Job Title & Role Description: The Care Specialist is a key member of the team responsible for delivering chronic care management to high-complexity patients. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual care (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, facilitating virtual appointments with providers, and ensuring that patients adhere to care plans. By building relationships, coordinating care, and providing education, the Care Specialist plays a critical role in promoting health, preventing unnecessary hospitalizations, and improving patient outcomes. Skills Required: Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.). High school diploma or GED required.     The Pet Sitter position is for am/pm and can be afternoon visits for dogs. The hours are typically between 7-10 am, 1-3 pm and 6-9 pm. Cat visits are typically one time a day and may be flexible on times. The visit time duration is either 30 or 60 minutes. These are vacation visits so most jobs last anywhere from 3 days to 3 weeks. All Staff are flexible part time. Compensation: Walkers receive a 45% commission of revenues paid by their clients After 6 months commission increases to 50%. Required Duties You must have internet access and smartphone. You will go to an initial consultation to meet the owner and pets to see if you are a good fit and vice versa. You will work out details for obtaining keys to client's home at the initial consultation. You will not be compensated for this as it is an interview. If you need to make additional visits before starting the job you will be compensated for that. Visits for cats include scooping litter daily and changing the litter entirely after a week, feeding and loving the kitties, giving fresh water daily, bringing in the mail and newspaper, and watering a small number of plants. Most cat visits are once a day and that time may be flexible. (If a client should want a specific visit time for a cat you will be informed before you start sit.) Pet Sitting visits for dogs should be done at roughly the same time each day to keep the continuity for the dogs. Dog visits are either 2 or 3 times a day. Visits for dogs include feeding, giving fresh water, playing, bringing in the mail and newspaper, and watering a small number of plants, plus a 30-60 minute walk per visit. Due to the bonding that occurs between sitter and animal we require at least a six month commitment to working with our company on an ongoing basis as a pet visitor. We get a lot of repeat clients. Clients (and animals) value the continuity and stability that our long-term staff provides them. (We assume that you do, or you wouldn't be reading this!) ... beyond that, we look for someone who can master the THREE ASPECTS OF THIS JOB: 1. Be great with the PETS! Lots of people say they love animals, but it takes a special kind dedication to care for animals as your JOB. Neither the snow, nor rain, nor hissing scaredy-cat nor gloom of night.. nothing will keep you from getting to the pets in your care! Are you the kind of person who stops to pet all the dogs, sometimes forgetting to say hello to the human walking them? WE WANT TO MEET YOU!2. Be comfortable communicating with PEOPLE! We do lots of emailing and texting among our staff and clients, so we are looking for a new hire who can get back to us swiftly and professionally. You'll use our scheduling app which keeps track of your jobs and earnings. Your basic iPhone 8 plus with 128g would be about the minimum reqs there. Any smartphone with a robust data plan will work! Here are some other basic job requirements: - authorized to work in the US for at least the next 12 months - 21 years of age - full-time resident of NYC - able to commit to this position for 12 months minimum from date of hire - able to pass a background screening and/or drug test - available to work M-F, 10AM-6PM, PLUS some nights and weekends as requested in advance, plus one major holiday season (Christmas or Thanksgiving) We are physically hardy, able to walk for multiple hours per day in ALL WEATHER, can get to client pets in a 6th floor walkup building without issue, can lift an animal 30 lbs or more if needed. A working smart phone with unlimited data and a reliable mode of transportation are required. (Our top earners get around by bike, scooter, e-bike, moped, car or some other form of wheeled transport. However, some routes might be doable by MTA.
