This position plays a vital role in helping patients regain mobility and function within the comfort of their own homes. Job Duties Administer physical therapy treatments as prescribed in the patient's Plan of Care developed by the Physician and Physical Therapist. Monitor and document the patient's response to therapy and the overall effectiveness of services. Continuously assess the home environment for safety hazards; recommend modifications to enhance patient safety. Educate patients and family member on exercise programs, use of assistive devices, and home safety practices. Identify the need for additional services or disciplines, communicate with the supervising PT, and refer as appropriate. Evaluate social, emotional, and physical factors affecting the patient's condition and report findings to the supervising PT. Participate in interdisciplinary team meetings; contribute to care plan updates and problem-solving discussions.
