In our newly created apprenticeship, you will play the Parent Educator role and will be responsible for providing home visits to clients enrolled in the Help Me Grow Program, focused on child development. This program assists families with their goals to provide support, encouragement, information and resources to promote positive parent-child interaction. As an Apprentice, you will have on-the-job learning, classroom instruction, support and mentorship along the way. At the end of the one-year apprenticeship, you will be credentialed and equipped to explore other career opportunities in the behavioral health field. Essential Functions: * Demonstrate the values and guiding principles of OhioGuidestone every day, performing with the highest standards of excellence, integrity and ethics. * Attend all scheduled classes, workshops, and mentorship sessions related to the Help Me Grow and apprenticeship curriculum. * Assist in building relationships with individuals and families in the Help Me Grow program. * Support and deliver appropriate health education on trained topics. * Learn all systems utilized in the Help Me Grow program. * Participate in community events, health fairs, and home visits under supervision. * Maintains the confidentiality of client or collateral interaction within Agency defined standards. * Exhibit positivity, flexibility and a willingness to take on new responsibilities as requested or required. * Demonstrate positive leadership, promote a team-based work environment and a culture of belonging. * Present the Agency in the most positive light with all internal and external contacts. Under administrative direction, this position provides guidance, instruction and direct supervision to one assigned MST team. As the MST Clinical Supervisor, you will monitor, review and document staff activity. We are currently seeking a part time 20 hr. per week Certified Medical Assistant for our Lorain medical office. The ideal candidate will have experience in a medical setting that treats Behavioral/Mental Health, Chemical Dependency/SUD including MAT, and/or provides Psychiatry services. The Certified Medical Assistant will be in charge of greeting incoming patients/vendors, answering phones, rooming, incoming patients, scheduling, and charting/communicating with the medical management staff. Essential Functions: Maintains complete and current medical records regarding all clients served. Greets incoming persons and directs/triages as needed. Checks patients in for appointments. Answers incoming phone calls, answers questions or forwards calls to the correct area. Completes any required paperwork, documentation, prior authorizations, release forms and other items as needed. In collaboration with the team, ensures a comprehensive health history through the collection of vital signs, lab work and OARRS reports as needed for further assessment and treatment. Documents client records, labs, medical records, and OARRS within Agency designated standards. Meets and sustains the OhioGuidestone designated productivity standard. Provides education to persons served, family members of persons served and other staff regarding medication, symptom management, wellness and health, as instructed by medical staff. Provides monitoring of clients' mental illness or substance use and physical symptoms; communicates any changes to the treatment team. Provides direct witnessed urinalysis, targeted case management, or individual psychotherapy to clients to monitor symptoms, carry out prescriber orders and support coordination of care. Maintains equipment and medical supplies, including reordering, stocking, cleaning, and troubleshooting. Meets and sustains the Agency's designated productivity standard. Travels to other Agency sites as needed. Maintains reliable attendance. Exhibits positivity, flexibility and a willingness to take on new responsibilities as requested or required. Demonstrates positive leadership, promote a team-based work environment and present the Agency in the most positive light with all internal and external contacts. Performance: Must be able to operate a computer, phone and other position related/office technologies Must be able to verbally exchange information in a confidential manner with employees, clients, students, participants, and/or families in various settings Must be available for crisis management by phone or in person as needed Must be able to work flexible hours as needed Must be able to clearly and confidentially communicate with OhioGuidestone employees, clients, students, participants, and other stakeholders Physical Requirements : Must be able to remain in a stationary position up to 50% of the time Must be able to access work locations and, as necessary, which may include ascending/descending stairs Must be able to stoop, stand, walk, bend, and stay upright to perform work Must be able to visually read written and digital information Must be able to visually determine the accuracy, neatness, and thoroughness of work assigned and/or to make general observations of facilities or structures Must be able to receive detailed information through oral communication Must be able to hear clients, students, or participants, co-workers, and supervisors at normal speaking levels Must be able to dictate with clients, students, participants, co-workers, and supervisors using all means of communication including but not limited to, in-person, phone, and computer Must be able to type case files and other paperwork Must be able to frequently drive to varying work environments including agency offices. Must adhere to all Our Promise Values and Guiding Principals Qualifications: High school education or equivalent required. Successful completion of an accredited Medical Assistant program, required. Medical Assistant credentials, REQUIRED. CDCA and QBHS licensure required within first 6 months of employment. Current CDCA a plus! Minimum of 1 year of related Medical Assistant experience, required. Demonstrated EMR experience required, Evolv preferred. Demonstrated computer literacy and excellent verbal communication. Must have a valid Ohio driver's license, safe driving record and proof of insurance required. Ability to take and pass a physical, tuberculosis test, and drug screening. Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment. Position Summary: The Clinical Diversion Specialist will ensure the appropriate placement of youth throughout our array of referral and treatment services for the Cuyahoga County Juvenile Court EIDC (Early Intervention and Diversion Center) and CALM (Coordinated Approach to Low level Misdemeanor) Program. He or she will provide clinical assessments based on verbal, written, and face-to-face interviews to determine appropriate level of treatment for youth either referred to the Intervention Center or brought to the program by the police for domestic violence calls. The Clinical Diversion Specialist may explore alternatives such as kinship placement or respite placement for up to three days, in addition to respite if deemed necessary to remove youth from the home. In less acute cases, he or she will connect youth to appropriate mental health and prosocial services. When necessary, the Clinical Diversion Specialist will complete a youth screener, a safety plan, symptomology screeners, and connect the youth and guardian with ongoing services. This program provides services 24/7. PRN on-call hours are Saturday and/or Sunday from 8am-12a. PRN staff answer these calls remotely and only go into the office if there is a respite placement. All other duties can be completed remotely. Responsibilities Include: Responsible for completion of behavioral health and juvenile justice screeners. Communicate results of assessment and recommended services to referring court/law enforcement staff. Administer, track, and submit outcome measures in accordance with program standards. Responsible for case management and referral tracking and ensuring proper funding of all cases before the case leaves intake. Face-to-face or virtual admission with clients and guardian including, but not limited to, completing all required clinical and administrative paperwork, completing and submitting behavioral health reports and safety plans to referring court personnel, and, when applicable, completing Psychiatric Diagnostic Evaluations and Treatment Plans for new admissions to Applewood Centers' programs. Coordinate and link with other community behavioral health and substance use providers to ensure continuity of care for open cases. Applewood Centers, Inc.
