Identifies history of trauma, adversity and/or toxic stress that influence presenting problem(s), functioning, and family interaction styles. Determines the level of immediate and future risk of abuse or neglect to children. Identifies cultural factors that influence strengths, functioning, and family interaction styles. Schedule and conduct therapeutic counseling sessions with individuals, families and treatment teams based on cases assigned and at the established acuity for each family. Appointments must be made at times that families can participate; evening work is expected. Work with supervisor to adjust acuity when the needs of the families change. Uses trauma-informed, focused interventions to engage clients. Educates clients, family members, and other providers about healthcare conditions, prevention, available treatments, illness and whole health self-management, peer support and recovery. Manages behavioral health crises through appropriate interventions. Respond to perceived resistance in a non-confrontational manner and intervene to help child regain control and reduce risk at it pertains to the position held. Consistently records client progress in relation to treatment goals. Provides accurate, concise narrative when needed on any documentation, including progress notes, treatment plans, behavior management programs and discharge planning on an accurate and timely basis. Make corrections to errors on any administrative or UM reports including but not limited to incorrect delivery methods, blank notes, overlapping notes, invalid diagnoses, and invalid/incorrect place of service. Completes paperwork within defined timelines (Progress notes should be documented on the same day services are provided but are due - at the latest - within 7 days of the service that was provided). Make contact with newly assigned cases within 48 hours of assignment and have first appointment within 7 days of case assignment. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children. This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family's home and community-Intercept is not an office-based program. Intercept provides intensive, strength-based support tailored to each family's unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills. Essential Duties and Responsibilities: The PRN Community-Based Counselor: * Carries a small case loas as assigned by supervisory on an as-needed basis * Holds individual and family sessions with each family 3 times a week, scheduled at the convenience of the families * Conducts on-going assessment of youth to determine their needs from a strength-focused, solution-based perspective * Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills * Drives up to 60 to 80 miles to meet with families in their homes and communities (additional travel may be required) * Collaborates with other providers, case workers, and courts to formulate a collaborative treatment plan * Provides on-call crisis support to the youth and family (schedules vary by location) * Completes accurate and timely documentation in an electronic medical record system (EMR) * Performs other duties as assigned Additional Information: * Schedule is flexible and non-traditional as it is based around the availability of youth and families served. * Applicants must possess a current, valid driver's license, an automobile for work purposes, and proof of auto insurance. * Community-based staff will be reimbursed for applicable mileage. Break down barriers to access mental health services by providing culturally competent and inclusive care to clients from diverse backgrounds, and advocating for their needs within the organization and the community. Maintain accurate and up-to-date clinical documentation, including progress notes, treatment plans, and other required documentation, in accordance with professional standards and agency policies. Provide crisis intervention and support to clients experiencing mental health crises or emergencies, and coordinate with appropriate resources and services to ensure their safety and well-being. Qualifications: A degree in counseling, social work, or chemical depenency. State licensure or certification as a Mental Health Counselor or equivalent (e.g., LPC, LPCC, LSW, LISW) in accordance with local regulations. Demonstrated ability to work effectively in a team environment, collaborating with colleagues from diverse professional backgrounds to provide client-centered care. Strong communication skills, both written and verbal, with the ability to establish rapport and build therapeutic relationships with clients from diverse backgrounds. Commitment to ongoing professional development and staying current with the latest research and best practices in mental health counseling. Ability to work independently and take initiative, while also seeking supervision and consultation when needed. Strong organizational skills, with the ability to manage a caseload, maintain accurate and timely clinical documentation, and meet deadlines. Ability to work in a fast-paced environment and adapt to changing priorities, while maintaining a high level of professionalism and ethical conduct.
