This position will report to the Director of Hospital Operations and work closely with the System Director for Quality & Patient Safety. The Quality Analyst will apply performance improvement and quality principles to outcome and process measures within the Inpatient Rehabilitation Facility (IRF) care setting. This position will also support interdisciplinary teams to achieve the Commission on Accreditation of Rehabilitation Facilities (CARF) accreditation/redesignation, provide onboarding education to the therapy department, and work collaboratively with IRF leaders to promote CARF survey readiness. This role will help create and collaborate action plans for process improvement for any required findings for hospital site surveys.
Job Responsibilities:
Quality Analyst responsibilities:
Assist with communication to the interdisciplinary team either through discussion or in writing (e.g., formal queries) regarding missing, unclear, or conflicting health record documentation, and clarifies the information as warranted.
Educates providers about identification of disease processes that reflect complexity and acuity to facilitate accurate application of code sets.
Gathers and analyzes information pertinent to documentation findings and outcomes and uses this information to develop action plans for process improvements.
Analyzes quality trends and collaboratively creates action plans for performance improvement surrounding acute care transfers/hospital readmissions with consideration at the hospital, unit and patient specific trend levels.
Patient safety indicators including falls, healthcare associated pressure injuries, infections
Functional improvement outcomes
Other quality measures as identified or regulated
Identifies performance improvement opportunities and develops processes, procedures, and policy to support these initiatives.
Evaluates workflow processes utilizing lean strategies, process mapping, and performance improvement principles to identify opportunities and efficiencies.
Completes audits to ensure processes and policies are being followed
CARF Accreditation Responsibilities
Perform gap analysis evaluation on a regular schedule to determine compliance with CARF standards.
Demonstrate expert knowledge of regulatory compliance of IRF hospital CARF programs.
Record and maintain documentation to validate compliance with program standards.
Coordinate the interdisciplinary teamwork and organization of data collection by effectively collaborating with clinical leadership teams.
Provides on-site support as the lead for surveys related to regulatory compliance
ORGANIZATIONAL/SERVICE MANAGEMENT
Submits required reports in a timely and professional manner.
Identifies variances to meeting departmental goals.
Proactively initiates action plans and develops controls to track variances.

Assists patients on/off exercise tables or equipment, or in/out of whirlpools. Provides general supervision/assistance with transfers, gait, and exercise. All the above are at the discretion of the therapist/assistant.
Prepares whirlpool baths, and dressings, and assists with dressing changes. Adheres to current infection control practices, including cleaning of whirlpool equipment.
Applies hot or cold packs under Therapist/Assistant direction. Prepares patients, guest, clients, and residents using proper positioning and draping, prior to equipment use including ultrasound, electrical stimulation, UV irradiation and therapeutic exercise equipment under therapist direction.
Monitors/reports response of patients to treatment and informs appropriate supervisor. Recognizes emergency procedures and reacts appropriately.
