The LTSS Service Coordinator - RN Telehealth is responsible for overall management of member's case within the scope of licensure; provides supervision and direction to non-RN clinicians participating in the member's case in accordance with applicable state law and contract; develops , monitors, evaluates, and revises the member's care plan to meet the member's needs, with the goal of optimizing member health care across the care continuum.

* Responsible for performing telephonic clinical assessments for the identification, evaluation, coordination and management of member's needs, including physical health, behavioral health, social services and long term services and supports.
* Identifies members for high risk complications and coordinates care in conjunction with the member and the health care team.
* Manages members with chronic illnesses, co-morbidities, and/or disabilities, to insure cost effective and efficient utilization of health benefits.
* Obtains a thorough and accurate member history to develop an individual care plan.
* Establishes short and long term goals in collaboration with the member, caregivers, family, natural supports, physicians; identifies members that would benefit from an alternative level of care or other waiver programs.
* The RN has overall responsibility to develop the care plan for services for the member and ensures the member's access to those services.
* May assist with the implementation of member care plans by facilitating authorizations/referrals for utilization of services , as appropriate, within benefits structure or through extra-contractual arrangements, as permissible.
* Interfaces with Medical Directors, Physician Advisors and/or Inter-Disciplinary Teams on the development of care management treatment plans.
* May also assist in problem solving with providers, claims or service issues.
* Directs and/or supervises the work of any LPN/LVN, LSW, LCSW, LMSW, and other licensed professionals other than an RN, in coordinating services for the member by, for example: Assigning appropriate tasks to the non-RN clinicians, verifying and interpreting member information obtained by these individuals, conducting additional assessments, as necessary, to develop, monitor, evaluate, and revise the member's care plan to meet the member's needs, and reviewing and providing input on the non-RN clinicians' performance on a regular basis.
Minimum Requirements:
* Requires an RN and minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role; or any combination of education and experience which would provide an equivalent background.
* Current, unrestricted RN license in NY required.
Preferred Skills, Capabilities, and Experiences:
* Bachelor's in Health/Nursing preferred.
* Bilingual in Spanish, Mandarin, or Korean highly preferred.
* May require state-specified certification based on state law and/or contract.
* CHHA and/or Medicare Experience Preferred.

PRIMARY FUNCTION:

Responsibilities
Primary:

Develop and manage project timelines, resources and deliverables and monitor project outcomes to define and track metrics and ensure successful execution.
Write copy for BAM-led content, primarily for ADL.org pages and ADL's Impact Story library.
Lead BAM team communication to update on content releases and waves of promotion, brand and content campaigns to track and contribute to integrated marketing across the team.
Project manage and contribute to the ADL Impact Story library, collaborating across the organization to identify, capture and upload impact stories, in addition to assessing and updating entries for keywords, categorizations, usage and archiving.
Play a key role in the planning and programming for Never Is Now, coordinating with stakeholders on topics, developing content and contributing to event experiences to elevate ADL's content and brand at the event.

Work Experience:

Compensation:
This position has a salary range of $85,000 to $100,000. This salary range is reflective of a position based in New York, New York. The Major Gift Officer serves as a frontline fundraiser within a region or division. They will maximize the donor engagement process to identify, qualify, cultivate, solicit, and steward individual and institutional prospects/donors capable of making gifts between $50,000 and $500,000 in support of ADL's highest priorities. Responsibilities
Primary:
Develop and implement identification, qualification, and cultivation strategies for a portfolio of individuals with the capacity to make major gifts (between $50,000 and $500,000), nurturing relationships through personalized communications, meetings, and engagement opportunities in accordance with ADL's policies, procedures, and funding priorities.
Lead the solicitation process for major gifts, including developing tailored solicitation proposals, presentations, and funding opportunities aligned with donor interests and ADL priorities.
In collaboration with the Donor Experience and Philanthropic Services teams ensure fundraising and implementation of approved, budgeted regional fundraising and non-fundraising event such as insider briefings.
Maintain an active, current body of knowledge of ADL and its mission, programs, activities, institutional needs, and fundraising priorities.
Work with prospect research team to identify new major gift prospects and understand their philanthropic interests, capacity to give, and inclination to give to ADL.
Negotiate gift agreements and terms with donors, in collaboration with the Philanthropic Services team.
Actively participate in Development team meetings to coordinate prospect review and discuss specific prospect strategies as they relate to prioritized fundraising needs.
Maintain accurate and up-to-date donor records in the CRM database, documenting all interactions, proposals, and gift agreements related to major gifts in accordance with CSC policies and procedures.

PRIMARY FUNCTION:

Primary:

Define data validation rules and automation.

As required by applicable state law and contract, the Clinician contributes to the LTSS care coordination process by performing activities within the scope of licensure including, for example, assisting the responsible RN with telephonic or face-to-face assessments for the identification, evaluation, coordination and management of member's needs, including physical health, behavioral health, social services and long term services and supports.
How you will make an impact:
* Assists responsible RN in identifying members for high risk complications. Obtains clinical data as directed by the responsible RN.
* Assists the responsible RN in identifying members that would benefit from an alternative level of care or other waiver programs.
* Provides all information collected to the responsible RN, who verifies and interprets the information, conducts additional assessments, as necessary, and develops, monitors, evaluates, and revises the member's care plan to meet the member's needs.
* Participates in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities as directed by responsible RN, and in conjunction with the RN, member and the health care team, to ensure cost effective and efficient utilization of health benefits.
* Decision making skills will be based upon the current needs of the member and require an understanding of disease processes and terminology and the application of clinical guidelines but do not require nursing judgment.
Minimum Requirements:
* Requires an LPN/LVN, LSW, LCSW, or LMSW or license other than RN in accordance with applicable state law and Nursing Diploma or AS in Nursing or a related field and minimum of 2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role; or any combination of education and experience, which would provide an equivalent background.
* Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.
* May require state-specified certification based on state law and/or contract.
Preferred Skills, Capabilities and Experiences:
* MA/MS in Health/Nursing preferred.
* Travels to worksite and other locations as necessary.

PRIMARY FUNCTION:
The Major Gift Officer serves as a frontline fundraiser within a region or division. They will maximize the donor engagement process to identify, qualify, cultivate, solicit, and steward individual and institutional prospects/donors capable of making gifts between $50,000 and $500,000 in support of ADL's highest priorities. While Major Gift Officers may be assigned to a specific geographic area, they may also take on individual prospects/donors throughout the division. This role requires significant travel of up to 50%.
Responsibilities
Primary:
Develop and implement identification, qualification, and cultivation strategies for a portfolio of individuals with the capacity to make major gifts (between $50,000 and $500,000), nurturing relationships through personalized communications, meetings, and engagement opportunities in accordance with ADL's policies, procedures, and funding priorities.
Lead the solicitation process for major gifts, including developing tailored solicitation proposals, presentations, and funding opportunities aligned with donor interests and ADL priorities.
In collaboration with the Donor Experience and Philanthropic Services teams ensure fundraising and implementation of approved, budgeted regional fundraising and non-fundraising event such as insider briefings.
Maintain an active, current body of knowledge of ADL and its mission, programs, activities, institutional needs, and fundraising priorities.
Work with prospect research team to identify new major gift prospects and understand their philanthropic interests, capacity to give, and inclination to give to ADL.
Negotiate gift agreements and terms with donors, in collaboration with the Philanthropic Services team.
Actively participate in Development team meetings to coordinate prospect review and discuss specific prospect strategies as they relate to prioritized fundraising needs.
Maintain accurate and up-to-date donor records in the CRM database, documenting all interactions, proposals, and gift agreements related to major gifts in accordance with CSC policies and procedures.
Collaborate and partner effectively across the organization to build and sustain the culture of philanthropy across ADL.
Serve as an effective and enthusiastic ADL spokesperson and representative.

Compensation:
This position has a salary range of $70,000 to $114,000. This salary range is reflective of a position based in New York, New York.
