Performs reassessments as assigned and according to state and facility guidelines, adjusting treatment plan accordingly: Selects treatment methods appropriate to the patient's diagnosis, age group, physical and emotional status as evidenced by observation and documentation. Recognizes the need for alterations in the treatment method and overall plan as the patient advances or regresses, and acts accordingly as evidenced by observation, documentation and team feedback. Notifies primary PT/ OT of non-routine changes in the treatment situation, discharge complications or other circumstances that require reassessment or staff guidance as evidenced by team feedback. Instructs ancillary staff in patient positioning, mobility and care giving as evidenced by documentation. Educates patient and/or family in home exercise programs, transfers, care giving, home safety and use of adaptive equipment, appropriate to level of understanding. Adheres to department policy regarding time frame, appropriate form and content of written rehabilitation documentation: As evidenced by chart audits, completes daily progress notes that reflect patient's response to treatment, actual performance, and an assessment as to status, all relevant to the treatment plan and eventual discharge needs. Completes treatment summaries when applicable. As evidenced by chart review and team feedback, progress notes are written for each intervention. Reasons for canceled treatments are documented. Documents clearly, concisely and legibly within established time frames (e.g. Allows other therapists to continue treatment, allows for interpretation by payers, supports reimbursement) as evidenced by chart review and team feedback. Documents evidence of patient and family/care education as evidenced by chart review. Communicates effectively with patients, family, medical staff, care team members, students, supervisors and outside contacts: Contacts families, physicians or other health care team members in anticipation of special needs of the patients or family in a timely manner to facilitate effective treatment plans. Provides educational materials and methods appropriate to the patient's level of understanding and motivation. Includes family/care giver in education and training as appropriate as evidenced by chart review. Demonstrates collaboration in communication with team members and is open to feedback and receptive to change as evidenced by team feedback. Gives appropriate instruction and supervision to assistive personnel as evidenced by team feedback. Completes all administrative and other responsibilities in support of the operating unit: Maintains productivity standards set by department/clinic. Makes efficient use of non-billable time and minimizes use of non-billable time. Completes routine administrative responsibilities within established time frames consistently (e.g. TACS, charge sheets, productivity logs, and other clerical duties as requested). Contributes to educational and training efforts of department /clinic (e.g.in-services, students, new hire orientation) as evidenced by program/clinic specific documentation. Demonstrates fiscal responsibility and knowledge about department expenses and revenue (e.g. supplies, equipment, billing, and staff resources) used in the department as evidenced by team feedback and observation. Periodically work beyond usual working hours as needed for facility emergencies, weekends and holidays.
