Program Summary:
Since 2007, Trans:Thrive has provided comprehensive medical and social services for the trans/GNC community of San Francisco. More than a drop-in center, Trans:Thrive is a safe space and “one-stop shop” where community members access case management, primary care, dental care, behavioral health services, gender-affirming care, substance use treatment, support groups, hot meals, and community events.
Role Summary:
The Receptionist is the welcoming face of Trans:Thrive and first point of contact for clients and visitors. This position plays a key role in ensuring a positive, affirming experience for all who enter the space. The receptionist manages client check-in and registration, performs new patient intakes, confirms insurance eligibility, and navigates clients to clinical, behavioral health, and supportive services. This position requires excellent communication skills, attention to detail, and the ability to thrive in a fast-paced environment. This position works closely with the Wellness Clinic, Behavioral Health, and Eligibility teams.
Essential Duties:
Client Reception & Navigation:
Greet all clients and visitors warmly, assess needs, and connect them to appropriate staff or services.
Answer incoming calls, return voicemails promptly, and respond to general inquiries about programs and services.
Maintain a safe, welcoming, and affirming reception area; ensure literature, forms, and signage are up to date and space is clean and organized. Inform manager and Managing Director of any janitorial needs.
Intake & Registration:
Complete new patient intakes for Trans:Thrive programs, ensuring required consent-to-treat and sliding scale paperwork are signed.
Register new patients in Epic, create charts when necessary, and accurately document client information.
Coordinate with clinic staff to ensure streamlined intake processes between Trans:Thrive and the Wellness Clinic.
Eligibility & Benefits:
Receive basic training in Medi-Cal eligibility and confirm client coverage prior to services.
Update Epic with current insurance benefits and guarantor information to prevent billing errors.
Collaborate with the Eligibility Team to ensure optimal coverage and enrollment for clients.
Administrative & Program Support:
Schedule appointments, send reminders, and check in clients for behavioral health, case management, and other services.
Perform clerical tasks, such as faxing, scanning, copying, etc., to support program staff.
Coordinate transportation for clients as needed.
Collaborate with the Safety Intervention Specialist to maintain client and staff safety.
Team & Agency Participation:
Attend staff meetings, trainings, and agency events as required.
Support program data collection and reporting needs as assigned.
Perform other duties as needed to ensure smooth daily operations.
Minimum Qualifications:
High school diploma or equivalent work experience.
Experience in customer service, office administration, or reception.
Comfortable and culturally competent in working with LGBTQ communities
Ability to work effectively with diverse populations, including LGBTQ communities, immigrants/refugees, people experiencing homelessness, and individuals with behavioral health and substance use challenges.
Strong written and verbal communication skills.
Detail-oriented with excellent follow-through and reliability.

Since 2007, SFCHC's Trans:Thrive has provided comprehensive medical and social services for the trans/GNC community of San Francisco. More than a drop-in center, Trans:Thrive is a safe space and “one stop shop” for community members to access programming to improve their quality of life - offering case management, medical services (including primary care, dental care, behavioral health services, gender affirming hormone therapy and surgeries), substance use services and treatment (including harm reduction supplies, counseling, treatment linkage), support groups, hot meals, and community events.
Programs:
San Francisco Transgender Alliance for Health Resources (STAHR): STAHR is an innovative, collaborative, client-centered Health Access Point (HAP) program specifically focused on addressing the needs of trans women in San Francisco (SF). STAHR operates through a close working partnership that brings together five highly respected SF agencies and programs, all of whom have distinguished and extensive experience in reaching, serving, and retaining trans women in HIV, HCV, and STD testing, care, and support. STAHR strives to reach and serve all trans women living in SF, with a strong emphasis on African American and Latina trans women.
House of Thrive (HOT): The HOT program is an innovative, multi-level intervention designed to build life skills, increase education and employability, and build healthier, more self-directive futures for low-income and marginalized trans people in SF. The H.O.T. Program is a year-long goal-setting case management program that supports a cohort of TGNC participants and is funded through the Mayor's Office of Housing & Community Development (MOHCD).
Role Summary:
The Case Manager will work closely with the Program Manager of Trans Services, the Program Specialist and Outreach and Community Engagement Specialist, and will be responsible for overseeing case management of clients in the STAHR and HOT programs.
Essential Duties:
Manage case management services of both STAHR and HOT programs by conducting intake, assessment, care plans, and re-assessment of eligible transgender clients
Provide case management services to ongoing caseload and develop individual-client centered goals, resource and care navigation, harm and risk reduction counseling, and treatment plans following agency's standards of practice.
Assess and enroll eligible clients into Enhanced Care Management (ECM) to ensure smooth and consistent care coordination across SFCHC departments as well as other community clinics and health care providers.
Provide comprehensive linkage support, education, shelter, and treatment regarding PrEP, PEP DoxyPEP, HIV/STI testing, substance use, harm reduction, access to basic need services, primary medical care, and behavioral health services.
Work occasional community pop-up testing events on weekends for outreach.
Meet with clients on a regular basis to ensure client retention in medical care, behavioral health services, substance use treatment and prevention, stable housing and shelter, and other individual client goals, depending on acuity level.
Ensure timely and accurate notes and documentation are input in applicable systems such as EPIC, CareLink, GMS, ONE System, and other secure tracking systems.
Enroll clients in various medical care services including primary care, mental health services, needle exchange and syringe access, gender affirming care, and safer sex and substance use.
Provide supportive services and resources to trans clients at Trans:Thrive, other SFCHC sites, as well as inside various SF jails, psychiatric facilities, and rehabilitation sites.
Conduct outreach and recruitment activities to deliver peer-based HIV prevention services in venues where transgender communities congregate and socialize including city-wide events, clubs, bars, support groups, etc.
Perform quality data collection, data entry, maintenance, tracking of client enrollment/ activities, and timely submission of units of services for monthly, quarterly, and yearly reporting requirements.
Promote and represent agency services at outreach events.
Participate in trainings, monthly staff, program, and department meetings, agency events, and necessary HAP meetings for SFDPH as well as MOHCD grantee meetings.

Minimum Qualifications
Ability to sit, stand and walk at least 50% of the work hours (this position requires outreach).
Two years of college or equivalent combination of education and experience.
Three years of experience working or closely associating with transgender individuals and issues.
Familiarity with issues and concerns of transgender sex workers and substance users; experience working with multiply diagnosed patients (HIV, mental health conditions, and substance abuse), sexual and gender minorities, people experiencing homelessness, immigrants and refugees.

Role Summary:

Provide onboarding and training for new ECM staff and teams to ensure consistent understanding of program goals, workflows, and documentation standards.
Coach and support existing ECM staff by addressing workflow updates, correcting recurring errors, and reinforcing best practices.
Conduct monthly ECM file reconciliations in collaboration with managed care plans, ensuring accuracy and compliance with program requirements.

Project HOPE (Hepatitis C and Overdose Prevention Empowerment) is a program which provides low-barrier and accessible hepatitis C treatment and navigation in high impact settings such as syringe exchange sites, opioid treatment sites, and various drop-in services as well as sexual health clinics which aim to reach target priority populations. The Program Specialist will also assist the Community Engagement team with its various projects and programming including the Community Living Room, and will report to the Program Manager of Community Outreach & Linkage.
Essential Duties:
Work collaboratively with direct supervisor and clinical team on to ensure that program activities are implemented and completed successfully;
Conduct city-wide outreach to engage and assess individuals at risk for Hep C, HIV, and accidental overdose and provide health education and linkage support to supportive programming within SFCHC and throughout San Francisco.
Develop and facilitate support groups for clients to address risks for HIV, Hep C, and accidental overdose, and monitor client progress throughout experience.
Coordinate with SFCHC Wellness Clinic providers to support clients living with Hep C with linkage and follow-up support, providing outreach and navigation assistance, as needed, to support with treatment adherence and engagement.
Become certified as a HIV and Hep C Test Counselor through SFDPH and provide as-needed rapid testing and counseling.
Work collaboratively with direct supervisor, Community Engagement department management and staff, and community partners to ensure high quality service delivery for agency guests, clients, and patients.
Provide high quality customer service to guests, clients, and patients accessing SFCHC programming and care.
Support guests, clients, and patients, with linkage and navigation support to other SFCHC programming as requested.

Program Summary: Taimon Booton Navigation Center (TBNC) is our shelter and service center for transgender, gender non-conforming & intersex people, as well as cisgender women, to find sanctuary, community, and a pathway to housing. Gender-supportive housing offers a crucial refuge, fostering belonging and safety. Role Summary:
Under the supervision of the Shift Supervisor and Director, Shelter Monitors are responsible for the operation and security of the shelter site and the supervision of the shelter guests. Key Responsibilities:

Ensure the safety and security of shelter site and respond to emergency situations as needed.
Always enforce all shelter policies with guests.
Have a deep understanding of and can explain shelter rules and policies to guests.
Report issues requiring repairs and maintenance to Operations Manager.
Create daily reports in compliance with shelter policies and regulations.
Facilitate daily check-in and check-out of guests from the shelter.
Mediate and de-escalate conflicts using methods learned in training.
Remain awake and alert during assigned shifts.
Attend regularly scheduled meetings, trainings, and activities.

The Network Support Technician is a visible role supporting and providing technical support to end-users in the office, as well as for all remote users. Job Description:
The Outreach and Community Engagement Specialist will work closely with the STAHR Program, Trans Services Team and collaborators and will be responsible for creating new and innovative outreach recruitment strategies (including social media) and implementing community events that target trans women and GNC folks that are at high risk for HIV and STDs (e.g. sex workers, multiple sex partners, etc.). Our Street Medicine providers deliver primary health care, urgent care, wound care, phlebotomy and lab results, behavioral health care, and case management outside of our clinic walls, directly to our community members living on the streets. We meet people where they, build long term relationships, and partner with people to support their health and wellness.
Position Description:
The Street Medicine Therapist is part of a multidisciplinary team that includes medical providers, nurses, case managers and community health workers. You will meet community members where they are located in the Tenderloin area, be it at their residences, in shelters or on the street. In this role, you will provide ongoing and episodic therapy to unhoused and recently housed people where they live, and housed persons who are not able to engage in clinic-based care. You will have the opportunity to collaborate with your direct team members, community organizations, other clinicians on the behavioral health team, and most importantly clients/patients. The Patient Service Associate [PSA ] is the first person at SFCHC primary care clinic welcoming and greeting patients. Patient registration is a critical phase of SFCHC's Clinic Business Operations as it directly impacts the financial stability, operational efficiency and delivery of patient-centered care. Position Description:
The Clinical Manager performs duties in office, clinic, and community settings that are within the scope of practice for a Certified Medical Assistant. This position is responsible for the direct supervision of clinical staff (MAs, phlebotomists, Patient Care Coordinator, Referral Coordinator, Medical Front Desk Staff, and other clinical administrative staff). The Complex Care team at San Francisco Community Health Center (SFCHC) cares for the residents of the Tenderloin and adjacent San Francisco neighborhoods. The team consists of case managers, community health workers and nurses who strive to deliver quality care management to patients experiencing barriers to care due to mental health, substance use, homelessness, or a history of incarceration. Medical care and case management is delivered with an objective client driven approach, at the homes, shelters, temporary and permanent housing, acute care settings and within the SFCHC clinic. The Community Health Worker (CHW) is a trusted member of and/or a person who has a close understanding of the community served in the delivery of health-related services through either working directly with providers or their partner organizations. Position Summary:

Essential Duties:

Work with the Program Manager for the Community Living Room to ensure supplies and materials needed for activities and events are ordered and available.
Create and distribute media, including but not limited to, promotional flyers for events and activities, educational presentations for the Community Engagement department and other programming within SFCHC.

Taimon Booton Navigation Center (TBNC) is a shelter and service center for transgender, gender non-conforming & intersex people, as well as cisgender women, to find sanctuary, community, and a pathway to housing. Position Description:
The Program Manager will oversee the day-to-day functions of the RESTORE case management program. RESTORE stands for Recovery Engagement to Start Treatment for Overdose Response Equity. The Wellness Clinic is a core function of San Francisco Community Health Center. In 2015, we became the first new Federally Qualified Health Center (FQHC) in San Francisco in 40 years, enabling us to expand our capacity and provide full primary care services. Role Summary:

The Patient Service Associate [PSA] is the first person at SFCHC primary care clinic welcoming and greeting patients. Patient registration is a critical phase of SFCHC's Clinic Business Operations as it directly impacts the financial stability, operational efficiency and delivery of patient-centered care. The Patient Service Associate works collaboratively with eligibility, Enhanced Care Management [ECM] and clinic enabling support staff. The PSA reports to the Revenue Cycle Manager under the daily supervision of the Health Eligibility and Access Supervisor.
Essential duties:
Schedule initial visit, pre-registration, check-ins and follow-up appointments and call or text messaging reminders for medical, behavioral and dental clinic patients.
