The Care Coordinator II is responsible for coordinating multiple aspects of the youth's and/or families' care. This role provides the coordination and delivery of high-quality psychosocial care to youth and families. The Care Coordinator II works with other care providers and community partners to ensure that the overall needs of the client(s) are met. The Care Coordinator II may also be assigned to lead projects or mentor program staff.
This position will be on our Monroe Region Youth ACT team. Hillside's Youth Assertive Community Treatment (Youth ACT) program serves children/youth with serious emotional disturbance who are returning home from inpatient settings or residential services, who are at risk of entering such settings, or whose needs have not been met in more traditional community-based settings. Referrals for Youth ACT must come from Children's Single Point of Access (C-SPOA), a centralized referral system for children with serious emotional disturbance who need intensive mental health services to remain at home or in their community.
This will be a Tuesday-Saturday, full-time (40 hours) position, hours are flexible based on client need. As stated below, there will also be on-call expectations for this position.
Essential Job functions
Coordinate multiple aspects of the youth's and/or families' care.
Assess the youth and/or family's needs and strengths while continuing to support development of the individual service plan in collaboration with the youth and family(ies), and/or multi-disciplinary treatment team.
Implement strength-based service plan including community based visits/home visits and assessments, identifying and coordinating collateral resources, recognizing gaps and needs, assessing results, and communicating with the multi-disciplinary treatment team.
Utilize experience, education, age-specific knowledge and cultural competence in order to assess and support youth and/or families to achieve positive outcomes.
Coordinate and deliver psychoeducational training for youth and/or families and groups in a variety of settings based on identified needs.
Meet with youth individually to assess and teach skill development and report on their progress.
May organize and facilitate skill-based groups for youth.
Develop, maintain and update all required documentation in a timely manner utilizing effective written and oral communication.
Provide or arrange for transportation of youth and/or families to and from medical, behavioral health, or other treatment related appointments as needed.
May be assigned to lead projects or mentor program staff.
On-call responsibilities for Youth ACT programs.

Education & Experience
Bachelor's degree required.

The Counseling and Runaway and Homeless Youth (RHY) Department provides a continuum of essential services to the most vulnerable youth in our community, including prevention counseling, two crisis nurseries, an emergency shelter, permanent and transitional housing, a food and clothing pantry, street outreach, and support for minor victims of commercial sexual exploitation.
The RHY Prevention Counseling Program Manager is responsible for the management and supervision of the RHY Prevention Counselors, including Community Counselors, Triage Counselors, Youth & Family Counselors, and Safe Harbour Advocates. They are responsible for reviewing and monitoring OASAS contract objectives, with a focus on program outcomes, as well as maintaining the New York State Safe Harbour database. The Manager plays a leadership role in planning, prioritizing and evaluating services, team outreach activities and linkages with other providers/collaboratives/committees. The Manager will be responsible for the on-going assessment of the team's service, identify expansion potential, assess community or catchment area needs and formulate possible responses. The Manager attends weekly Case Review and works closely with the other RHY Managers.
The RHY Prevention Counseling Program Manager participates in the On-Call RHY Supervisor rotation (for 1 week, every 5-6 weeks). This emergency response system is intended to provide after-hours and weekend support to the 24-hour residential and crisis programs.

Transcribe all medical orders accurately and efficiently and carry out orders for treatment as prescribed.
Report any significant finding to the appropriate medical provider and guardian when appropriate and carry out the prescribed intervention.
Support all members of the team in support of the care plan(s); assure quality care is delivered in an effective, timely, and team-based manner.
Communicate respectfully, accurately and clearly both in verbal and written form with all team members and families.
Administer nursing care for medically prescribed orders for treatment.
Administer medication safely, accurately and in accordance with New York State and agency guidelines. Monitor patients for any reactions or adverse side effects and report medication side effects to prescriber.
Administer first aid and CPR during emergency situations and coordinate patient care until Emergency Medical Services (EMS) arrives or the youth is transported to hospital emergency department.
Triage phone calls regarding health-related concerns of clients.
Oversee and supervise care provided by the LPN and other health office staff to ensure proper nursing care.
Provide assessments, monitoring and care to the youth in the event that a physical restraint is needed, and ensure all regulatory requirements for managing and documenting restraint are followed.
Maintain timely, accurate, and detailed documentation and progress notes in the patient medical records.
Independently complete commitments and take responsibility for actions; recognize and fix errors and omissions.
Advocate on behalf of youth to establish appropriate health care.
Consult with pediatrician or other members of the health care team to follow up on medical concerns, including physical and psychiatric concerns. Implement follow up care recommendations.
Identify needs and provide education to staff and youth as needed on topics relevant to child and adolescent health, such as health education, disease prevention, and health improvement programs. Monitor to ensure proper care and follow up needs are addressed.
Understand and adhere to all federal, state, and local statutes, regulatory agency standards and Hillside policies including but not limited to all federal, state, and local statutes, regulatory agency standards, and Hillside policies, including but not limited to, Office of Mental Health (OMH), Office of Child and Family Services (OCFS), Council on Accreditation (COA) and New York State Education Department (NYSED).

Education & Experience
Successful completion of an accredited Associate or Bachelor's Degree Nursing Program required.
Minimum of 1 year related work experience required.
SPECIAL REQUIREMENTS
Active, unrestricted RN license in NYS required.
Unrestricted, valid NYS driver's license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.
Basic Life Support (BLS) training within 60 days of hire and recertification annually.
