Job Title: Registered Nurse (RN)
Reports To: RCM
Hours/Schedule: Full-time or Part-time, Non-Exempt
Environment: 100% on-site
Provide direct nursing care to residents that meet their physical and emotional care needs
Supervise NA-C to ensure clinical care appropriately meets residents' changing needs
Administer medication, check medication cards for accuracy and completion
Responsible for resident admission and discharge planning
Follow specific isolation procedures for infection control
Prepare care plan and administer ongoing care plan revision as needed
Participate in resident health care conference
Document progress notes, write summary reports and maintain resident care records
Observe, evaluate and maintain proper documentation for compliance and record
Participate in meetings as needed and other duties as assigned by the Resident Care Manager
QUALIFICATIONS, KNOWLEDGE AND SKILLS
Education: Graduated with an accredited program in nursing education
On the date of hire, hold and have ability to continuous renew while employed the RN license through the Washington State Board of Nursing
IV/PICC experience required
Working knowledge of Electronic Health Record System is preferred
A gentle compassionate, empathic personality with clear communication skills desired as population serviced is the elderly.
Person in this position must be able to communicate effectively with residents and their families often over complicated medical related matter.

Job Summary

Under the supervision of the PACE Center Manager, and as a member of the Program of All-Inclusive Care for the Elderly (PACE) interdisciplinary team, the Day Center Supervisor assesses the needs, interests and capabilities of participants and works with Recreational Therapists and other team members to develop individualized therapeutic recreation plans. In addition, the Day Center Supervisor plans, organizes, directs and participates in comprehensive care tailored to the general and individualized needs and limitations of frail elderly program participants of the Healthy Aging and Wellness Program, including PACE, Assisted Living, and Adult Day Services. This role provides leadership, supervision and support to the Recreational Therapy team. As a Registered Nurse /Licensed Practical Nurse is responsible for providing clinical expertise, supervision, and support to the staff. This includes direct clinical care, family/caregiver education, oversight of Caregivers, and working collaboratively with an Interdisciplinary Team (IDT) to provide holistic care for our participants. This position is a member of the Interdisciplinary Team, pursuant to 42 CFR §§ 460.102(b) and (d), as the Day Center Supervisor and backup for the Personal Care Attendant Representative. As an RN or LPN, Day Center Supervisor is responsible for day center coverage, services, managing risk, medication management, and overall programming.
Education - Associate's Degree or higher in Nursing.

Job Summary
The Clinic Care Coordinator (CCC) plays a key role in the daily operations of the School-Based Health Center (SBHC), ensuring a smooth and welcoming experience for patients and supporting both clinical and non-clinical staff with essential administrative functions.
Primary responsibilities include patient registration, check-in, scheduling, and managing key administrative tasks such as verifying eligibility, handling medical records, and processing referrals. The CCC also collaborates closely with school staff to develop and maintain systems for student referrals, parent consent, and managing student flow to and from class for appointments. Additional duties include assisting with data tracking and reporting, ordering and maintaining inventory of office and medical supplies (including vaccines), and helping to promote awareness of the SBHC services to students, families, and the broader school community. The CCC may also assist families with insurance eligibility and applications, helping reduce barriers to accessing care.
Education - High School Diploma or GED required, BA preferred.
Experience - Four (4) years' experience in a clinical setting, with one (1) year in a lead role. Experience serving patients from a wide range of backgrounds, including those who speak limited or no English is preferred. Experience working in a medical or dental clinic preferred.
Other Requirement(s) - Ability to speak and write fluently in one of the following languages, Spanish, Vietnamese, or an East African Language is valued.

Job Summary
ICHS' behavioral health services are collaborative, patient centered, and designed to improve and enhance patient's emotional, mental, and behavioral health. As a member of the patient's care team, ICHS' behavioral health program is accountable for the behavioral health care of all ICHS patients through direct referral, indirect review and implementation of guidelines and workflows impacted by the patient's behavioral health. Patients access behavioral health services as a member of their care team. In addition to patient care within the framework of established behavioral health guidelines and ICHS' Principles of Practice and approved by the appropriate licensing board in the State of Washington, the Behavioral Health Specialist functions as a member of the collaborative care team that includes the patient, primary care provider, behavioral health provider, consulting psychiatrist, and other medical staff. This position is responsible for providing integrated behavioral health care within a primary care setting.
The ideal work schedule for this position is Tuesdays-Saturdays but is negotiable. This position requires an Associate License (example: LMHCA, LMFTA, LSWAIC)
Education - Graduate degree in mental health field (social work, psychology, counseling, or related field.
Experience - Postgraduate experience working with individuals with co-occurring mental health and/or substance use disorders; individuals with a disability; individuals of a vulnerable population (elderly, mothers, children) and/or other high risk groups required. Full, current and unrestricted license/certification to practice as a healthcare provider as a substance use disorder professional or substance use disorder professional trainee preferred. One (1) year experience in a multicultural health care clinic and direct service experience with bilingual/bicultural patients preferred. Experience with motivational interviewing techniques preferred. Experience with assessing mental health and substance abuse use disorders and providing appropriate interventions based on the patient's level of need preferred.
Other Requirement(s) - Suicide assessment, treatment, and management training required by WA State Department of Health.
