Job Summary: Responsible for identifying and recruiting qualified foster parents to provide safe, nurturing, and supportive homes for children and youth in the Enhanced Family Foster Care program. Engages with the community, conducting outreach, and ensuring all prospective foster homes follow NYSOCFS regulations. Duties and Responsibilities: · Responsible for developing new EFFC foster homes and identifying foster parents with an increased focus on prospective foster parents that are interested in working with teens and sibling groups. · Develop and implement recruitment strategies to attract potential foster/adoptive parents through community events, social media, and partnerships with schools, faith-based organizations to increase awareness of the need for foster homes. · Responsible for scheduling monthly recruitment events. · Conduct initial screenings and orientation for prospective foster parents. · Respond to inquiries and guide applicants through the licensing process. Tracks all inquiries from agency/community recruitment events, ACS WISHLINE, and BINTI. · Responsible data collection and maintaining the confidentiality of all documents and information including scheduling prospective foster parents for fingerprinting, submitting SEL, and SCR clearances. · Assists in facilitating NTDC and TRIPP training for prospective foster parents. · Attend all ACS, CCP/SICCAN recruitment events, and participate in monthly recruitment events in Staten Island and Brooklyn. · Assists in coordinating foster parent retention events, including but not limited Foster Parent Recognition and holiday parties. · Achieves monthly recruitment targets set by the Home Finding Director Requirements Skills, Knowledge, and Abilities: · Excellent oral, written, and communication skills. · Microsoft 365 proficiency and knowledge of Connections · Excellent interpersonal skills. · Ability to perform the essential functions of the job with or without reasonable accommodation. Work collaboratively with survivors to define case goals, objectives, and action plans. · Meet program expectations for client contacts. Will contact client within 24 hours due to the critical nature of the work. · Advocate on behalf of clients to overcome systemic barriers related to court, immigration, housing, public benefits, etc. · Facilitate/co-facilitate groups for clients. · Develop content, schedule, and participate in monthly outreach events and document attendance of participants. · Collaborate with internal and external partners to advocate for survivors in court matters, housing matters, public assistance, immigration, etc. · Complete all data collecting and reporting promptly. Job Summary: The Educational Specialist works with up to 50 students within the Foster Care program at any time. Job Summary: The Care Manager (CM) works under the direct supervision of the Care Manager Supervisor and plays a critical role in achieving the overall goals for coordination, integration, and partnership in the delivery of care to children, youth, and families with complex medical, developmental, and behavioral needs between the ages of 0-21. Completes all program/case relevant documentation in the Electronic Medical Record (EMR) within the assigned deadlines. Requests and obtains the necessary documentation in a timely manner from service providers and/or families. Partners with multi-disciplinary team to determine service needs and actively participates in ongoing communication with all partners in care. Participates in service coordination meetings and team conferences. Provides ongoing collateral contacts and follow up on coordinated services. Facilitates the transitional care process and discharge planning, when applicable. Attends and participates in individual and group supervision. Carry a caseload as determined by the complexity levels of the cases (20-30 cases) with the ability to travel to complete face-to-face visits via car or public transportation.
