This role involves collaborating with a multidisciplinary team, including Jail Caseworkers, Psychiatrists, NursePractitioners, Physicians, Support Staff, etc. This position is intended to provide in-person care with the potential for remote work (hybrid schedule).
Essential Functions:
Provide psychiatric services to individuals, including evaluations, treatment, and monitoring of psychotropic medications.
Evaluate / monitor individual's progress, including individuals on behavior / suicide watch. This includes individuals who are in restricted housing, behavioral units, etc.
Work as part of a multi-disciplinary team, including a Lead Psychiatrist, NursePractitioners, Physician Assistants, and/or Psychiatry Residents/Fellows.
Provide psychiatric consultation to all professionals within the unit.
Available for emergency psychiatric coverage, including evening and weekends on a rotating basis.
Refer individuals as necessary to appropriate crisis centers / psychiatric facilities when appropriate. Provide supporting documentation.
Work collaboratively with on-site pharmacy to order psychotropic medications, consistent with the jail's formulary.
Provide discharge prescriptions.
Ensure complete, accurate and timely psychiatric entries are made within the Electronic Health Record.

Job Requirements and Qualifications:
Education:
Master's Degree in Nursing, from a nationally accredited university.
Preferred: Completion of a Psychiatric-Mental Health NursePractitioner Program is preferred; PMHMP-BC Certification is preferred.

Position Summary
The NursePractitioner (NP) works collaboratively with a supervising physician to assess, diagnose, and treat clients with current or potential psychiatric-mental health conditions. Responsibilities include conducting comprehensive physical and psychosocial evaluations, prescribing medications, and providing both scheduled and emergency psychiatric services. The NP develops and manages individualized treatment plans, ensuring effective and appropriate care with ongoing evaluation and supervision.
Key Responsibilities

Current unrestricted NursePractitioner in Michigan required.
Minimum of one-year experience in a Medicare-certified home health/hospice agency preferred.
Demonstrated knowledge of home health nursing/hospice and/or outpatient services preferred.
Willing and able to meet the agency need to support hospice Medical Director/designee.
Understanding of performance improvement with the ability to communicate and operationalize performance improvement initiatives at the departmental and organizational level.
Excellent interpersonal skills, sound judgement, effective organizational, prioritization and follow-through skills, attention to detail, tact, dependability, emotional intelligence, ability to maintain confidentiality and promote positive, constructive relationships with communication and collaboration at all levels required.

Position Specific Responsibilities
- Performs routine and urgent assessments:
Availability PRN/Part Time
Conducts medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.
Orders, performs, and interprets laboratory and radiology tests within scope of professional practice to assess patient's clinical problems and health care needs.
Prescribes medications including controlled substances to the extent delegated and licensed.
Orders treatments and durable medical equipment as indicated.
Performs other therapeutic measures as indicated.
Assists patients and families in identification of goals of treatment and ongoing plans of care at every visit. Communicates these goals among Interdisciplinary Team.
Consults with palliative care physician or designees as needed, informs primary physician of services provided and collaborate with other physicians as needed.
Assists in all facets of care coordination for palliative care referrals.
Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
Ensuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas.
Participates in Quality Assurance activities and committee related to NursePractitioner scope of practice and policies/procedures.
Complies with applicable laws and regulations with respect to Collaborative Agreements.
Provides training and ongoing education and onboarding support for staff.
Defines goals for professional growth and participates actively in professional activities and organizations.
Engages in active and frequent self-care activities for personal and professional growth and longevity.
Adheres to the practice of confidentiality regarding patients, families, staff and the Organization.
- Collaborates/communicates with Medical Director, attending Physician, hospital staff, and Interdisciplinary Team:
Participates in Interdisciplinary Team meeting bi-weekly.
Collaborates/communicates with Interdisciplinary Team needs of patient for clinical and psycho-social interventions.
Assists in identifying the need for intervention of other Interdisciplinary Team members.
Effectively communicates patient and family needs to Interdisciplinary Team.
Completes and submits required clinical documentation within 24 hours of visit completion.
- Educates patient and caregiver regarding:
Care of patient.
Disease process.
Goal setting.
Symptom control.
Treatment options.
Prognosis.
Advance Care Planning.
- Supervises nurse trainees, including graduate and advanced practice nurses in field placements.
- Must be able to operate an automobile/has access to personal transportation.
- Able to lift/transfer/push/pull up to 100 pounds infrequently using assistive techniques and devices appropriate to manage the task.
- Must be able to complete multi-tasks and meet multi-deadlines.
- Proactively promotes census growth and seeks to offer care that meets the unmet diverse needs of the community.

Qualifications
On an Average Day You Will:
(includes, but not limited to)
Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children
Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record
Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services
Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations
Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification
Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners
Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members
Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician
Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols
Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care
Fulfill the obligation of requested and/or accepted assignments
Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues
Complete all other duties as assigned
Requirements:
Must be a graduate of an accredited School of Nursing and accredited NursePractitioner Program
Must have current nursing licensure in State
Must have a master's degree with a minimum of one (1) year NursePractitioner experience and a minimum of one (1) year hospice or palliative care experience
Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
Must be a licensed driver with licensed automobile that is insured in accordance with
organization/state requirements and is in good working order
