POSITION SUMMARY:
The Medical Records Coordinator is responsible for coordinating the workflow and assisting the coverage of the Medical Records Department. Maintains the electronic medical record. As a Medical Records team member, this position is also responsible for coordinating and maintaining patient record information and for performing administrative functions related to the Medical Records department.
AREAS OF RESPONSIBILITY:
Receives direction and supervision from the Medical Records Manager.
Participates as a member of the Medical Records Team.
Participates as a member of the overall Pathways clerical support team.
Proficiency in using electronic health record systems, office software, and other relevant technology.
Maintains the integrity and presentation of patient electronic records by assuring timely and accurate filing in accordance with the federal and state regulations and ACHC standards.
Coordinates with the branch office staff and Community Relations to assure that physician orders are obtained in a timely manner.
Organizing, filing, and updating patient medical records, including both physical files and electronic health records (EHRs)
Maintains the processes for resending of physician orders for Home Care and Hospice programs, including contacting the physicians' offices, to assure timely signatures are obtained in accordance with Federal and State regulations. Coordinates with the Community Relations Department to assure that problematic physician orders are obtained.
Coordinates with accounting staff to assure that physician information regarding demographics and orders is entered timely and accurately for the successful transmission of filed claims to the federal government.
Communicates with the Team Support Coordinators/Administrative Assistants, Clinical Managers, Case Managers, Quality Improvement Department, and other identified staff to assure that patients clinical records are maintained and completed in an accurate and timely manner.
Works with management and staff to assist in Medical Records portions of month end billing closing.
Processing requests for medical records from patients, healthcare providers, insurance companies, and other authorized entities.
Assists in preparing patient records for review and audits.
Supports the preparation of Foundation Check processing
Cross-trained to other functions within the Medical Records Department.
Prepares lists for offsite storage and destruction. Retrieves and returns records from off site storage. Participates in purging medical records in preparation for offsite storage.
Supports assurance of confidentiality by processing shredding of agency's various shredding collection receptacles.
Supports and participates in opening and securing of the records room and records areas.
Participates in quality improvement activities and team related activities.
Performs other duties as assigned consistent skills and training, and the mission and goals of Pathways.
QUALIFICATIONS:
High school graduate or GED equivalency.
Minimum of 3-5 years experience in a medical office setting; medical records experience required
Typing accurately with a speed of no less than 40 words per minute.
Computer proficiency; Google based systems.

AREAS OF RESPONSIBILITY:
* Knowledgably discusses overall history, philosophy and services of Pathways Home Health and Hospice.
* Completes an in-depth spiritual assessment of patients/families assigned for follow-up care.
* Maintains the dignity of the dying patient and supports the patient's and family's unique spiritual and cultural beliefs.
* Develops an individualized spiritual care plan, based on the spiritual assessment and the patient's and family's acceptance of these services. The plan of care integrates input from the various disciplines to create a comprehensive interdisciplinary plan of care including goals, interventions, and projected visit frequencies.

POSITION SUMMARY: The Hospice Client Relations Executive (CRE) makes essential contributions to the achievement of the organization's objectives as a field-based business development representative, consistently meeting the volume expectations for referrals and admissions. The CRE is expected to travel to various referral sources and potential patients and communicate Pathways Hospice services, patient criteria and agency policies to secure appropriate contracts for patient care. The position maintains and grows current market share and grows new sources of business sufficient to support the branch's business plan. This position will work with the Director, Business Development; Regional Vice President, Market Development; or Vice President, Sales and branch Director, Operations to develop and execute specific strategies and tactics to achieve revenue targets, patient experience and profitability goals.

* B. Develops professional working relationships with health care providers and facilities, generating
an open flow of information and support of home care goals; provides value added counsel,
teaching and resourceful problem solving.
* C. Available to all discharge planners, physicians, other personnel and clients to 1) analyze
eligibility for hospice and provide general guidance in determining if a patient would benefit
from hospice services.
* D. To ensure that patients receive the right care that addresses their needs, only a registered
nurse, licensed vocational nurse, medical social worker, chaplain, or counselor can complete a
patient's "election of hospice, informed consent, completed signatures, and counsel on the
election of hospice to a patient, patient's family, or patient's representative."
QUALIFICATIONS:

POSITION SUMMARY:
The Spiritual Care Manager is responsible for the management and coordination of the Hospice spiritual care program, for overseeing the provision of spiritual counseling services to patients and families when requested and for psycho-spiritual support services to Hospice staff.
AREAS OF RESPONSIBILITY:
Assures the development and implementation of a spiritual plan of care for patient/family with the interdisciplinary team.

POSITION SUMMARY:
The HHA provides personal care, home hygiene services, and other supportive services to persons who are receiving Hospice care at home. All such services will be provided in accordance with a comprehensive patient care plan and under the supervision of a registered nurse. The Hospice Aide will communicate with the Case Manager to provide feedback regarding patient status and participate in the patient care planning process.
AREAS OF RESPONSIBILITY:
Provides in-home personal care and supportive services to Hospice patients. Personal care and supportive services include assisting a patient in and out of a shower, giving bed bath, sponge bath or shower, shampoo, shave, skin care, nail care, backrub, oral hygiene, passive range of motion, linen change - making an unoccupied bed and occupied bed; assisting with ambulation devices (cane, walker, wheelchair), transferring from bed to chair and from chair to bed, use assistive devices such as Hoyer lift; assisting a patient with toileting activities (use of commode, bedpan, urinal, toilet); emptying reservoirs such as Foley Catheter bags or colostomy bags; assisting with feeding patient; maintaining safe kitchen and bathroom; monitoring of vital signs and assist patient/family as specified by case manager or supervisor.
Other supportive services include comfort measures and integrative therapies, such as aromatherapy and Comfort Touch©™. Comfort measures include providing companionship and conversation with patient and family members and activities such as reading to patient or sitting quietly with the patient, talking with the patient about patient's life or hobbies, and patient projects, such as organizing pictures in a book.
Under the direct supervision of each patient's case manager, initiates the hospice aide care plan; provides feedback to case manager regarding changes in patient's physical, social, or psychological condition; and makes changes in care as directed.
Assists in training individuals or families in the care of terminally ill patients.
Maintains the dignity of the dying patient and supports the patient's and family's unique spiritual and cultural beliefs.
Performs other tasks as requested by the case manager to assist the patient in obtaining needed health services or follow-up care.
Accesses electronic device for care plan prior to patient visit.
Completes appropriate records following all patient visits.
Synchronizes electronic device data after the patient visit.
Completes any other documentation requested by the agency.

Notifies Home Health Aide Department daily of intended visit route and visit times. Communicates promptly and appropriately with patients and families regarding scheduled visits.

Meets productivity standards established by the Home Health Aide Program Director.
Maintains the ethical and confidentiality standards in the administration of Home Health Aide services.
When in a facility (RCFE or SNF), utilizes customer service practice and department procedures for working in and with facilities.
Maintains communication with a case manager, patient, family and facility staff regarding all aspects of patient care.
Demonstrates awareness of and adherence to the safety, infection control and other policies of the facility.
Attends and actively participates in the hospice interdisciplinary group (IDG) meetings. When attendance is not possible, provides appropriate input on assigned patients.
Attends educational in-service trainings, department meetings and other agency meetings as requested by the manager.
Maintains and updates the level of knowledge and skills required to provide quality hospice aide care.
Adheres to all agency policies and procedures; including updates, as communicated in written, spoken and electronic forms.
Submits time log when due.

Serves as a resource to the new clinical provider, after orientation is complete.
Assists with the orientation of other providers, such as new nurses, therapists, social worker, spiritual care counselors and any office staff, such as schedulers and new managers. Participation in these orientations is usually accompanied by patient visits.
Maintains a positive attitude and willingness to share expertise to new clinical provider or office staff.
Acts a Mentor for Home Health Aide students' practicum.

TITLE: Occupational Therapist (OT) - Home Health

POSITION SUMMARY: Occupational Therapy goals are to restore and maximize function, facilitate activities of daily living after disabilities have occurred in order to prevent further restrictions of function and to help the client reach his/her maximum performance level within the limits of his/her capabilities. The Occupational Therapist evaluates and treats patients using medically prescribed occupational therapy programs.
AREAS OF RESPONSIBILITY:
Makes evaluation visits. Notifies assigned Clinical Team Manager of OASIS scores within established timelines. Notifies appropriate team members of evaluation and general plan for follow up.
Performs an initial, comprehensive assessment which includes but is not limited to the patient's eligibility for home care services in accordance with third party payer regulations, an accurate reflection of the patient's current health status, review of all medications the patient is currently using, an environmental assessment which includes physical, social and mental status, identified needs and potential to reach treatment and discharge goals.
Documents observations, clinical findings, problems, skilled interventions/treatments, goals and discharge plans.
In consultation with the assigned Clinical Team Manager, initiates and regularly re-evaluates and revises the plan of care.
On an on-going basis, documents observations, clinical findings, problems, skilled interventions/treatments, goals and discharge plans.
Assesses the need for the services of other team members (RN, PT, ST, MSW, and HHA).
Provides and documents skilled care, interventions/treatments on all visits (includes skilled observation of the patient's condition, skilled care, medical procedures or treatments performed and teaching of the patient and/or family).
Utilizes appropriate evaluation tools for planning and administration of programs as well as evaluating treatment effectiveness and equipment, in accordance with physician's orders.
Considers the patient's own needs and goals, conditions, and environment and coordinates with ancillary services when planning treatment goals.
Follows established standards for point of service technology, documentation, and synchronization.
Uses agency provided telecommunication devices (i.e. cell phone, voice mail, and pager) according to established guidelines.
Submits weekly visit schedule of assigned patients. Collaborates with Clinical Team Manager(s) to address scheduling needs.
Performs resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager.
Submits all documentation according to agency established timeframes.
Meets established productivity standards.
Attends and actively participates in the clinical team multidisciplinary patient conference. When attendance is not possible, provides appropriate input on assigned patients.
Demonstrates and consistently utilizes established clinical competencies.
Seeks out and maintains skills and knowledge relevant to rehabilitation in the home care setting.
Plans a treatment program to develop or restore function through cognitive/perceptual retraining, facilitation/inhibition techniques, muscle re-education, developmental motor plan, coordination exercises, transfer training with ADL's, joint protection techniques.
Plans a treatment program to maximum performance and safety through environment adaptation, use of appropriate adaptation aid, setting up of ADL home program based on identified needs and customary clinical practice standards.
Instructs patient and family in home programs to be continued in the therapist's absence
QUALIFICATIONS:
Graduate from an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association.
Current California registration by the AOTA.
Minimum of two years experience as an Occupational Therapist. Prefer experience and interest in gerontology and home care.
Current California driver's license and automobile insurance.
Current CPR Certificate.

The Home Health Licensed Vocational Nurse (LVN) is responsible for providing skilled nursing care to assigned patients and their families. Documents observations and interventions related to the patient's environment physical, mental, social and emotional status. Communicates patient status and needs to be based on a plan of care with the Clinical Team Manager and other members of the home care team. Performs duties consistent with the Vocational Nurse Practice Act.

QUALIFICATIONS:
Current California LVN License.
Preferred: IV Therapy and Blood Withdrawal Certification
