Coordination of care. Educate family members/caregivers on patient clinical care to enhance positive outcomes. Preventative initiatives to protect quality of care for patient. Assess patients' post-acute care needs in collaboration with the Clinical Care Managers and the patients discharge care providers to ensure continuity of care when discharging home. Provide interim care to patients experiencing gaps due to call offs, vacations and staff shortages with varying acuity and scheduling needs. Provide one-on-one training to nurses in the field to ensure a smooth clinical and non-clinical transition assuring continuity of care when the Halo's Home Team exists the case.
