The Weekend Coordinator works from a recovery framework within the team providing individual and group psychotherapy using psychotherapeutic interventions (evidenced based practices preferred), cultural competence and trauma informed care to help individuals obtain their recovery goals.
Location: Walnutport, PA
Shift: Saturday/Sunday 8 am-8pm
Responsibilities:
Coordinates and implements the on-site and community-based clinical program to ensure that adequate and creative learning opportunities are available to individuals in recovery. (These opportunities reflect the interests, needs, and recovery goals of the individuals residing in the LTSR.) Participates in clinical program design as assigned.
Provides individual therapy and group therapy utilizing evidence based practices as assigned.
Supervises staff on assigned weekend shifts, including task completion/assignments, monitoring of responsibilities, and attendance.
Coordination and implementation of activities with residents, including educational, social, and leisure programs for which residents' input is elicited.
Completes detailed documentation for individuals in their clinical record and in other areas as directed. Satisfies compliance with regulations regarding record keeping and reporting.

Responsibilities:
Support the mental and physical health care of patients on an assigned patient caseload.
Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.
Provide patient education about common mental health and substance abuse disorders and the available treatment options.
Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.

Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.
Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.
Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Be comfortable utilizing Excel and Word and completing assessments and productivity trackers daily.
Time-management skills are a must and the BHCM needs to be able to manage their day effectively through calendars and the EMR system.
This role is PRIMARILY telephonic, however in person visits are done and the BHCM has to be comfortable completing them both ways.

As an Outpatient Therapist at Ellie Mental Health - Trooper, PA, you'll share our vision for improving and expanding access to quality mental health care and thrive in an environment driven by our core values of authenticity, humor, compassion, creativity, acceptance, and determination.
Ellie Mental Health was founded in 2015 by an LMFT and LICSW when they opened the first Ellie clinic in Minneapolis. Since then, the Ellie Fam has continued to grow with multiple locally owned and operated clinics across the country.
Ellie was proudly founded on the principle of destigmatizing mental health. The mental health industry is full of barriers, and we've made it our goal to fill the gap and find innovative ways to break down these barriers for the communities we serve.

Responsibilities Include:
Evaluate mental health diagnosis, create, and implement a treatment plan, complete ongoing documentation including further diagnosis, treatment plan reviews, and case notes according to company policy
Provide excellent customer service for clients and collaborate with a dynamic team to further the mission of filling gaps in our community
Utilize creativity in interventions to help clients achieve and exceed goals
Prepare and submit individual documentation for each session per company guidelines and protocol
Our goal is set you up for success and we will work with Full-Time clinicians to eventually reach a minimum of 25 client visits per week
Coordinate services with case managers, families, work personnel, medical personnel, other Ellie staff, and school staff as needed
Attend and participate in all clinical staff meetings and trainings
Required Qualifications and Skills:
Candidates are required to have a master's degree in one of the behavioral sciences or related fields from an accredited college or university.
Candidates must hold clinical licensure (LMFT, LPC, LCSW)
Required experience with completing diagnostic assessments, treatment plans and clinical case notes
Effective written and verbal communication skills
Ability to demonstrate and model stable, appropriate boundaries with clients
Ability to complete and submit documentation of services and other documents in a timely manner
Comfort and familiarity working with a diverse client base
Proficient in the use of Office 365 and Electronic Health Record systems (Valant experience a plus!)
Fully Licensed Clinicians will ideally be credentialed with insurance panels
Clinic Location: 2550 Eisenhower Avenue, Suite A-203, Trooper, PA 19403 (right by Audobon, Eagleville, East Norriton, and Norristown)
