    CONTRACTOR SALARY: $21/HR Scope: Under the direction and supervision of the Home Care Manager, the Personal Care Worker (PCW) is responsible for providing and assisting clients with activities of daily living and supportive care necessary to the preservation of the home environment. The PCW will assist in monitoring the health status, providing a safe and positive living environment to enhance the client's quality of life. Job Responsibilities: Provide unskilled personal care services and homemaking services. Accompany clients to appointments, on errands and shopping as appropriate. Provide assistance or stand by assistance with bathing, skin care, hair care, toileting, dressing, shaving, nail care, mouth care, etc. Assists with IADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) Observe clients taking prescribed medications. Clean stove, kitchen, bathroom, and only the client's living area and bedroom Vacuum, sweep, dust, take out trash, run errands as needed, mop floors. Advises Program Manager of requests which cannot be met or of conditions which are unsafe. Record and report observed noticeable changes in client's physical condition, behavior, or appearance immediately to Program Manager and/or nurse. Documents services provided to clients consistently and accurately. Documentation is completed in a timely manner. Participates in agency required and assigned training programs and any required staff and training meetings. Document daily activities and report any significant changes in a client's needs or living conditions. Additional responsibilities may be added as appropriate. Various programs offer community support services to families and are designed to meet the needs of individuals with disabilities throughout their life span. Scope: This position will focus primarily on ensuring that the rights of the Host Home clients are being met, implementing the regulatory and safety standards of the agency, the Division for Individual Developmental Disabilities and the Community Center Boards. The position will be under the supervision of the Host Home Program Director. Essential Duties: Assure that applicable standards of Health Care Policy and Financing are implemented and audited on a regular basis and that residents receive a high quality of services. Assist with the process of reporting protocols to the IDT, to ensure the safety of the client. Ensure that each client's agency file is completed and up to date at all times. This includes the client's therapy and shared drive. Conduct provider monitoring visits according to the monitoring schedule protocol ensuring they are completed and that there is a written follow up plan to address any deficiencies identified and how they will be corrected. Assess the health and safety of each client in the home; obtain required documentation and signature of the provider and client. Attend HRC meetings for assigned clients. Problem solve issues and concerns with the Program Director, Host Home team, IDT team, provider and client; and ensure that recommended actions are consistent with agency policies, procedures and protocols. Schedule and attend staff meetings related to the client service plan. Create plans, implement and monitor ISSP's & BISSP's for assigned clients. Assist in working with the providers to coordinate the medical, dental, behavioral, psychiatric and other needed services for the client. On a monthly basis, oversee the Personal Needs Funds cash ledgers of clients on a caseload and submit all necessary paperwork as delegated in a timely manner. Meeting and communicating regularly with the Residential team and other adult services staff and managers as needed. Assist the Host Home Department team and Director of Adult Services in providing and implementing provider training as delegated by the Residential Services Program Director. On call as scheduled which could involve communication via phone or text or in person assistance after hours. Perform related work as assigned. Minimum Qualifications: Prefer a Bachelor's degree in a Human Service field Require a minimum of two years' experience in serving individuals with developmental disabilities as a Host Home supervisor. Prefer 1-2 year experience as a Host Home Supervisor or department team leader Strong leadership abilities and problem solving skills required. CPR, First Aid & CPI, & Universal Precautions certification within the first 30 days of hire. A valid driver's license & current vehicle insurance, and an acceptable driving record per SKSF standards. SALARY: $50,000/yr.
