Overview

Assignments are based on the individualized plan of care and the aide's training and competency.
Key Responsibilities
Provide personal care including bathing, dressing, grooming, toileting, oral hygiene, and skin care as assigned
Assist with ambulation, transfers, repositioning, and range-of-motion exercises as directed
Observe and report changes in patient physical or emotional condition to the supervising nurse
Assist with meals, feeding, and hydration support according to the plan of care
Provide companionship, emotional support, and respectful presence to promote comfort and dignity
Take and record vital signs when trained and directed by the nurse
Maintain a clean and safe care environment, including light housekeeping related to patient care
Provide short-term respite support to family caregivers during scheduled visits when appropriate
Document care provided accurately and timely in accordance with agency policy
Follow standard precautions, infection control practices, and HIPAA privacy requirements
Participate in team meetings and required in-service training sessions
Deliver care in any setting where the patient resides, including private homes and licensed facilities
Provide weekend or after-hours support when directed by clinical leadership

Qualifications
Active certification as a Certified Nursing Assistant (CNA) in good standing with completion of a state-approved training program
