Completes psychosocial assessment, develops plans and carries out interventions for patients identified through referral and case finding to have psychosocial risk factors. Conducts assessment of patient's psychosocial needs through intensive interviewing of patient and family members, conferring with interdisciplinary team, and reviewing medical record. Evaluates coping skills, cognitive and intellectual functioning, support systems, resources, and other factors, that could affect responses to illness, treatment and discharge plan. Identifies barriers and plans for intervention to overcome or lessen barriers to achieve outcome as evidenced by treatment plan. Communicates findings and plan to interdisciplinary team. Documents assessment, plan and interventions in medical record. Provides short term supportive counseling for individuals experiencing a temporary or situational problem. Performs assessment for cases of suspected elder, child, sexual or domestic abuse or neglect. Complies with required reporting, according to state law and hospital policy. Utilizes crisis intervention skills to assist victims/families of child abuse, elder abuse, domestic violence, sexual/criminal assault and traumatic injury. Acts as active team member in the discharge planning process with primary responsibility for identifying complicating social and financial factors and barriers to appropriate discharge. Facilitates family meetings when there is disagreement or lack of clarity around goals of care and plan of care. Collaborates with the health care team to help resolve family conflict around care decisions. Maintains a working knowledge of available community resources by establishing a relationship with liaisons and admissions staff at agencies and facilities in the region.

Essential Functions

Provides individual and group treatment as indicated on Individual Education Plans (IEP) and documents all clinical activity.
Writes annual IEP goals and notes monthly progress and advisory reports related to IEP goals
Documents efforts to reach non-responsive students and keeps monthly contact sheets.
Conducts functional behavioral assessments and creates behavior intervention plans (FBA/BIP) as needed for students.
Completes risk assessments for suicidal/homicidal ideation
Participates in annual IEP meetings and Manifestation Determination Hearings, as needed.
Participates on the RTI/MTSS Team and provides services and interventions as assigned including FBA/BIPs, individual counseling, parent interviews and psychosocial histories, community referrals, etc
Advises and collaborates with teachers, case managers, teams on students who exhibit behavior problems and / or failing grades.

Job Summary: Project SAVE has been a program of Catholic Charities Archdiocese of New Orleans for over thirty years. Our program provides free legal services to victims of domestic violence in Orleans Parish Civil District Court. Our attorneys represent clients at their Protective Order hearings and help to empower them throughout the legal process. The Court may grant relief including temporary child custody, temporary use of the home and community property, and temporary child and spousal support.
The Domestic Violence Counselor will work for CCANO on a part-time basis. The Counselor will have flexible work hours and will be able to provide tele-health services to clients. The Counselor will follow best practices and adhere to ethical and legal mandates. The counselor is expected to maintain necessary client records, maintain confidentiality and work closely with the Program Director to exceed contractual compliance. 1, Provide on demand integrated care consults to a diverse patient population in a fast-paced health center setting in
collaboration with medical care team. Population can include pediatrics, family practice, geriatrics and/or prenatal

Compliance and documentation
1. Thoroughly, accurately, and promptly document all service rendered in the electronic medical record for
every patient.

infection control, equipment operation, confidentiality, and other applicable areas (e.g., patient identification and
standard precautions).
3. Participate in proficiency testing as required by NOELA or departmental policy to ensure that proper standards of
care are being maintained.

Canal Pointe is seeking a Treatment & Recovery Counselor (LMSW, PLPC, LCSW, LPC, CAC, RAC or CIT) for our Residential Center, Monday - Friday 10a to 6:30p.
The Treatment & Recovery Counselor, under the direction of the Clinical Director, is responsible for conducting individual, family and group therapy for mothers residing at the residential center. This individual completes concurrent reviews with Managed Care Organizations (MCOs) to ensure approval of services based on the American Society of Addiction Medicine (ASAM) level of care for the maximum amount of time based on medical necessity. This individual updates and reviews person-centered care plans by required deadlines. This individual will work closely with supervisory and administrative staff to ensure efficient and high-quality service provision for all persons served by the program.
As the residential center opens, the Treatment & Recovery Counselor must be flexible and support the team in successful launch and scaling of services.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Support the Assistant Vice President of Integrated & Behavioral Health in the successful design and implementation of programming across American Society of Addiction Medicine (ASAM) residential levels of care (3.5, 3.3, and 3.1) and aftercare peer support services.
Support the Assistant Vice President of Integrated & Behavioral Health in the successful completion of the Implementation and Evaluation Phases of the Regional Partnership Grant (RPG), including adherence to RPG Research requirements.
Work closely with supervisory and residential center staff to ensure the provision of team-based care for all persons served is high quality, efficient, and in compliance with all accreditation and funding source requirements.
Provide direct counseling and crisis intervention services to persons served, utilizing individual, family, group interventions to the mothers in the residential center.
Conduct required admission, assessment and screening instruments with persons served to guide service delivery in line with best practices based on the individualized needs of each person served.
Update and review person-centered care plans by established deadlines.
Complete concurrent reviews, and other requirements with MCOs to ensure approval of services based on the ASAM level of care for the maximum amount of time based on medical necessity.
Precisely document and track prior authorizations, concurrent reviews, and other requirements in the Electronic Health Record (EHR) and additional tracking spreadsheets as required. Ensure clear communication of deadlines for future concurrent reviews and other requirements to other staff.
Utilize concurrent documentation and record accurate information in the EHR of each person served within 1 business day. Ensure documentation is completed in compliance with all accreditation and funding source requirements.
Responsible for ensuring that all policies, procedures and practices of VOASELA are met.
Perform additional duties as assigned.

Ensure VOASELA and program staff create and maintain a recovery-oriented environment based on the principles of Therapeutic Communities.
Provides support to all program activities, including Child Watch.
