OUR CLIENT SERVICES MANAGER POSITION
ElderSAFE Client Services Manager is a key contributor to the ElderSAFE Center at Charles E. Smith Life Communities (CESLC). The primary focus for our Client Services Manager is to manage the ElderSAFE shelter admission process, collaborating with the resident, family members, the ElderSAFE team, and CESLC associates.
Duties include:
collaborating with admissions, accounting, social work, nursing and medical to evaluate clients for eligibility for temporary shelter admission
provide case management, coordinate services and safe protection of confidential information for shelter clients.
Essential job responsibilities:
Manages the ElderSAFE shelter admission process, and collaborates with the ElderSAFE team and CESLC associates, including: admission, accounting, social work, nursing and medical to evaluate clients for eligibility for temporary shelter admission, provide case management, coordinate services and safe protection of confidential information for shelter clients.
Input and review client information in the healthcare portal PCC.
Manages the helpline (Mon-Fri 9am-5pm) providing community referrals and resources to professionals and community members.
Participates in care plan meetings for shelter clients and coordinates safe discharge from the temporary shelter program.
Coordinates with internal team members and external partners to assist clients with medical, legal and other appointments during their shelter stay. Coordinate health care aides and transportation services to accompany clients as appropriate.
Coordinate with outside vendors to schedule and provide trauma informed services to shelter clients.
Provides safety planning to shelter clients in coordination with the ElderSAFE team and CESLC security team.
Sends out communications to campus partners via campus security alerts upon client admission and discharge.
Provides consultative guidance and training to CESLC associates on issues of abuse, neglect and financial exploitation.
Assists ElderSAFE team in training internal and external partners on abuse, neglect and financial exploitation
Participates in internal and external meetings, coalitions and conferences. ·
Maintains program data and assists in writing reports. ·
Provides occasional after-hours coverage, in coordination with ElderSAFE team members. ·
Performs other duties as required.
Required Skills and Experience
This position requires a BSW; Licensed Clinical Social Worker (LCSW) preferred.
Must hold State of Maryland licensure. License can be in pending status.
Two or more years of experience as a social worker providing direct services and case management to older adults in a health care setting, senior living community or Adult Day Care Center.
Experience working with survivors of elder abuse, domestic violence, family violence or a related field is preferred.

OUR RESIDENT ACCOUNTING SPECIALIST- MEDICARE POSITION
We are seeking a detail-oriented and experienced Medicare Billing Specialist to join our healthcare team. This role is essential in ensuring accurate and timely submission of Medicare claims, managing accounts receivable, and maintaining compliance with billing regulations. The ideal candidate will have a strong background in healthcare billing, particularly within long-term care settings and SNF Medicare Part A and B Billing.
MAIN RESPONSIBILITIES
Prepare and submit Medicare claims and invoices promptly.
Monitor claim acceptance and ensure timely processing.
Review delinquent accounts and generate reports to support bad debt reserves.

OUR BEHAVIORAL HEALTH SOCIAL WORKER POSITION
The primary focus of the Social Worker role.
The primary purpose of this position is to assess and support referred residents who demonstrate behaviors that can result in adverse outcomes for themselves or others and act as a resource person, providing behavioral strategies to their care team. The identified target population includes residents residing in Hebrew Home (Smith Kogod and Wasserman buildings). Services are provided in accordance with current federal, state and local standards, guidelines and regulations that govern our facility and may be, as directed by the Director of Social Work or Administrator, to assure the highest degree of quality care is provided at all times.
Ensure residents referred as the result of behavioral health concerns receive appropriate treatment and services to correct the assessed problem or attain the highest practicable mental and psychosocial well-being.
Ensure residents referred due to behavioral health concerns who display or have been diagnosed with dementia, receive the appropriate behavioral health services to attain or maintain his or her highest practicable physical, mental, and psychosocial well-being.
Support Hebrew Home in its provision of medically-related social services to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.
Create an environment of non-pharmacological behavioral strategies to address behavioral health issues related to residents/patients who have been referred.
Collaborate with the Care Team in identifying and implementing behavioral modification/intervention strategies with a focus on reducing use of psychotropic medications.

What will I do as a Maintenance Mechanic?

What is required in this position?
High school diploma is preferred; 1 year technical trade school or equivalent may be substituted for diploma.

What is required in this position?

Responsibilities
Performs initial and ongoing nutritional assessment of assigned dialysis patients in the clinic.
Provides education and resources to the patients and family members to support and maximize the nutritional status of the patient.
Monitors nutritional parameters and maintains documentation utilizing the RIMS dietary charting module per Advanced Dialysis Center policy, procedures, guidelines, federal, state, and local regulations.
Participates with other members of the multidisciplinary team in the development of patient care plans and participates in developing goals for patient dietary care management.
Performs nutrition related QA/CQI and encourages multi-disciplinary approach in the improvement of quality care.
Develops educational resources for patients as needed and maintains current information on nutritional product lines.
Participates in appropriate research activities as directed by the physician and approved by Advanced Dialysis Center.
Promotes a constructive approach to problem solving within the facility and takes an active role in QA/CQI process.
Coordinates nutritional in-service education for health professionals. May provide in-service education for community organizations if requested with the approval of the Director of Nursing.
Promotes team concept throughout all clinic activities; assists other staff in the performance of their duties in accordance with policies and as allowed by state regulations.
Qualifications
B.S. Degree in Dietetics or Food and Nutrition from an accredited university.
Current Licensure in Maryland as a Dietitian/Nutritionist.
A minimumof one to two years experience in clinical/ renal dietetics in a health care facility.
Effective communication skills.
Current knowledge of nutrition therapy.
