POSITION OVERVIEW
The Medical Director engages in the professional, organizational, and administrative activities of the provide quality care to clients based upon their medical and psycho-social needs. The Medical Director develops, implements, evaluates the medical practice delivery model in accordance with specific goals. The responsibilities of the Medical Director are to be performed on a full time basis and are designed to provide medical leadership, services, direction and coordination in the provision of medical care and physician services. The Medical Director is the liaison between administration and the medical staff. The Medical Director assumes responsibility for all applicable Federal, State and City regulations for the department. THIS POSITION IS LOCATED IN ALBANY, NY
KEY ESSENTIAL FUNCTIONS
Participating in planning, development, and organization of medical services provided.
Meets with all scheduled patients efficiently and reviews scheduling with supervisor daily.
Develops and reviews all medical policies related to patient care.
Conduct physical examinations of patients upon their admission and thereafter on an annual, regular and on an “as-needed-basis.”
Review the completion and appropriateness of all intake documentation, including patient health history (i.e., other medications and allergies), drug abuse history and previous treatment failures.
Review and approval of initial needs assessment and patient initial treatment plans and reviews.
Participates in preparation for regulatory agency site visits.
Anytime the program is open, and a physician is not present, a physician must be available for consultation and to attend to any emergency situation.
Perform additional relevant duties as requested by supervisors/management
Knowledge of Federal, State and City regulatory requirements and ensures all aspects of clinical care conforms to these requirements.
Order, evaluate, adjust and terminate prescription of medication dosages using sound medical, ethical and responsible discretion and meets with the patient to discuss any adjustments in medication dosage.
Reviews all accident/incidents related to patient care and provides input into actions necessary to resolve identified problems. Implements corrective actions as necessary and appropriate.
Ensures physical participation in the Interdisciplinary Care Planning process.
Prescription and provision of treatment in accordance with patient's medical and mental condition and only upon their individual needs.
Determine physiological opiate dependency upon admission and patient eligibility for admission to program.
Determine patient qualification and fitness for OTP replacement therapy and/or alternate medication assisted therapy services needed. Establish buprenorphine service.
Participate in a weekly and on an “as-needed-basis” case conferencing sessions with MDT members to discuss preventative treatment measure to adverse changes as well as progress in patient's medical condition.
Medical services provided by the program
Medical director will provide medical services to the program. The medical director will have oversight of the development and revision of medical policies and procedures and ongoing training
Collaborative supervision with the Program Director of non-medical staff in the provision of substance use disorder services.
Supervision of medical staff in the performance of medical services.
Oversight of the development of policies and procedures to ensure the provision of routine services,
Oversight of the establishment of policies and procedures for public health education and screening
Assisting in the development of necessary referral and linkage relationships with other institutions and agencies
Ensuring the program compliance with all federal, state and local laws and regulations.
Assures medical staff compliance with all regulatory and licensure guidelines.
Ensures compliance of all members of the medical staff to the rules, regulations and bylaws of the Medical Department. Assures medical staff competence with all applicable policies and procedures of the facility and initiates action in cases of non-compliance.
Participating in the comprehensive care planning, discharge planning, and client and staff development by providing ongoing medical training and education to staff.
Providing medical assessment, consultation, treatment, and referrals that facilitate comprehensive and coordinated patient care.
Providing crisis intervention and consultation on medical/psychiatric emergencies.
Assessing client's need for hospitalization and consultation with client's Primary Care Provider to include other care providers the patient is linked with and/or referred to.
Reviewing all patient medication and, if indicated, prescribes medication in consultation with client's Primary Care Provider upon patient need.
Providing the Program Director with reports on recommendations concerning the medical services.
Sharing in the administrative functions that directly impact medical services; collaborating with the management team in overall planning and budget activities; assuring delivery of quality care
Services to all patients in a respectful and professional manner.
REQUIREMENTS
Active and unrestricted PHYSICIAN licensure to practice medicine in New York.
Doctor of Medicine (MD) degree or Doctor of Osteopathic Medicine (DO).
Proof of completion of three (3) years of residency and fellowship (if applicable).
One year of related practice experience in substance use disorder.
Board certification in Addiction Medicine required must be obtained within 4 years of hire.
Application and/or maintenance of National Provider Identifier (NPI).
Active and unrestricted Drug Enforcement Agency (DEA) registration.
Active and unrestricted DATA 2000 Waiver (buprenorphine) required.
NYS mandated Infection Control certification (NYS DOH approved; 4 hours, valid for 4 years).
Current Basic Life Support certification.
Knowledge of ICD-10 diagnosis and procedure coding.

POSITION OVERVIEW
Under supervision of the Clinical Supervisor the CASAC-T Counselor will provide counseling services in integrated outpatient program. The CASAC-T Counselor provides comprehensive assessment services to clients who may have substance use and mental health barriers to employment by applying clinical judgment to determine appropriate treatment needs and level of care. This includes performing SUD diagnostic screening, assessment, evaluation, orientation, treatment planning, case management, counseling, intervention, crisis intervention, patient education, reporting and record keeping in both individual and group counseling settings. The CASAC-T Counselor is also responsible for referrals and coordination of care in consultation with other professionals.
KEY ESSENTIAL FUNCTONS
Intervention and employing a range of treatment modalities to include person centered evidence-based treatment approaches to include but not limited to motivational interviewing, stages of change, trauma informed care, anger management, medication assisted treatment, dialectical behavioral therapy, cognitive behavioral therapy and contingency management.
Ability to perform SUD and mental health screenings and intakes, orientation, SUD assessment, SUD diagnostic Impression, level of care as determined by LOCADTR, treatment planning, individual/group counseling, family interventions, psychoeducation, case management, crisis intervention, client education, referral, reporting and recordkeeping, care coordination and administrative supervision with regular supervision.
Participates in supervision regularly.
Develops and conducts group and individual counseling on a regular scheduled basis to caseload clients within the integrated outpatient program.
Obtain random observed urine samples according to established protocol.
Orients clients about program goals, objectives, rules and regulations, client's obligations, right's and program operations
Knowledge of the obligation to engage in prevention as well as treatment techniques.
Maintains and updates referral services and resources that may be useful to our client population; makes referral when necessary and appropriate.
Ability to demonstrate proficiency in English including the ability to speak, write, comprehend aurally, and read at a level necessary to perform as a CASAC Counselor.
Participates in outreach functions to reengage client lost to contact and maintain census capacity.
Ability to complete all client progress update reports in a professional and timely manner to ensure coordination of care with client linkages to include the criminal justice system and entities.
Participates in program meetings and required trainings.
Ability to complete any reasonable tasks or requests made by a supervisor.

POSITION OVERVIEW
Program Director will provide clinical oversight and quality assurance to all unlicensed and licensed staff clinicians who provide substance and alcohol abuse services in chemical dependence within the integrated outpatient treatment program. The program director is legally responsible for adherence to all applicable licensing, supervision, billing, documentation requirements and overseeing the timeliness and quality of a staff member's clinical work. The program director is responsible for oversight of case record reviews, performance improvement, quality improvement and utilization review to include all required monthly reports.
KEY ESSENTIAL FUNCTIONS
Responsible for the daily operations and performance of the integrated outpatient programs
Manages clinic operations to budgeted and planned results
Interviews, hires, trains, evaluates, manages, counsels and terminates subordinate personnel
Works closely with staff via weekly supervision to ensure the completion of performance goals. Provides ongoing, regular feedback and performance evaluation to clinicians on the quality and timeliness of their clinical work as well as their caseload. Also solicits feedback from clinicians regarding supervision and whether needs are being met
Monitor caseloads and ensure the supervisees comply with all agency, clinical, legal and ethical practices.
Oversight and monitoring of billable hours of service required. Provide support, training, and remediation where needed to assist supervisees in meeting program's billing expectations and caseload requirements, charting, recordkeeping, documentation standards and all other quality assurance standards
Develops and implements policies and procedures in outpatient programs. Ensures all administrative and clinical policies and procedures are implemented appropriately and effectively. Also ensures all medical and other services to which patients may be referred are known to staff and made available to patients
Communicates with and insures compliance with regulatory bodies (OASAS, Joint Commission, DEA, SAMHSA, HIPPA) to include state and federal regulations
Assists with partnering opportunities, linkage/support agreements and other critical relationships to drive census growth and meet regulatory requirements
Works closely with Medical Director, Clinical Supervisor and Assistant Director in a variety of areas including but not limited to: compliance with federal and state rules; laws and regulations; compliance with Joint Commission, SAMHSA, OASAS and DEA; performance improvement initiatives; productivity goals; quality client care; customer service; operational enhancement; growth of client census; contractual requirements; standardization and conformity; best practices and plans for continuous improvement
Participates in community relations and development activities to drive and maintain census
Identifies and implements tactical steps to increase and retain census and productivity amongst staff
Works with clinic team to insure operations are prepared to handle increased census
Participates in political activities to keep company connected within appropriate circles such as client and community advisory board meetings
Identifies training needs and makes recommendations. Provides education and training opportunities to all staff to meet required training needs for program
Participates collaboratively as part of a multidisciplinary team, able to proactively collaborate with other supervisors and program director to ensure best possible training for supervisees and service delivery for clients
Participates in monthly clinical and medical staff meetings to include incident review committee, client satisfaction surveys, PI, QI, UR and chart review findings
Participates in Acacia meetings, program meetings and required trainings as required
Contributes to a six month and annual performance evaluations of clinical staff. Helps staff identify personal development goals for the year and collaboratively monitors progress toward these goals
Leads effort to identify, hire and train personnel with cultural diversity to meet the future clinic needs based on patient demographics-identifies cultural community leaders and networks
REQUIREMENTS
3+ years previous relevant clinical work experience in the chemical dependence field of which one year must be supervisory.
Must be a qualified health professional (QHP) with completion of certificate

POSITION OVERVIEW:
The Nurse Practitioner is responsible for working alongside our physicians and facilitating advanced primary and specialty nursing services to our patients. The primary responsibility will be to provide advanced primary and/or specialty nursing care to patients in the facility. Duties will include, but are not limited to, performing physicals exams, recording patient medical histories, creating patient care plans, administering treatment and diagnostic tests and observing the patient's response to treatment.
KEY ESSENTIAL FUNCTIONS:
Conduct psychiatric evaluations and assessments
Conduct mental status examinations and pharmacological evaluations
Prescribe medications, teach clients about side effects and monitor response to prescribed medications, including ordering of laboratory tests as needed
Administer injectable medications as needed
Complete all documentation (i.e. progress notes and psychiatric assessments) as required in a timely manner in the electronic medical record (EMR)
Review and provide recommendations for treatment plans
Provide expedited services to clients based on risk assessment status as clinically indicated.
Provide crisis intervention services to clients as needed
Communicate with other medical practitioners in order to coordinate treatment for clients.
Speak with managed care insurance representatives to obtain authorizations for prescribed medications when needed
Participate in staff meetings and case conferences
Collaborate with primary therapists, clinical supervisor and program director to discuss status of client progress in treatment and provide input/guidance on treatment planning and risk management issues.
Maintain an 80% show rate for clients who are scheduled for regular appointments
Provide a behavioral risk screening of the newly admitted residents, and share the findings with the primary care team. Provide a comprehensive psychiatric evaluation as requested.
Provide ongoing psychopharmacological management as clinically appropriate.
Recommend and provide behavior management interventions as appropriate and functions as a member of behavior rapid response team.
Collaborate psychiatric management with the members of the primary care team and the attending psychiatrist.
Participate in inter-departmental collaborative committees as needed and quality assurance and performance improvement activities.
Document clinical activities in the medical record, and maintain documentation of administrative activities.
Maintain a collaborative relation with an attending psychiatrist for all clinical activities and seek supervision as needed.
REQUIREMENTS:

POSITION OVERVIEW
Under supervision of the Senior Administrator, the Human Resources Coordinator/Program Assistant will support the administrative duties of the Program including providing Human Resources functions. The Human Resources / Program Assistant must be able to work effectively in a team environment as well as work independently, establish workload priorities, manage multiple tasks, take initiative and work under pressure to perform and complete a high volume of work with speed and accuracy to meet established deadlines.
KEY ESSENTIAL FUNCTIONS
Pre-Employment/Transfer/Promotion Management
Review all submitted Status Change Forms (SCFs) are accurate and approved by the appropriate personnel.
Review resume, job description and candidate assessment forms to establish appropriate compensation based on budget and/or union scale.
Ensure submitted candidates meet eligibility requirements (experience, education, degree, certification/licensure).
Coordinates HR Representatives assigned to onboarding process for assigned portfolios.
Responsible for effective and efficient management of pre-employment processes under applicable regulatory agencies including but not limited to OASAS, OMH, DOH, DHOMH, DHS, OTDA, DOE, etc.
Assist in ensuring compliance with pre & post-employment background check requirements including but not limited to SEL, SCR, CBC, CHRC, etc.
Background Check Process
Responsible for timely and accurate processing any/all background check applications required by the regulatory agencies.
Ensure required candidates complete all background check documentation and provide all required information for submission and compliance.
Monitor and track the progress of background checks submissions until results are received.
Review background check results and immediately report adverse findings to SVP of HR and Director of Human Resources.
Promptly update appropriate regulatory databases and related systems to remove employees from active records upon separation of employment.
Ensure all completed background application documents are clearly scanned, saved, and correctly labeled in the HR Shared Drive.
HR Administrative Support
Works closely with the Human Resources Manager and Director of Human Resources to provide ongoing trainings to assigned portfolio.
Prepares weekly HR reports for all areas of responsibility.
Maintain knowledge and understanding of existing and proposed federal and state labor laws/regulations to ensure compliance.
Attend and actively participate in departmental, organizational or external meetings, trainings, workshops as required.
All other duties as assigned.
Prepare final employee file for all new hires ensuring proper scanning, filing, and labeling of all documents and HR forms consistent with departmental standards.
Program Support
Assist in monthly audits as needed.
Assist with various hiring projects, job fairs, specially assigned projects, filing projects and audits for Acacia Network.
Accepts incoming and outgoing calls, maintain accurate messages, and disseminates messages as needed.
Schedule meetings for leadership as needed
Reroutes, transmits, or delivers reports to all departments.
Acts as directed by Senior Administrator's office in providing support and communication between the Director's office and other departments within the facility.
Consult with department supervisors concerning the operation of the integrated outpatient program to assist in eliminating/ correction problem areas and/or improvement of services, including corrective action and performance improvement plans. Report such findings/solutions to the Director.

REQUIREMENTS
2+ years previous relevant experience in Human Resources required.
Bachelor's Degree in Human Resources Management, Organizational Development, Business Management or an equivalent related field preferred.
High School Diploma or GED equivalent required.
Working knowledge of HR functions including recruiting and hiring, benefit administration, employee/labor relations and compensation.
Familiarity with medical/dental/life insurance enrollment and administration.
Knowledge of federal, state and local labor laws.

Work closely with clients to reach an agreement on formal treatment plan that address mental health co-morbidity treatment, including improving adherence to recommended treatment.
Evaluate and provide therapeutic interventions in individual, and in group sessions, using evidenced-based practices.
Complete all paperwork requirements and statistical reporting to ensure program compliance with applicable intergovernmental oversight agencies.
Maintain accurate and complete records as required.
Attend professional meetings, educational conferences, and workshops, in order to maintain and improve professional competence.
All other duties as assigned by the director and clinical supervisor.
REQUIREMENTS
Previous relevant experience in a Mental or Behavioral Health environment required.
Master's degree in social work or an equivalent related field (i.e. LMHC, LMFT ) required
License or limited permit in Social Work, Mental Health Counseling or an equivalent related field required within 6 months of employment.
Ability to use business acumen to exercise good judgment and decision-making.
Ability to deal with sensitive issues with diplomacy and discretion.

POSITION OVERVIEW
Provide HIV counseling, testing, referral services, and education session to patients receiving services in the integrated outpatient treatment programs.
KEY ESSENTIAL FUNCTIONS
Conduct screening and intake with patients presenting for HIV Counseling and/or Testing within the integrated outpatient treatment program.
HIV Pre and Post Test counseling.
Conduct oral and/or intravenous blood testing for the detection of HIV antibodies, if necessary.
Provide information and referrals services to HIV positive individuals and high risk negatives.

REQUIREMENTS
NYS HIV Pre and Post Test Counseling Certification and/or Certified Prevention Specialist Certification.
Experience in drawing blood preferred.
Two (2) to five (5) years of experience in HIV/AIDS field.
One (1) year of individual and group counseling experience in a social service environment, particularly in HIV/AIDS related services.
Bilingual: Spanish strongly preferred.
Knowledge of HIV/AIDS and sensitive to the needs of HIV+ individuals and those at high risk of infection.
Aware of the varying cultures and ethnic diversity of minority communities, particularly in Hispanic cultures.
Understanding of issues experienced by active and recovering substance abusers.

POSITION OVERVIEW
Under supervision of the Nurse Supervisor/Manager (RN) and with the collaboration of the Program Director, the Licensed Practical Nurse (LPN) provides medical care and services for individuals residing in the residential treatment program for substance use disorder(s). Services include gathering and assessing health history, physical examination, minor office procedures, monitoring of daily medication administration, treatment, referral, counseling and follow up care and services.
KEY ESSENTIAL FUNCTIONS
Under the supervision of the program director, act as the health coordinator for the residential program.
Review referred potential client medical histories submitted by the intake department.
Assist with referring clients to appropriate health care provider specialists.
In an 820 residential program, assist with managing the symptoms of withdrawal for clients who are in the stabilization element of care, this may include referral to seek further medical attention.
Conduct medical assessments with all new clients.
Assist with the managing client prescriptions with various pharmacy (e.g., ChemRx).
Coordinates with the client's primary care physicians to inform decisions regarding management and treatment of problems identified and provides care as prescribed by physician.
Manage client care: follow up plan and monitor progress, determine when referral to another provider is necessary and make arrangements for further care, determine when emergency room visit is necessary and when illness is resolved.
Work with interdisciplinary team members regarding modification of treatment as necessary according to health concerns.
Monitoring the wellness center to promote organized and efficient medication tracking, refills, and disposal of medication, as needed.
Document all medications in electronic health record system.
May participate in staff meeting or professional development activities as needed.
Conducts and documents tuberculin implanting and reading within appropriate time frame, during admission and at annual examination.
Ensures the provision and coordination of staff teaching regarding prevention, transmission, risk reduction and risk elimination strategies for HIV/AIDS.
Provides educational health related materials at admission, as needed, at the request of a client or as medically/clinically indicated.
Implements nursing care effectively and with courtesy according to clinic policy and procedures.
Follows accepted standards of nursing practice in providing direct care, performing procedures, and using equipment.
Maintains infection control standards per policy and in collaboration with Medical Director.
Coordinates all necessary client services with interdisciplinary team members in the comprehensive care planning process.
Maintains accurate documentation of all nursing activities.
Assist in our OASAS programs in events of nursing shortages.
REQUIREMENTS
Must possess a High School Diploma or GED.
Minimum three (3) years practical experience in health care facility.
Must possess an active and unrestricted license as a Licensed Practical Nurse (LPN) in New York State.
Must possess current Basic Life Support (BLS) certification.
Must possess current NYS approved Infection Control certification (4.0 hours, valid for 4 years).
Must have specific training in chemical use, abuse and dependence specific to the services provided.
Must obtain Mandated reporter (2 hours) prior to hire date.
