Position Summary:
Responsible for providing building maintenance, custodial and grounds keeping services to ensure buildings and grounds are clean and in working condition.
This full-time position works Monday through Friday 7:30am to 4:00pm with weekends off!
MINIMUM QUALIFICATIONS:
* Demonstrated knowledge and prior experience with preventive building and equipment maintenance, basic plumbing, electricity principles, basic welding, refrigeration and air conditioning systems, and/or boiler systems repair
* Ability to read and write
* Must possess a valid driver's license which legally permits operating a motor vehicle in Pennsylvania

Job Responsibilities - Highlights
Provide direct care supports to people with intellectual or developmental disabilities and specialized care in daily living, social, and recreational activities.
Ensures compliance of the Department's Policies and Procedures relative to Community Homes. Assembles, maintains and monitors all in-house and persons records. Assists with and ensures the upkeep and maintenance of the home.
Implements formal and informal outcome plans to assist the person in achieving their personal outcomes.
Develops the weekly staff schedule. Adjusts staff schedules due to vacation, absenteeism or illness with prior approval. Ensures all shifts are covered.
Communicates and resolves program and individual concerns with the Community Homes Supervisor on a regular basis. Participates in general staff meetings.
Assign and ensure completion of each person's Annual Assessment and supporting documents and ensures the implementation of the outcomes identified in the Individual Support Plan.
Distributes individual funds, obtains and submits receipts and assists with banking as advised.
Administer medications (upon completion of certification; all necessary training provided).
Must obtain CPR certification within 6 months of the start date.
In addition to paying for federal and state clearances, Achieva offers 24 hours of training, annually, relevant to Human Services. In addition, we have implemented Medication Administration training, which would need to be completed successfully. This training assists our DSP's in ensuring all prescribed medication is dispensed and documented per P&P in partnership with the individual.

Participates in Achieva workgroups or committees as needed.

Qualifications

The Triage Nurse will perform selected nursing acts in the care of the individuals Passavant Memorial Homes serves in a safe, competent, and humane manner, and will report to work in-office at the Harmar location.

DUTIES & RESPONSIBILITIES OF THE TRIAGE NURSE:
Perform clinical nursing skills, with knowledge and understanding of fundamentals.
On-site monitoring of the community residential settings as required.
Monitor staff completion of new medication orders and documentation.
Provide on-going assessment and planning to assure health needs are met.
Administer injections as needed (i.e. Depo-Provera shots, Hepatitis B vaccines, TDap Boosters).
Complete dietary in-service for specialized diets and send to community homes.
Filing of labs, physicians orders, consults and other paperwork relative to consumers.
Communicate daily with hospital staff and provide updates to the Clinical Services Director and Regional Program Directors.
Complete other duties as assigned by the Clinical Services Director.
REQUIRED SKILLS AND KNOWLEDGE OF THE TRIAGE NURSE:
Graduate of an accredited school of professional and/or practical nursing, with a current license by examination from the State of Pennsylvania, required.
Valid PA Driver's License, required.
An understanding of the Intellectual Disabilities field, preferred.
One (1) to two (2) years' experience in medical/surgical or geriatric nursing, preferred.
