Position Specific Duties/Responsibilities:
Maintain a case load of up to 12 families, providing direct face to face contact with youth and families in accordance with Erie County C-SPOA and Erie County Department of Social Services.
Work closely with families and youth to identify strengths and needs and build a Child Family Team (CFT) of supportive people that include natural, community and system supports.
Organize and facilitate all Child and Family Team Meetings (CFT) to develop, implement and evaluate a comprehensive Plan of Care and effective Crisis and Prevention Plan.
Refer and link all family members to culturally competent , community based, formal and informal supports that address needs identified in the assessment process and help families meet their established goals and the expectations of ECDSS and Family Court . Ensure all contracted services provided through the HFW Vendor Network are delivered timely and effectively to families.
Communicate and collaborate with all necessary providers and supports related to the planning, coordination and care for youth/families, including consistent contact with the Erie County DSS Case Manager
Assist the family and their CFT to achieve their goals, learn and develop skills, and create a support network that sustains success and allows for timely transition from High Fidelity Wrap.
Ensure accurate, thorough, and timely completion and filing of all required documentation in Fidelity EHR and/or CONNECTIONS, according to program and contractual expectations
Attend all required trainings and meetings provided by Gateway-Longview and Erie County Wrap Around.

Position Summary
The Day Habilitation Specialist (Day Hab Specialist) will assist individuals with developmental disabilities to maximize their personal and vocational potential in their center and community. The Day Hab Specialist provides direct care support and assists in the implementation and documentation of each individual's program plan. Position Standards:

Position Specific Duties/Responsibilities:
Quickly engage families in a face-to-face in-home contact to identify precipitating safety & risk issues, family strengths/needs to develop a safety plan and establish a comprehensive service plan to achieve treatment outcomes.
Work intensely with referral sources, community service providers, and all family members to ensure collaborative efforts toward established service plan. Build upon support networks for each family and ensure adequate and stable supports are in place to stabilize families, address immediate needs and minimize safety and risk concerns.
Maintain intensive in-home contact, service provision, and therapeutic rapport with families which includes face-to-face support, education, advocacy, case management, and crisis intervention services, for a minimum of 10-15 hours per week per family
Ensure accurate, thorough, and timely completion and filing of all required documentation in CONNECTIONS according to contractual and agency requirements.
Develop, teach, model and evaluate effective, culturally competent, trauma informed techniques and interventions with parents/caretakers and children/youth.
Ensure all court mandated and/or recommended services are offered, utilized, and maintained via referral/linkage or provided directly to minimize risk, while enhancing the family's safety, permanency, and overall well-being
Actively participate in required/offered meetings and training opportunities.

Master's Degree in a human services related field

Compensation and Benefits
The expected salary range for this position is between $260,000 and $390,000.

Provide individual, group, and family therapy services at a school-based satellite clinic location and main clinic sites.
Assess each client's mental health strengths and needs in accordance with the most current edition of the DSM / ICD 9 codes and in accordance with pertinent regulatory and agency standards

Who We Need
We are currently seeking a full-time Speech Language Pathologist for our Middletown Education Program. This is 32.5-hour position 48 weeks a year. (School year will be 42 weeks with a 6-week summer program) This position is crucial to the development of children of all ages.
Schedule: Monday- Friday 8:30am-3:30pm, 35 hours per week, 48 weeks per year (which includes a 6-week summer program)
Annual Salary: $65,271.20-$66,271.20.
Responsibilities:
Initial evaluation/periodic re-evaluation
Program planning and implementation of program
Scheduling of caseload in conjunction with classroom staff and other related services
Coordination of program between student, counselor/teacher and case manager

Provide advanced ECG monitoring while assessing patient status through ECG strip analysis and arrhythmia interpretation.
Identifying and responding to potentially life-threatening rhythm changes, initiating codes, recording rhythms during codes.
Performing continuous EKG monitoring with accurate interpretation of cardiac rhythms and dysrhythmias, troubleshooting monitoring problems, and appropriately responding to all monitor alarms.
Perform electrocardiograms in multiple settings including proper electrode placement.
Communicate appropriately and clearly with charge nurses, co-workers, and physicians.
Enter and verify orders in the computer system.
Retrieve laboratory results from the computer system and bring results to the attention of the charge nurse.
Ensure documentation meets current standards and organizational policies.

Requirements:
Dysrhythmia Course.
High School Diploma or equivalent.
