Responsibilities:
This position assists residents with personal care (bathing, dressing, and eating). You provide transportation to/from residential homes and community events. You will help with their exercise programs, play games, work on projects, and share time outside. As a member of the care team, you’d help encourage individuals to attend scheduled therapies, clean, and help keep the program pleasant and comfortable. You will be required to complete daily reports/communications, and ensure resident safety.
We provide the same level of care in our homes that we’d want for anyone we love.
Qualifications:
Direct care experience is great, though we can help you learn what you need to know so long as you have commitment to the work. · Basic computer/office skills Microsoft Office (daily reports/communications).· Physical ability to perform the requirements of this position.· A valid driver’s license and acceptable motor vehicle record.· High School Diploma or GED is required.· Background check.·

This job description is intended to describe the nature and level of work being performed by employees assigned to this position. It is not to be construed as an exhaustive list of all responsibilities and duties required of the job incumbents.
GENERAL SUMMARY
Provides community habilitation services to individuals through the implementation of service and support plans, documentation of data and other means assigned and, as established by the planning team to address habilitative needs for the development and support of independent living. Community Support Professionals may be assigned to work with any individual served by the Community Habilitation Program.
ESSENTIAL FUNCTIONS
- Maintains regular attendance and punctuality in order to provide quality habilitative services to individuals served. This position performs all hours at physical locations required for services for the individuals served including but not limited to individual’s homes, retail stores, banks, fitness centers, etc. Work hours will also include required training at assigned JMM Murray and/or off site locations.
- Follows all safety rules and regulations; wears and/or uses required protective equipment while working; immediately reports any safety concerns or unsafe working conditions to Support Services management.
- Conducts all aspects of community habilitation services to individuals served in compliance with the JM Murray mission statement, policies and procedures, professional standards, and applicable governmental laws, regulations, rules and standards.
- Provides community habilitation services to assigned caseload, both individually and in groups. Duties include support with food shopping, general purchases, fitness activities, self-care skills training, specific to individual goals as outlined in the Staff Action Plan.
- Follows the schedule as assigned by the Sr. Team Leader. Works cooperatively with others to ensure individuals receive service as scheduled and that requests by individuals for schedule changes are communicated to the Sr. Team Leader.
- Provides Sr. Team Leader with information and feedback regarding individual staff action plans and makes recommendations for modifications in order to enhance the skills and learning of individuals served.
- Provides safe transportation and on site supervision to support individuals at community sites for assigned caseloads. If using a company vehicle, ensures it is left clean inside and free of debris, it has gas in it, and that first aid kits are on hand. Immediately reports any mechanical, safety or other needs for repair and maintenance. Ensures their personal vehicles are clean inside and free of excess debris.
- Completes appropriate and timely documentation of services which includes the use of the JM Murray Electronic Medical Record, and the use of equipment such as a computer, laptop, and/or a tablet.
- Attends all required and requested training and meetings, and maintains certifications and updates to training, as assigned.
- Conducts self in a manner as to always contribute to maximum individual growth and in accordance with all regulations, the DSP Code of Ethics and Company policy and procedures. Represents JM Murray and the JM Murray mission in a positive manner, treating people with respect and dignity.
