Assist case manager by explaining purpose of linkage and reason for specific linkage recommendations.
Provide culturally appropriate, competent and comprehensive support in accordance with the individual's age, gender and gender identity, sexual orientation, race, ethnicity, religious beliefs and culture.
Assist staff in developing policies and procedures by drawing from personal experience in recovery and personal interactions with providers.

ESSENTIAL DUTIES & RESPONSIBILITIES
Works with the teacher to develop curriculum and daily lesson plan and uses the lessons/materials provided by the teacher to assist in teaching the lesson.
Works with the whole class for lessons, as well as works individually with students to maintain a positive atmosphere in the classroom.
Works cooperatively with other Teacher Aides and/or volunteers.

The Crisis Triage and Stabilization Supervisor will assist in developing C4's Cook County Crisis Triage and Stabilization Program at the shared Collaborative Bridges clinical site in West Garfield Park at 4223 W Lake Street. The Agency Nurse provides comprehensive case management and nursing services to C4 clients diagnosed with medical disorders and severe mental illness. He or she also oversees and coordinates C4's infection control program and provides appropriate training to C4 staff concerning infection control.

Clinical Duties
Provides routine nursing care to clients including, but not limited to, administration of STAT oral and injectable psychotropic medication and long-acting antipsychotics, evaluation of physical condition, and measurement of vital signs

Pathways to Success is a program for Medicaid enrolled children under the age of 21 in Illinois who have complex behavioral health needs and require intensive services and support. About the role:
As a member of the Community-Based Clinicians Team, the Youth Clinician will provide trauma-informed therapy services to children, adolescents, and families, as well as some individual adults. Interventions include a range of therapeutic and behavioral services for individuals and their families with trauma and mental health needs. Services are flexible to the family's and individual's needs and delivered in the community whenever possible. The Youth Clinician will provide services in a culturally sensitive and appropriate manner, as well as contribute to equity goals by prioritizing clients from communities where disinvestment has created or exacerbated mental health issues.

The Collaborative Bridges team is an innovative care coordination pilot program to connect individuals and families with substance use and mental health needs upon their discharge from hospitals and provide support with therapy and care coordination services until linked to long term community providers or consumer needs are resolved.
Duties and Responsibilities
