This role is open to NPs only, with no call responsibilities. The clinic serves patients of all ages, primarily adults, and uses EcoVantage EMR. The provider will work alongside three psych providers and two primary care providers, with support from a dedicated RN and MA. Your Role: APP - Nurse Practitioner Responsibilities
Will primarily support in-home, patient-facing visits to support patient care needs in the market.

Patient Care Responsibilities
* Take patient histories, sufficient to address the patient's chief complaint and other health concerns.
* Provide women's health assessment and treatment for students with women's health problems. Appropriate treatments may include individual, couples and group psychotherapy if applicable, referral for hospitalization,and be consistent with limitations of MHU staff resources.
* Prescribe, administer, and dispense prescription and nonprescription medications.
* Provide emergency triage for crisis evaluation and management, including development of a safety/crisis plan, when necessary.
* Follow up with patients to review lab test results and progress in an appropriate and timely manner.
* Make referrals to other providers as appropriate to facilitate treatment plan with standard and crisis interventions. This may include out-patient psychotherapeutic techniques or facilitating hospitalization at community hospitals on behalf of students.

The Nurse Practitioner (VSP) works on problems of diverse scope and complexity ranging from moderate to substantial.
This is a variable service provider NP/PA (APP) role with an average of 20-30 hours per week need for a 6-month period. Licensure in **at least 2-3** of listed states: **FL, LA, KS, TX, MS preferred** prior to the start of employment. There may be opportunity to move to a similar role within CenterWell Medical group but that is not guaranteed. There also may be opportunity to cover other aspect of the program, such as covering PTO for APPs in CenterWell Medical group (phase 1). There are no benefits tied to this position. The APP will spend their day performing the following duties:
+ Calling Humana members to discuss open care gaps and placing needed orders (specifically women who had a fracture in the last 6 months)
+ Creating and registering a patient record withing Athena
+ Verifying benefits
+ Gathering medical and surgical history
+ Verifying and entering medications
+ Coordinating management of members social needs to help facilitate gap closure - providing transportation services.
+ Connecting with outside partners to ensure testing is done prior to member specific timelines.
+ Ensuring adequate follow up- receipt of results, medication initiation, and PCP follow up.
