Job Description
The Senior Housing Coordinator provides day-to-day supervision to the Housing Coordinators and oversees the activities assigned to this unit. Guides the Housing Coordinators in achieving the goal of attainment of resident
permanent housing goals by ongoing assessment of resident housing needs and preferences and the development of
housing opportunities in the community.
ESSENTIAL TASKS:

1. Provide administrative and clinical supervision to the Housing Coordinators in accordance with agency and
division policy.
2. Establish and maintain department work schedules in accordance with agency policy and assignments of staff.
3. Responsible for managing the completion of time clock edits of department staff. .
4. Ensure department staff compliance with the agency's time and attendance policy; recommends disciplinary action
for those out of compliance

6. Manage the completion of department performance evaluations, recommends department staff for the employee
recognition award and promotions.
7. Administer department disciplinary actions in accordance with agency disciplinary policy, conduct disciplinary

8. Review authorized department staff positions and personnel budget, submit Requests for Personnel (RFP), on
vacant staff position(s), interview candidates for employment in accordance with agency policy and employment
law; recommend changes to authorized staffing patterns and personnel budget.
9. Ensure that department staffs have attended required in-service training and schedule employees to attend both inservice
training and training provided by vendors; conduct training on clinical subjects, charting, fire safety plan, fire
drills, emergency plans and procedures, building security, volunteers and interns, and site specific exposure control
plan.
10. Manage departmental employee accident reporting and initial submission of workers' compensation claims.
11. Monitor department employee morale and the climate of the work environment to stop or eliminate sexual
harassment or any form of employee harassment or employee discrimination
12. Monitor department employee accrual totals, approve or recommend approval of employee requests for time off,
and notify the Department of Human Resources when an employee should be placed on or taken off Family and
Medical Leave (FMLA); maintain contact with staff while out on FMLA
13. Conduct department employment interviews that meet federal, state, and agency regulatory guidelines.
14. Inform staff at meetings and in memoranda of agency policies, directives and guidelines affecting the facilities
management operations and personnel.
15. Conduct weekly meetings with Housing Coordinators to address: (a) length of stay, including barriers to
attaining employment and housing goals; (b) the clinical status of residents; (c) service plan reviews; and
(d) coordination with other staff.
Can Perform all functions of a Housing Coordinator:
1. Develop and maintain relationships with housing providers and the Veterans Administration.
2. Develop and maintain community resources guides for each of the five boroughs.
3. Find suitable permanent housing in the community.
4. Serve as a liaison between the residence and landlords regarding move-in schedules, leases, rent and utility issues;
5. Collaborate with case management staff to understand the specific housing needs and preferences of residents and
act accordingly when searching for housing options.
6. Serve as liaison to the Veteran's Administration to identify housing for residents that qualify for VA housing.
7. Participate in ongoing reviews of existing linkages for their adequacy and seek to extend the program's and ICL's
linkages as part of Continuous Quality Improvement.
8. Document efforts to locate and secure housing for residents.
9. Assist in the orientation of new personnel when requested.
10. Maintain up-to-date, accurate individual paper and electronic case records as directed by the Program Director..
11. Comply with attendance and timekeeping rules and report reliably and regularly to work on an on-going basis.
12. Attend regularly scheduled clinical meetings, staff meetings and supervision as well as in-service training and
development activities.
13. Review the staff communications log and make entries concerning essential elements of information designated by
program management.
14. Obtain and maintain CPR and First Aid certifications.
15. May have on-call responsibilities.
16. Comply and promote compliance with all applicable laws, regulations and agency policies helping to strengthen and
maintain an ethical organizational culture.
17. Immediately report serious incidents, allegations of abuse, incidents, or sensitive situations and completes incident
reports in accordance with agency policy.

Under the general supervision of the Team Leader/Program Director, and the guidelines provided by the NYS OMH for the Mental Health Comprehensive Medicaid Case Management Program (MH CMCM), functions as part of a multi-disciplinary treatment team with shared caseloads, assists participating individuals who have serious and persistent mental illness in developing and maintaining viable living, working and social situations in the community by obtaining needed medical, social, psychosocial, educational, financial, vocational and other services. Has lead responsibility for integrating family goals and services with the tasks of all team members and for providing family psycho-education individually and in groups. Participates in the provision of crisis intervention services to participants 24 hours a day, seven days a week.
THIS IS A FIELD OPPORTUNITY
ESSENTIAL TASKS
* Provide counseling, treatment, rehabilitation, case management and support services in the community (homes, shelters, street), as part of a mobile multi-disciplinary team, to mentally ill individuals who have a history of decompensation and relapse as a result of poor follow through with outpatient treatment.
* Assume primary clinical management responsibility for 10-15 adults with serious and persistent mental illness, who may also have a co-occurring substance abuse or other disorder, within the context of a 68-person-caseload ACT Team.
* Assume lead responsibility within the multi-disciplinary team for integrating family goals and services with the tasks of all team members and for providing family psycho-education individually and in groups.
* Assist consumers in attaining or maintaining housing, employment, functional social relationships, and relief from psychiatric symptoms and side effects through medication management support.
* Assertive engagement and provision of mental health services in the community where the service population lives - up to 80% of weekly work schedule may involve fieldwork.
* Continuous responsibility (24-hour) for people enrolled in the ACT Team caseload.
* Provide all services in a culturally relevant and competent manner.
* Maintain accurate and up-to-date Case Records, including but not limited to progress notes, service plan and service plan reviews, assessments and contact logs within keeping of all-applicable OMH, Medicaid, and ICL Regulations and Guidelines.
* Provide support and psychoeducation to family members, friends and other collateral individuals.
* Provide referrals and engagement support for services not directly provided by ACT Team, e.g., medical services, participation in psychosocial clubhouses and self-help groups, vocational training and placement, etc.
* Provide accurate and timely reports, within federal, state, and city regulatory and agency confidentiality guidelines, to other providers and agencies, e.g. housing, vocational, criminal justice, child welfare, etc.
* Participates in individual and group supervision, treatment team meetings, case conferences, in-service training and other staff development activities as assigned.
* Observes all ICL, OMH, and all other applicable rules, procedures or laws regarding the reporting of incidents, including the reporting of alleged child abuse and neglect.
* Perform other related duties as required and assigned by the Team Leader/Program Director.
Minimum Qualifications:
Master's Degree in one of the following disciplines recognized by the New York State Office of Mental Health as qualifying a staff person to be counted as professional clinical staff: Social Work, Psychology, Occupational Therapy, Rehabilitation Counseling, Pastoral Counseling, Creative Arts Therapy, Therapeutic Recreation. Applicable professional license in field issued by NYS Education Department strongly preferred. Plus, specialized experience or training of at least one year in family assessment and intervention, psychoeducation and/or other family support services, including cognitive-behavioral strategies..
