The Economic Self Sufficiency Care Coordinator is the point of contact in linking to governmental benefits and entitlement programs for individuals engaged mental health services.
The Economic Self Sufficiency Care Coordinator assists individuals in applying for and following up with Social Security Disability, Supplemental Security Income, Medicaid, SNAP, Housing programs, and other governmental assistance. The Economic Self Sufficiency Care Coordinator uses the SSI/SSDI Outreach/Access and Recovery (SOAR) model to guide the application process. The Economic Self Sufficiency Care Coordinator is also responsible for coordinating intra and interdisciplinary with the treatment team, for instance, the Case Manager, or Care Coordinator, as well as with other members involved in the client's care.
The Economic Self Sufficiency Care Coordinator coordinates an individual's initial application and monitors outcomes and follow-up needs. The purpose is to provide better access to basic needs and improve the overall quality of life. Experience interviewing customers, evaluating their needs, and providing personalized counsel on the benefit programs and options. Extensive knowledge and experience working with public benefits, health care options, and other benefit related concerns desired. Advises and counsels persons and their families regarding eligibility rules and application procedures for public benefits; provides information and suggestions by personal meetings and through phone communications; assesses needs of individuals and records actions taken and contact made to ensure progress is made with each consumer.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provides linkage and monitoring of individuals with governmental assistance programs such as Social Security Disability Insurance, Supplemental Security Income, Supplemental Nutrition Assistance Program, Medicaid, and Medicare, insurance through healthcare.gov.
Provides services and documentation in accordance with Metrocare and Care Coordination Guidelines as well as Metrocare policies and procedures.
Coordinates with inpatient facilities, including emergency departments, behavioral health hospitals, state hospitals, and residential treatment facilities, in arranging benefits for referred individuals.
Coordinates with the multidisciplinary team in ensuring consumers receive timely linkage to benefits upon intake or re-initiation of services. Benefits assistance must include an effort to schedule a follow-up appointment with a Metrocare provider within 72 business hours of the individual's discharge. The liaison (or designee) will communicate with the referred individual within 24 hours of that discharge, continuing contact every day until the first benefits application appointment is completed.
Assists clients with housing search, locating housing options; verifies and documents clients eligibility for homelessness prevention or rapid re-housing assistance, or other assistance.
Assists with getting clients added to the housing priority list for Dallas and Collin County.
Be responsive to communication with assigned individuals, collaborating team members, and collaborating agencies/benefit programs. Responds to phone and email within 24 hours in a timely and comprehensive manner.
Interacts with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to an individual's ability to meet basic needs and medications.
Aids individuals keep all appointments with benefit programs to support successful applications and follow-ups.
Facilitates communication between Medical Records and Social Security Administration/Disability Determination Services.
Develops close working relationships with internal and external referral resources.
Maintains confidentiality of information concerning individuals in care and family members
Attends and participates in all scheduled team meetings.
Contributes to team knowledge and understanding benefits. Assists the business, clinical, medical, and nursing staff by reporting benefit needs, services, and any potential barriers to benefits.
Assist clients in linking with resources and a system of community support and entitlements.
Completes paperwork requirements and maintains quality, up-to-date records regarding benefits assistance.
Documents successful and unsuccessful provision of supportive services within 24 business hours.
Documentation requirements will include the Release of Information and Consents to expedite the transition of care.
Works with family members/support system to gain support in accessing benefits
Is knowledgeable about and remains in compliance with internal and external policies, procedures, regulations, and standards, including Metrocare policies and procedures, Quality Assurance Plan, and all applicable regulatory standards.

The QMHP-Adult Intensive position provides psychosocial/rehabilitative service to adults with serious mental illness. Services may include, but are not limited to, assessment, development of measurable treatment goals and objectives, engagement into treatment, referral, linkage, advocacy, monitoring, crisis intervention and continuity of care. The overall goal of this position is to maximize the individuals potential level of functioning, reduce hospitalization and aid in the successful reintegration of individuals into the community. Provide overall service coordination, psychosocial rehabilitation, emergency services/assessment, referral, transportation, linkage, and advocacy to individuals with varying needs. They are responsible for working with youth, adults, and their families in a manner that is trauma-informed and responsive to needs, culture, gender, and military status. The Qualified Mental Health Professional CCBHC IA & Assessment QMHP is responsible for collaborating in the development of a person-centered and family-centered recovery plan aimed at helping every individual achieve their goals and objectives. The QMHP position is responsible for applying a holistic approach in assessing needs, developing safety plans, and providing any other clinical services of benefit, including case management, psychosocial rehabilitation, and skills training. This position maintains responsibility for a 100% productivity expectation.

Work with the consulting psychiatrist to ensure that clients' behavioral health needs are addressed and help to facilitate and integrate the psychiatrist's recommendations.
Models and supports clients in health and wellness activities and with using healthy coping skills.
Facilitates psycho-education groups and life skills development groups as needed.
Maintains continuous services to an assigned caseload by providing Engagement, Outreach, Case Management, and Psychosocial Rehabilitation services.
Attends and participates in all scheduled team meetings.
Collaboratively develops safety/crisis plans
Maintains confidentiality of all reports concerning staffing and consumers.
Contribute to team knowledge and understanding of each consumer. Assists the medical and nursing staff in monitoring by reporting potential problems.

They will serve as a role model and demonstrate competency in recovery and ongoing coping skills, serve as a consumer advocate, and will provide information and peer support in the community. The Recovery Support Peer Specialist will also provide essential expertise and consultation to other team members in promoting a recovery-oriented culture. The Recovery Support Specialist will facilitate and/or co-facilitate peer-to peer discussion groups as well as individual sessions with individuals in services using a person centered recovery model.

Installs, repairs, and maintains water fixtures on or in Metrocare's property to improve or maintain level of service; checks restroom fixtures, repairs any toilets, urinals, seats, or other plumbing, and addresses clogs and damages.
Plans and implements the installation of natural gas pipelines and potable or wastewater pipes, including the cost projection for parts and man-hours to support the installation of improvement projects.
Instructs and educates lower-level plumbers in the uniform plumbing code, the use of plumbing tools, and plumbing practices to qualify them for more responsibility and promotion.
Serves as a crew leader to ensure jobs are completed according to code and maintains leave use, hours worked, and performance records on lower-level plumbers.
Measures, cuts, and threads iron pipe for natural gas pipelines or water/wastewater lines.
Removes clogs from wastewater systems using manual and/or small power tools.
Makes minor mechanical repairs to small water or fuel pumps; maintains, repairs, and fixes irrigation lines and related issues.

The Metrocare Services Mental Health and Primary Care Division includes a Crisis Services Program that provides 24/7 services to those in need via the crisis hotline and mobile crisis outreach teams. Crisis services are offered for to anyone, anywhere, and at any time the need arises. Crisis services will be provided regardless of an individuals ability to pay or whether he/she is homeless or unable to produce verification of residence. The Crisis Interventionist Specialist-QMHP plays a vital role on a team comprised of a Licensed Practitioner of the Healing Arts and a Mental Health Peer Specialist.
Work Hours: 11am-7pm (2nd Shift)
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provide crisis intervention services in a variety of modalities, including telephonic, telehealth, and face to face intervention.
Ensure follow up is provided within 1 hour of the emergent crisis
Assists inpatient facilities, including emergency departments, behavioral health hospitals, state hospitals, residential treatment facilities and the criminal justice system, in continuity of care arrangements for consumer referred to Metrocare
Hotline staff screen/assess the crisis, provide an evidence-based intervention, and stabilize the crisis. The caller is then linked with service coordination assess the level of further stabilization needs and appropriateness for other services in the crisis continuum
When Hotline interventions are not able to adequately stabilize the situation, the MCOT team to further assess the crisis. If screening and assessment indicates that there is an emergent/imminent risk of harm to self or others, the Hotline staff notifies 911 and attempts to stay on-line with the caller until 911 personnel arrive on site where the individual is present.
Develop person-centered crisis plan and assist the individual in executing the plan during and post crisis intervention.
Ensure continuity of care post crisis intervention to appropriate outpatient services, including but not limited to, outpatient mental health, substance abuse services, and/or primary care services.
Be accessible for referrals and communication through our electronic health applications, by phone and email to provide a timely and comprehensive response.

The LPHA Interventionist uses a basic knowledge of diagnostic characteristics, symptomology of primary mental illness and knowledge of medications, side effects and benefits to provide services in a person-centered, supportive, office-based environment. This position is monitored by periodic supervision.
