Summary: This position independently performs requested ultrasound examinations, searching for any possible pathology as indicated by the patient's symptoms/medical history and/or results from previous testing. The position reviews and discusses the ultrasound findings with the radiologist and a diagnosis is made. Responsibilities And Duties: 60% Performs various ultrasound examinations. Subjectively interprets information based on judgmental decisions by Ultrasonographers, requiring in-depth knowledge of human anatomy, physiology and pathology. Maintains complete knowledge of wave form physics, i.e., , time gain compensation curves, attenuation factors, etc. , required to set equipment variable properly for appropriate imaging of specific anatomy. Assists Radiologists with performance of invasive procedures. 10% Independently evaluates normal vs. abnormal test results in correlation with patient history, previous examinations, lab results, etc. 10% Independently gives preliminary ultrasound results to attending physicians, surgeons, residents, etc. regarding any vascular ultrasound procedures as determined by site accreditation. 10% Teaches/educates imaging students, medical students and residents in basic scanning methods and instrumentation of ultrasound. 10% Maintains equipment and stocks supplies. JOB SUMMARY: Community Psychiatric Supportive Treatment (CPST) Service provides an array of services delivered by community based, mobile individuals or multidisciplinary teams of professionals and trained others. Services address the individualized mental health needs of the client. They are directed towards adults, children, adolescents, and families and will vary with respect to hours, type and intensity of services, depending on the changing needs of each individual. The purpose/intent of CPST is to provide specific, measurable, and individualized services to each person served. CPST services should be focused on the individual's ability to succeed in the community; to identify and access needed services, and to show improvement in school, work and family and integration and contributions within the community. ESSENTIAL FUNCTIONS AND DUTIES: The CPST service is comprised of the following activities as they relate to the individual's symptoms of mental illness and corresponding deficits in current functioning: (1) Coordination and implementation of the service recipient's ISP, including ensuring that the ISP reflects the most current interventions necessary to address the individual's mental health needs and symptoms of his/her mental illness, as evidenced by the service provider's: (a) Participation in the development of the ISP; (b) Coordinating other services and providers identified in the ISP to ensure that the ISP is being implemented as written; (c) Monitoring the individual's progress in achieving goals and objectives/anticipated outcomes as documented on the ISP; and (d) Monitoring the individual's status in relation to his/her ISP goals to identify when a change in mental illness symptoms indicates the need for a clinical review of the individual's mental health assessment and ISP. Such clinical review shall be performed by an appropriately qualified individual in order to determine whether a revision of the goals, objectives and/or interventions is warranted. (2) Support in crisis situations, including the service provider: (a) Working with the individual, and family, guardian and/or significant other, as appropriate, to develop a crisis management and contingency plan; and (b) Coordinating and/or assisting in crisis management and stabilization as indicated. (3) Assessing the individual's needs, including psychiatric, physical health, entitlement benefits, wellness, support system, and community resources, e.g., the need for housing, vocational assistance, income support, transportation, etc., in order to: (a) Incorporate those needs and accompanying rehabilitative services and activities in the ISP; and (b) Coordinate linkages to needed community services, support systems and resources. In addition, when the individual's mental illness impedes his/her ability to access these services him/herself, the service provider shall: (i) Assist the individual in accessing needed community services, support systems and resources, and (ii) Assist the individual to develop the skills to access needed services, support systems and resources for him/herself. (4) Individualized, restorative interventions and training to improve interpersonal, community integration, and independent living skills when the individual's mental illness impacts his/her ability to function in and adapt to home, school, work and community environments. Specific training may address: (a) Socialization abilities, including communication, interpersonal relationships, problem solving/conflict resolution, and stress management; (b) Support system development; (c) Employment readiness activities, excluding skill specific vocational training. Examples of employment readiness abilities which may be impacted by a person's mental illness include work related social and communication skills, personal hygiene and dress, time management, etc.; and (d) Other interventions and training necessary to ameliorate life stresses resulting from the individual's mental illness. (5) Assisting the individual to acquire psychiatric symptom self-monitoring and management skills so that the individual learns to identify and minimize the negative effects of the mental illness that interfere with his/her daily functioning. (6) Advocacy and outreach when the individual's mental illness prevents him/her from doing this for him/herself. (7) Mental illness, recovery and wellness management education and training. The education and training may also be provided to the individual's parent or guardian, and family and/or significant others, when appropriate, and when: (a) This education and training is based on the individual's mental illness and symptoms; and (b) This education and training is performed exclusively on behalf of and for the well-being of the individual, and is documented in the ISP. (8) Adhere to the agency's personnel policies and procedures, ODMH, Medicaid and Insurance standards, and fulfill documentation and reporting requirements. (9) Inform both the school site and/or (Your Names) when ill or unable to attend. (10) Participates in quality assurance and program evaluation studies. (11) Attend and participate in scheduled or required training, staff meetings, peer review, workshops, and supervision. (12) Participate in professional development trainings and workshops offered in the community and agency that is pertinent to jobs duties and responsibilities. SUMMARY We are currently looking for someone skilled at engaging and working with children, youth, and families with significant behavioral health needs to be a Care Coordinator. PRIMARY DUTIES AND RESPONSIBILITIES Coordinates services as the lead member of the care team by coordinating, attending and actively facilitating team meetings to monitor/assess case progress, appropriateness of services, and meet the safety and treatment needs of the child, youth and family. Identifies cultural factors that influence strengths, functioning, and family interaction styles to ensure ongoing engagement and success in care planning. Coordinates family-based-services for children, youth, and families in their home, school, and community. Link service to families and support appropriate referrals to local community services and resources. Provides crisis response by phone and linkage to appropriate resources as part of an On-Call Rotation after regular business hours. Completes training in High Fidelity Wraparound and skills-based training to provide ICC and/or MCC and ensures maintenance of training and certification requirements. Utilizes Assessment, Care Planning and Coordination through the High-Fidelity Wraparound model to match the intensity of services to the needs of the children, youth and families. Ensures the utilization of the CANS for ongoing assessment to inform care planning and coordination and review care plan in accordance with coordination activities (OAC 5160-59-03.2). Updates services in care plan as children, youth, and family's needs change pertinent to care plans and CANS assessments. Plans visits and attends scheduled meetings around family's needs (i.e., work schedules, school activities, etc.). Develops collaborative relationships with partners and community resources tailored to meet the needs of culturally diverse healthcare consumers and family. Maintains fluency in systems and software pertinent to completion of required documentation and submission of required documentation. Participates in ongoing fidelity review and monitoring system focused on consistent application of system of care principles, adherence to ICC/MCC planning process and service components. Participates in staff and team meetings for the schools and agency, staff development and in-service training, planning interventions and regular supervisory conferences. EDUCATION & EXPERIENCE Background in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field. Experience providing community-based services to children, youth, and their family or caregivers, family systems, community systems and resources, case management, child and family counseling or therapy, child protection or child development. Three years relevant experience with a high school diploma or equivalent; or Two years relevant experience with an associate's degree or bachelor's degree; or One-year relevant experience with a master's degree or higher. SKILLS & ABILITIES Reasoning Ability Ability to maintain a high degree of empathy and compassion in meeting the needs of agency clients and client families. Ability to build strong bonds with employees to foster open, honest and candid communication. Ability to multi-task and maintain organization in a fast paced, changing environment. Ability to manage change in an organization reengineering its culture and approach to workload management. Ability to successfully operate with ambiguous guidelines where ethical decision will be required. Ability to create and maintain highest levels of confidentiality when dealing with client information, SVFS proprietary information and sensitive situations. Language Skills Ability to effectively communicate plans, goals, directives and diagnosis information between clinician and clients. Technology Skills Computer skills, Word, Outlook, GPS systems, and phone skills needed. ADDITIONAL Applicants will occasionally be asked to work evening and/or weekend hours due to the service delivery and administrative needs of SVFS clients and families. Applicants must have received or be willing to receive the COVID-19 vaccine by date of hire to be considered for employment.
