Other duties may be assigned. • Provide exemplary customer service to patients, visitors and coworkers • Provide convenient, friendly patient care to all who come through our door seeking health related services • Provide leadership, direction, and assistance during emergencies while maintaining a comforting and reassuring affect for patients • Working environment subject to varying fast paced and high stress conditions, including long and variable hours, changing locations, and continuous and frequent contact with others • Performs medical assessment, treatment, management and follow up of clinic patients including, but not limited to: primary care, urgent care, occupational health, DOT physicals and workers compensation • Performs medical procedures including, but not limited to: laceration repairs, nail removal, pap tests, injections, splinting and abscess incision and drainage • Establishes and monitors a medically appropriate level of care for clinic patients • Oversees, directs and supports on site laboratory, x-rays, injections, and collections for direct access services, if applicable • Maintains clinical core competency • Works with Center Leadership and other staff to ensure operations are consistent with medical policy and professional standards • Cooperates and participates in the development, implementation and revision of policies affecting medical practice, quality of care and patient satisfaction • Participates in and assists with in-service and continuing education programs for physicians, nurses and other personnel employed by or associated with Company • Recommends potential means of growth and development for new and/or existing services within the Center • Assures compliance with licensing, certification and accrediting bodies • Participates in development and presentation of education programs for staff • Represents Company and the clinic as reasonably required at internal and external meetings or events • Provides guidance to other medical staff (e.g. physician assistants, nurse practitioners and medical assistants) to ensure quality professional services and patient satisfaction WHAT'S REQUIRED? • Experience as a Family Nurse Practitioner is preferred • Active state license as an APRN • Current Family Nurse Practitioner (FNP) Board Certification • Current DEA license, or eligibility to obtain upon hire • Current DOT certification, or willingness to obtain upon hire • Current BLS certification WHAT'S THE SCHEDULE? The Nurse Practitioner is an Advanced Practice Registered Nurse who provides a wide range of health services, including urgent and emergent evaluation of episodic and chronic problems; and comprehensive, integrated primary health care which addresses a majority of the client's health care needs on an ongoing basis. The Nurse Practitioner is knowledgeable in the assessment, treatment and evaluation of a comprehensive range of health care problems; emergency measures; preventive health; prescribing medications and other treatments; ordering, conducting, interpreting laboratory, diagnostic studies and procedures; and patient and family education utilizing community, social, and health resources. The Nurse Practitioner maintains knowledge of current practice, acts as role model and mentor to other APRNs, acts as client advocate, and participates in professional activities at local and state level. Required Skills REQUIRED EXPERIENCE AND SKILLS: • Three years of clinical nursing experience is required. Growing together.** **Position Highlights & Primary Responsibilities:** + Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable + Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter, including visits to address changes in condition + Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization + Consults with Physician Leaders and coordinate with PCP in these decisions as available + Comfortable with basic procedures of episodic nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, basic wound care, etc. New hire and annual skills training provided + Performs post discharge visits to ensure patient stability is consistent with their goals of care + Participates in HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery + Participates in clinical after hours on-call program on rotational basis You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. **Required Qualifications:** + Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA) + Current, unrestricted NP licensure in State of Practice + Current, unrestricted RN licensure in State of Practice + 2+ years of clinical experience, primary care, home-based medical care, geriatrics, palliative care, emergency/urgent care, or similar setting + Access to reliable transportation **Preferred Qualifications:** + Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems + Knowledge of CPT, ICD-10 and HCPC codes + Knowledge of clinical standards of care + Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts and benefit systems Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
