About the Role
We're looking for a builder - someone who wants to take a messy zero-to-one commercial motion and turn it into something that works, fast. As our first Head of Provider Growth, you'll own provider adoption end-to-end: testing channels, refining targeting, knocking on clinic doors with our first reps, and scaling the system once we know it works.
This role is perfect for someone who has built growth engines before (in startups, agencies, or their own companies), loves rapid experimentation, and wants to have massive impact in a short amount of time. What You'll Do - Impact in Your First 3-6 Months
Own the growth target. Build and execute a plan to reach meaningful PA volume across provider settings.
Run fast experiments. Test messages, channels, outreach methods, and install flows to identify what moves the needle fastest.
Get your hands dirty. Shadow field reps, visit clinics, talk to staff, learn the real workflows, and refine our pitch based on firsthand insight.
Target smarter. Partner with our Head of Strat/FinOps to map specialties, geos, and practice types with the highest PA density and frictionless install paths.
Create momentum. Build a simple but effective engine: outbound sequences, enrichment, list building, mini‑landing pages, CRM flows, funnel dashboards.
Produce early proof points. Deliver data that shows what works, what doesn't, and where we should double down.

SUMMARY OF THE POSITION:
With Supervision, coordinates activities for infants/toddlers as guided by Head Start Performance Standards, Service Area Written Plans, Procedures, and Policies, and California State Community Care Licensing regulations. Job Description
Job Title: Home Dialysis Care Giver (NxStage Experience Preferred) Schedule: 4 days per week Compensation: Competitive - based on experience About the Role We are seeking an experienced and dependable dialysis-trained professional to support an elderly gentleman who completes his dialysis treatments in his private home. This is not a facility or multi-location role. You will be the primary support person for one patient, four days a week, helping him maintain safety, comfort, and stability during each treatment.
The patient uses the NxStage home hemodialysis machine, and we are looking for someone who is already familiar with dialysis equipment, cannulation, and treatment protocols - or someone with strong dialysis experience who is confident transitioning into the home-care environment.
Key Responsibilities
• Set up, operate, and monitor the NxStage home dialysis system during each treatment session
• Ensure adherence to dialysis protocols, sterile technique, and infection-prevention standards
• Observe and document vital signs and treatment responses
• Troubleshoot machine issues and escalate clinical concerns appropriately
• Maintain a clean, organized treatment area within the home
• Provide calm, compassionate support and companionship during treatment
What We're Looking For
• Experience with home hemodialysis or in-center hemodialysis (NxStage strongly preferred)
• Competence in cannulation and dialysis machine operation
• Reliability, professionalism, and strong attention to detail
• Ability to work independently in a private home
• A calm, supportive presence - patient is elderly and values consistency

DEFINITION:The Companion is a person who possesses adequate communication skills and abilities to provide companionship to clients in the home setting. The Companion does not render any hands-on care to the client. LINE OF AUTHORITY:Reports directly to the registered nurse clinician. QUALIFICATIONS:
Completion of at least the ninth-grade level of secondary education preferred.
Documentation on file of having received, training in topics related to human development and interpersonal relationships, nutrition, marketing, food storage, use of equipment and supplies, planning and organizing of household tasks, and principles of cleanliness and safety.
Minimum of 18 years of age.
Demonstrates maturity and a sincere interest in the client.
Demonstrates good personal grooming and health habits.
Must have reliable transportation.
Demonstrates mental acuity high enough to write, read, and adequately communicate with clients.
DUTIES:Renders companion services to clients in the home setting:
Listening
Reading to client
Supervising environment
Communicates verbally
Recognizes life-threatening situations, and acts according to protocol
Offers emotional support
Accompanies client to outside appointments
Provide light housekeeping tasks, such as preparation of meals, and laundering the client's personal garments.
Performs casual cosmetic assistance, i.e. brushing the hair, assisting with the application of makeup, filing and polishing nails (not clipping).
Stabilizes the client when walking as needed.
Maintains a chronological written record of services.
Report any unusual incidents or changes in the client's behavior to the registered nurse.
Maintains confidentiality of client and agency matters.
Submits required documentation in a timely manner.
Attains quarterly goals established by the DON at quarterly evaluation.
Performs other duties as assigned.
