate in land/air critical care transport. May assist with high risk deliveries. May assist with surfactant replacement in neonates. May participate in pulmonary rehabilitation, home care. May assist with discharge planning. May assist with community outreach. May initiate and implement therapist driven protocols.
* Supervision and Administration: May assign, organize and coordinate the work of assigned Respiratory Care Practitioners. May demonstrate respiratory care procedures to trainees and other health care personnel. May train assigned practitioners and complete assigned practitioner competency reviews.
* Contributes to departmental quality improvement activities. May schedule staff and perform staffing functions.
* Assesses priorities for delivery of patient care and redistributes resources as necessary throughout shift. Protects data from intentional or unintentional destruction, modification, or disclosure.
* Maintains a current knowledge base of departmental information management systems, including collection, maintenance, and dissemination of information, as well as appropriate audit trails.
* May develop policies and procedures, both clinical and administrative. May serve as Timekeeper for TIME system.
