Responsibilities MEDICAL RECORDS TECHNICIAN JOB DESCRIPTION: The Medical Records Technician coordinates the maintenance of medical record files (acute, residential, and outpatient), provides telephone and reception coverage for the Health Information Services (HIS) Department, coordinates processing of discharge and active charts, maintains accurate computerized databases, medical transcription and decrypting, processes daily census reports, handles Release of Information (ROI) requests, and other duties to assist in the efficient functioning of the HIS Department.
JOB RESPONSIBLITIES:
* Abides by the system's policies and procedures, State and Federal regulations, and HIPAA guidelines while processing patient's ROI requests.
* Enters and maintains computerized deficiency system and prints weekly reports; analyzes each record, tags and untags deficiencies.
* Assembles charges upon discharge for retrospective and discharge processing; copies records for Retro-Insurance Reviews.
* Analyzes each discharged record, tags and untags deficiencies ensuring compliance with all applicable policies.
* Assists in verification of billing ICD-10 codes as needed.
* Retrieves records for direct caregivers to complete or review; purges records as necessary.
* Prints records from microfilm
* Ensures records are filed in appropriate slots/order.
* Decrypts, corrects, and prints transcribed reports, tags reports with appropriate physician's tag, and places in appropriate medical record.
* Provides release of information assistance as needed; forwards referral data to next provider; maintains computerized tracking system of all requests processed.
* Proactively addresses any problems that may jeopardize patient care.
* Maintains adequate inventory of departmental supplies.
* Performs other duties as assigned.
EDUCATION/EXPERIENCE: High School Diploma and Associates Degree or combination of training and experience. One-year experience working in medical records.
KNOWLEDGE/SKILLS:
* Ability to perform assignments with minimal supervision;
* Ability to work successfully under highly stressful conditions;
* Ability to make sound, independent judgments based on scientific and/or ethical principles;
* Capability of adapting to varying workloads and work assignments on a constant basis;
* Effective comprehensive reading skills, strong communication skills, written and verbal.
* Must have a working knowledge of computers.
Qualifications
MINIMUM REQUIREMENTS OF THE POSITON:
* Must be able to complete new hire requirements such as State of Alaska Background Check and Drug Testing.
* Must be able to demonstrate special training, knowledge and skills specific to job and/or program competency within the first three (3) months of training.
* Must complete all mandatory in-services annually.
* Must be tested for Tuberculosis with a PPD skin test or chest x-ray upon hire; PPD skin test required annually or chest x-ray annually thereafter.
GENERAL WORKING ENVIRONMENT: Working conditions for all employees are as follows, but not limited to a psychiatric hospital setting; some risk involved in the event of aggressive patient(s); work load may include day, evening, night, weekend and/or holiday shifts. Employees occasionally exposed to blood borne pathogens, and loud noise levels.
Mental demands described herein are representative of those which must be met by an employee to successfully perform the essential functions of the job, but are not limited to the ability to remain calm in a stressful environment; and the emotional stability, physical stamina and agility to handle stress and respond quickly and effectively to emergency situations.
PHYSICAL REQUIREMENTS: Requirements include, but are not limited to the ability to communicate effectively with employees; the ability to effectively utilize communication equipment; the physical agility to manage cleaning equipment; the ability to read, write.
* Frequently utilize physical ability for standing or sitting for long periods of time.
* Regularly utilize physical ability for speaking, hearing, and seeing with correction for close and distance vision, color discrimination, peripheral vision, depth perception and focusing ability. Speaking and hearing may be necessary for conversing with and assessing employees.

Key Responsibilities
Risk Management Oversight
* Maintain and oversee the Healthcare Peer Review Reporting System to ensure timely and accurate reporting of incidents and occurrences.
* Analyze trends and data to identify actual and potential risks, determining root causes and facilitating appropriate interventions.
* Refer incidents to relevant departments or medical committees; escalate Level III/IV occurrences to the Peer Review Committee.

* Develop and support risk-reduction and loss-prevention programs that positively impact clinical care.
* Work closely with the Patient Advocate to evaluate complaints with potential legal implications and recommend appropriate resolutions.
* Assess liability exposure, determine the likelihood of legal actions, and identify high-risk practices to inform preventive strategies.
* Serve as a key resource for addressing complex treatment issues such as informed consent, HIPAA violations, patient refusals, and discharges Against Medical Advice (AMA), in consultation with the Administrator, Corporate Legal Counsel, and Corporate Risk Management.

* Coordinates travel arrangements as directed by the therapists and communicates all information to therapists in a timely manner; contacts family with travel information and provides copies of necessary travel documents to family if directed by the therapist/treatment team.
* Coordinates discharge appointments and communicates to therapist and family if directed by therapist/treatment team.
* Maintains documentation on all cases; ensuring progress notes on all case management contact/discharge planning efforts are completed no later than 24 hours after the contact; documents discharge appointments in the discharge paperwork.
* Participates in community activities as requested by the Director of Social Services to build relationships with community providers.
* Serves as an effective team member and works closely with the Utilization Review team and clinical team.

Position Overview: As an Intake Clinician, you will play a crucial role in providing compassionate care and support to individuals seeking assistance with mental health challenges and substance abuse issues. Conduct comprehensive neuropsychological evaluations for patients referred from inpatient and outpatient programs.

3. Partner with Facilities Manager to ensure ICRA form is completed for facility projects.

6. Effectively monitor the effects of intervention strategies on infection rates and modify the interventions as necessary.

8. Present monthly reports to Environment of Care Committee.
9. Present quarterly reports to Quality Council.
10. Present annual reports to Medical Executive Committee.
11. Ensure compliance with State, Federal, and Joint Commission requirements.
12. Enter and maintain data in VacTrAK in compliance with State guidelines.
13. Maintain all vaccines within State guidelines and complete all documentation with the Medical Director.
14. Is educated on changes, trends, and developments in infection control management of both child and adult psychiatric patients and is sought out as a subject matter expert and resource by other members of the nursing staff.
Employee Health:
1. Conduct employee health functions, which include: pre-employment health screening, maintenance of employee health files, and ensure annual employee health requirements.
2. Develop and deliver educational efforts that improves employee's understanding of their health and safety on the job.
3. Maintain complete and accurate employee health records, including initial and annual employee health requirements.
4. Performs other duties as assigned.
Qualifications
EDUCATION/EXPERIENCE: Minimum of an Associate's Degree in Nursing and a valid State of Alaska RN License. 2 years of nursing experience required.
KNOWLEDGE/SKILLS:
1) Able to frequently travel to assigned areas;
2) Ability to perform assignments with minimal supervision;
3) Ability to work successfully under highly stressful conditions;
4) Ability to make sound, independent judgments based on scientific and/or ethical principles;

Key Responsibilities:
* Completes Activity Therapy assessment on patients, ensuring each assessment is thorough and addresses all required fields.
* Participates in development of Master Treatment Plan and recommends effective alternatives or modification to treatment plans based on patient progress and response to treatment program.
* Provides therapeutic recreation and activity therapy groups; able to provide a variety of appropriate therapeutic interventions depending on client needs and abilities.
* Establishes a positive rapport and minimizes resistance with a wide variety of patients.
* Manages group process to maximize therapeutic benefit to patients.
* Documents patients' response, progress, and behavior in the group in a complete and timely manner.
* Maintains a safe, orderly, and therapeutic physical environment for patients and staff members; maintains a clean and organized activity therapy area.
* Demonstrates and/or explains the correct use of equipment and safety involved in various recreational or experimental activities.
* Demonstrates a thorough understanding of behavioral concepts applicable to the treatment of psychiatric patient population.
* Communicates with the DON/ Manager regarding planned activities for the day, patient acuity, expected group participants, personnel schedule changes, etc.
* Collaborates on unit schedule with building leadership to develop a robust and therapeutic programming schedule
* Demonstrate proper belay, knots, and Challenge by Choice" techniques in facilitating ropes course.
* Maintain supplies and equipment necessary for the safe and effective operation of the ropes course.
* Act individually or in concert with another individual in the operation of the ropes course.
* Transport patients via company vehicle when providing a community integration outing.
* Performs other duties as assigned.
Qualifications
Qualifications: To excel in this role, you should have:
* Clinical Expertise: A bachelor's degree in Recreational Therapy, humanities, social work or a related therapeutic discipline field.
* Licensure: State of Alaska CTRS Preferred
* Therapeutic Skills: Proficiency in providing psychotherapeutic services.
* Communication: Strong communication and interpersonal skills.
