    Job DescriptionDescription: The Targeted Case Manager - D&A Specialist will provide services to participants, living in the community with persistent mental health and drug and alcohol challenges. After assessing needs, the Drug and Alcohol Specialist helps the participant with obtaining resources and services in the community, in a partnership that focuses on recovery and the achievement of the participant's goals for community living. The Drug and Alcohol Specialist also is instrumental in assisting homeless individuals to locate adequate shelter/residence. The Drug and Alcohol Specialist also engages individuals who are incarcerated acting as a community link in reestablishing a productive life within the community once released. The D&A Case Manager is a full time position, Monday - Friday, 8:30 AM - 5:00 PM (with the opportunity to create a flexible schedule with manager's approval). This is a hybrid work from home position, with required in-person participant visits and office days and occasional work from home options. The rate of pay for this position will be an exact rate of $24.00/hour. WHAT YOU'LL DO • Provides assessment of substance use to determine the need for treatment and other services including specialized housing and other recovery supports in the community. • Develop individualized plans with the participant and other members of the case management team to address substance use treatment needs. • Links the participant with the appropriate substance and alcohol use services and monitors these services with other case management team members. • Provides consultation and support to case managers in identifying and engaging individuals who are experiencing substance use issues. • Supports individuals in the community with obtaining necessary substance and alcohol use treatment. • Facilitates a weekly Drug and Alcohol Recovery group. • Continually engages ambivalent individuals to assess their readiness for treatment or services, assesses their stage of change and actively works with them utilizing motivational interviewing. • Conduct outreach to locate individuals on their caseload who may be missing and have a concern about substance use. • Collaborates with participants to identify, develop, and access resources and supports to increase their success in self-sufficiency, community integration, and community inclusion. • Collaborates with participants to assess their hopes, strengths, accomplishments, and challenges and create goals; assists in identifying support and arranging services and resources to achieve goals. • Supports, teaches and models recovery and recovery tools and models personal responsibility, self-advocacy, and hopefulness. • Coordinates and/or chooses relevant supports to support participants in developing their self-directed recovery and crisis plans; facilitates the participants' self-review of their progress upon each encounter. • Provides crisis intervention and de-escalation to participants as necessary. • Obtains necessary information during the intake process and collaboratively documents participant progress in MHP's electronic health records system at the time of service. Credible; ensures that all data is correct by verifying the information with the participant. • Supports and assists participants in the community when necessary and ensures participants safety when participating in events, visits, and other interactions. • Complies with all other established quality and regulatory standards, including supervision and training requirements. • Applies general knowledge of Recovery services to complete small projects or conduct a series of tasks with a limited degree of supervision. • Creates weekly schedules, two weeks in advance, that meet program productivity standards and supervision. • Maintains compliance with all MHP policies and procedures. Reports known conduct taken by any MHP employee (including yourself) who violates any MHP policy or law on which you have been trained or should know of, such as laws criminalizing theft or violent crimes, and reports the matter to management or to the Director - Quality Improvement & Compliance and/or Human Resources. • Other duties as assigned to fulfill MHP's mission. Requirements: • Bachelor's degree with major course work in one of the following areas: sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling or education; or be a registered nurse whose duties will be limited to TCM responsibilities only. • Minimum of 2 years of experience in a behavioral health work environment with a focus in substance use. • Must know the local community and statewide substance use treatment facilities and other resources. • Must know treatment techniques and procedures as applied to a substance or alcohol use. • Must have a BHTEN certification or be able to obtain one within one year of hire date. • Must possess a valid driver's license. • Must be able to qualify for and obtain a prison ID. • Must possess basic computer skills to perform job duties, including desktop computing, e-mail, timesheet management, electronic health records, internet research, etc., and the ability to learn software applications relevant to your position. • Must have access to a car to use for work on a daily basis. • Must have automobile insurance coverage at the state requirement levels and current state registration. • Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices. • Must be able to work under pressure and meet deadlines and billable unit requirements while maintaining a positive attitude and providing exemplary customer service. • Strong skill sets in oral and written communication, cultural competence, interpersonal relationships, and teamwork, attention to detail, customer service, time management, autonomy, problem-solving, and organization.
