Job Summary
Odyssey House Louisiana, Inc. is seeking a full-time SocialWorker for the Community Health Center. The schedule is Monday-Friday 8am-5pm. The SocialWorker will provide comprehensive assessment and diagnosis of behavioral health clients. The position will also assist primary care providers and Program Managers with implementing disease management programs; participate in the development, review, and evaluation of the family and/or client's plan of care; utilize information from various sources to identify patients who would benefit from disease management and case management services; educate and engage patients in their own care management; identify social and/or medical barriers that impede health outcomes; link patients to appropriate levels of care/appropriate and supportive services; work collaboratively with a multidisciplinary inter-agency team to facilitate achievement of desired treatment outcomes, monitor patient ED and hospitalization utilization rates; and utilize an Internet based care management system to track patient progress and health outcomes.
Responsibilities and Duties
It is the duty and responsibility of the SocialWorker to:
coordinate/facilitate patient care;
provide direct care to patients within the Scope of Practice for which the provider is licensed;
provide direct care to clients/patients to support the agency's department;
manage patient care under Best Practices, UDS recommendations, and as prescribed to maintain accreditation;
conduct thorough documentation of visits and medical orders to support billing charges;
complete billing components of visit to optimize revenue within dictated time frames;
work collaboratively and maintains communication with patient care team (providers, nurses, etc.) to provide effective, timely, and appropriate patient care management;
assist patients and providers to adhere to evidence-based treatment protocols for specific disease states;
educate patients and helps to engage them in their own care management, provide asthma and diabetes education;
monitor patient care by tracking patient charts, counsel patients during physician visits, follow up through telephone calls and home visits as needed, and utilize Internet based case management tracking system;
meet directly with patient/family to assess needs and develop an individualized care plan as necessary;
ensure/maintain plan consensus from patient/family, physician, and health care team;
collaborate with relevant community-based social services and health care organizations (socialworkers, home health care, school nurses, hospice providers, etc.) to address patient needs/barriers to improved health outcomes and to secure necessary care and equipment;
address/resolve system problems impeding progress; proactively identify and resolve delays and obstacles to expedite care;
utilize advanced conflict resolution skills as necessary to ensure timely resolution of issues;
collaborate and communicate with multidisciplinary team at all phases of planning process, including initial patient assessment, planning, implementation, interdisciplinary collaboration, and teaching and ongoing evaluation;
identify patients with need for Patient Assistance Program submission to pharmaceutical companies in order to offset the cost to patient (This includes assisting patient in filling out application, identifying needed documents to support income, assure provider licenses are submitted, application is signed by provider, manage the course of application, notify patient of arrival of medicine, and when need for refills occurs; log of applicants will be kept and submission of application and arrival of meds will be documented in log);
perform utilization management and quality screening for assigned patients;
identify at-risk populations using approved screening tool(s) and follows established care and reporting protocols;
monitor medical resource use on an ongoing basis and take action to achieve continuous improvement;
refer cases and issues to PCP and follows up as indicated; follow up on referrals from PCP as appropriate;
actively participate in clinical performance improvement activities;
participate in development, implementation, evaluation and revision of disease management tools as a member of the clinical change team;
attend local meetings as necessary;
participate in training as necessary to successfully complete job responsibilities, including training in disease management protocols, care management strategies, use of network software programs, and data analysis skills;
use data to drive decisions and plan/implement performance improvement strategies related to care management for assigned patients, including fiscal, clinical, and patient satisfaction data;
collect special study data for specific performance and/or outcome indicators as determined by program;
participate in discussions of key variances and outcomes that relate to areas of direct responsibility;
use data in collaboration with other disciplines to ensure effective patient management;
provide effective treatment planning and assist clients in successfully achieving goals;
evaluate crisis situations and apply appropriate interventions;
actively participate in meetings that support the integrated health care model to provide comprehensive care for clients;
assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration;
assist the primary care team in developing care management processes including guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease;
provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders;
teach patients, families, and staff, care, prevention, and treatment enhancement techniques
monitor the site's behavioral health program to identify and make improvement recommendations for problems related to patient service; and perform other duties as assigned.
Qualifications and Skills

Minimum qualifications include the following:
Master's in Social Work
LCSW unencumbered licensure
Greater than 1 year Clinical/UM/Discharge Planning/Home Care/Social Work/Ambulatory experience
Current CPR certification
Excellent interpersonal, communication, and negotiation skills

Strong organizational and time management skills
Ability to work independently
Familiar with medication-assisted treatment and including opioid treatment programs.

The School SocialWorker might serve on the Schools MTSS Team and as a member of the Pupil Appraisal Team. Essential Functions
Serve as member of the in-school Student Support Services Team (SSS Team) for initial and re-evaluations.

Provides individual and group treatment as indicated on Individual Education Plans (IEP) and documents all clinical activity.

Conducts functional behavioral assessments and creates behavior intervention plans (FBA/BIP) as needed for students.
Completes risk assessments for suicidal/homicidal ideation
Participates in annual IEP meetings and Manifestation Determination Hearings, as needed.

Functions of Madison High SBHC LMSW:
Works with the student family and other team members to provide needs assessment including gathering and assessing information regarding the client's needs, mental status, support systems, and available financial community and governmental resources.
Evaluates data gathered in terms of psychosocial plan of treatment and management and available social work programs.
Plans pertinent program that will provide maximum benefit for each student.
Provides on-site evaluation of students referred by other members of the SBHC.
Performs a variety of services such as advising on inter-relational problems, and alleviation of anxieties and/or fears.
Presents appropriate educational topics related to psychosocial health issues in the classroom.
Provides short-term counseling that is goal-oriented to meet specific need and to explore available options. This may include work with independence issues, identity crisis, body image, etc., involving family as necessary.
Maintains timely and pertinent documentation on counseling interventions, accurately maintains statistical data. Collects and maintains information for quality assurance indicators as required.
Ensures confidentiality of interview, treatment and documentation.
Provides support to obtain needed services and to negotiate and promote the student's concerns within the health care team as well as the community.
Responsible for coordination of mental health services. Works closely with all members of the health care team to develop a comprehensive treatment plan to meet the identified needs and to revise and drive the plan as needed to ensure optimal treatment benefit.
Acts as a bridge between the health care team and the community to link services and form an alliance whose goal is to provide a diverse network of assistance to meet the adolescent's needs.
Acts as an information source concerning available community services. Functions as a consultant to school personnel in matters concerning psychosocial needs of students.
II.Critical Responsibilities of Madison High SBHC SocialWorker:
Continues growth and self-improvement as evidenced by participation in in-service programs, workshops/meetings, and other educational offerings required by the SBHC, the LABSWE Board, the LEA and the Office of Public Health.
Maintains required professional certification, competency and growth through continuing education.
Implements established programs required by the state and/or LEA.
Maintains accurate and timely records/reports in area of specialization.
Accounts for fiscal record keeping and reporting.
Participates in personnel accountability program of the local system.
Fosters positive relationships with parents and community
Reveals a positive attitude and set appropriate models for students as evidenced by appearance, relationships with students and teachers.
III. Other Requirements:
Adheres to the regulations, policies, and procedures established by the Office of Public Health, State Board of Elementary and Secondary Education, and Madison Parish School Board
Performs other such duties and perform responsibilities as assigned by the director of the SBHC

Key Responsibilities
• Comprehensive Psychosocial Assessment
• Conduct in depth psychosocial evaluations for all patients within 72 hours of admission, identifying strengths, challenges, and support systems.
• Collaborate with patients and families to establish individualized goals for rehabilitation and discharge.
• Care Coordination & Team Leadership
• Serve as the primary point of contact for patients, families, and the interdisciplinary team regarding care planning, progress, and discharge needs.
• Lead and document regular team conferences, synthesizing input from therapy, nursing, and medical staff to ensure unified, patient centered care.
• Facilitate family conferences and provide ongoing counseling for psychological, financial, or family issues related to disability or recovery.
• Discharge Planning & Community Integration
• Develop, coordinate, and implement comprehensive discharge plans, arranging for community services, equipment, and follow up care as needed.
• Ensure smooth care transitions by communicating discharge plans to families, payors, and receiving facilities, and by providing post discharge follow up.
• Regulatory & Quality Leadership • Ensure compliance with Medicare (CMS) guidelines, including documentation, utilization review, and the QIO appeals process.
• Participate in quality improvement initiatives, hospital committees, and community education programs to advance patient outcomes and organizational excellence.
• Educate the interdisciplinary team on cost containment, benefits plans, and evidence based practices.

JOB SUMMARY:
The SocialWorker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and helps coordinate a plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members.
MINIMUM QUALIFICATIONS:
LMSW required - must be under supervision for LCSW; LCSW preferred.
Working knowledge/experience in case management, payors and reimbursement.
Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.
Organized and efficient in establishing priorities and performing tasks within specific timeframes.
Self-motivated requiring minimal supervision.
Views problems as a challenge and investigates alternatives which demand creativity, flexibility and the ability to work under stress.
Responsible for maintaining strict confidentiality of all verbal & written information and documents. Familiarity with and appropriate use of community resources.
Preferred Qualifications:
LCSW preferred. Experience in psychosocial and therapeutic counseling preferred.
