Innovative services offered across Chicagoland and Central Illinois include community living options, novel day programs, behavioral health services, employment programs, respite for families, and foster care for children with disabilities. Purpose: Performs functions to recruit, orient, assess and monitor prospective and current foster families. Essential Responsibilities, include but are not limited to: Recruit prospective foster family applicants and conduct orientation meetings. Respond to telephone inquires regarding foster parent licensing. Initiate licensing process by providing the applicant with necessary documents and when completed, forward forms to Springfield. Assess family home environment, financial readiness, family relationships, supports, references and motivations. Recommend to licensing supervisor approval or denial of foster family license in accordance with DCFS Rule 402. Inform foster families of their responsibilities and CARC responsibilities by providing a foster parent handbook and signing the foster parent agreement. Complete CFS590 Compliance Record for Foster Home via home site visit observations. Complete written home study and/or home study update to attach to recommendation document. Maintain the licensing files/or licensing section of Foster Parent Records for each home and develop mechanism to track expiration date and compliance visits. Maintain close communication with DCFS licensing office to maintain efficient flow of the licensing process and procedural updates. Attend DCFS Licensing training sessions. Maintain a schedule of twice yearly monitoring visits to existing licensed homes for continued compliance. Inform licensing supervisor and child welfare worker of any unusual activities or problems affecting the licensing compliance of a home. Assist in coordinating and document training for foster parents. Responsible for carrying and responding to assigned pager on a 24 hour basis. Perform other related duties as assigned. MINIMUM QUALIFICATIONS: Must have a bachelor's degree in social service-related field and a minimum of 1-3 years' residential experience as a Community Living Specialist (CLS), Direct Support Professional (DSP), or Rehabilitative Service Aid (RSA) in a CILA program. Must be at least 21 years of age, have a valid driver's license, approval through agency's insurance carrier to drive agency vehicles, and willing/able to drive a 15-passenger van. Must have a mobile phone number in which you can be reached/accessible during work hours. GENERAL FUNCTIONS: The Residential House Manager will participate as a member of the multidisciplinary team, responsible enforcing overall residential house efficiencies, rule 115 compliance, and quality of care provided to clients in the MH Residential Program based on treatment needs identified in each client's individual treatment plan. Must maintain Medicaid compliance for all clinical documentation and must achieve and maintain established productivity targets, completing all clinical documentation on a daily basis, and ensuring that all house staff do the same. Must be able to work, 1pm-9pm Monday-Friday. Will alternate residential on-call responsibilities once a month with other House Managers, for all 5 houses, outside of normal shift hours. Responsible for assisting Assistant Director of Residential Services with scheduling house staff to maintain consistent shift coverage, maintaining an external registry, and overseeing overall needs of assigned house to ensure proper functioning. ESSENTIAL FUNCTIONS: Will oversee the facilitation of healthy meal planning with staff and clients throughout the week (based on client's dietary needs), will shop for ingredients needed for planned meals, and in the preparation of meals as indicated by the client's skill level and treatment plan. Responsible for obtaining bulk supplies for assigned house, overseeing inventory lists, and taking clients grocery shopping while managing member LINK card and/or cash purchases. Responsible for assisting clients with banking. Such as, to cash allowance checks, distributing client funds and collecting/submitting receipts. Responsible for maintaining accuracy of client attendance logs, safety logs, vehicle logs, MARs books, and conducting medication inventory on a daily basis and report any errors or inconstancies to the A.D. of Residential Services and MH nurse. Assist and oversee the planning and scheduling of weekly recreational activities for clients, out in the community, assisting with outings as needed. May provide supervision to staff in absence of Assistant Director during these events. Responsible for assisting with linkage/discharge services for clients needing, and/or are ready to move out of the MH residential program for various reasons. Responsible for the onboarding and orientation of new clients coming into the MH residential program. Maintain communication with client's medical and benefits case manager regarding client medical benefits/insurance, SNAP status/approval, spend-down, and SSI review forms, and transportation services as needed, to ensure benefits and entitlements remain active. Maintain and update client's pertinent information within the MH electronic care system i.e. face sheets, contacts, consents, and diagnoses. In conjunction with the client's treatment team, keep client's family, guardian, and other Envision Unlimited staff informed of the client's progress, including emergency room visits, hospitalizations, and medication changes/adjustments. Provide information to outside agencies as requested to coordinate and assure the appropriateness of services. Provide and document crisis interventions as needed, within 24 hours of an incident, and complete an incident reports when necessary. Facilitate De-Briefing sessions with staff as needed to help them learn and grow as paraprofessionals to reduce likelihood of future incidents. Achieve personal productivity targets and timely completion of clinical documentation. ADMINISTRATIVE DUTIES: Will oversee compliance regarding CARF, Medicaid, and BALC regulations within assigned home, including enforcing staff adherence to safety drills per safety drill schedule for assigned home; Provide technical assistance and support to staff to aid in maintaining compliance with Medicaid Standards. Will aid A.D. of Residential Services with monitoring the timeliness of staff note completion on a daily basis, ensuring that staff are consistently achieving productivity targets assigned to their shift. Provide initial and ongoing training to staff on community support individual and/or group skill building activities as needed.
