We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish. Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our my Place PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant's preferred place. my Place Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country's most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.
About This RoleAs a Licensed PhysicalTherapistAssistant (LPTA) in our PACE program, you'll play a vital role in helping older adults maintain their independence, mobility, and overall well-being. This is more than a clinical role-it's an opportunity to truly connect. You'll build meaningful relationships, support whole-person care, and make a lasting difference in the lives of frail seniors. Whether you're helping a participant walk more confidently, recover from a hospitalization, or simply stay active and engaged, your care will be essential to their ability to thrive at home and in the community they love.
If you find joy in helping others move forward-literally and figuratively-and you're inspired by mission-driven work in a collaborative, supportive environment, we'd love to welcome you to the team. What Does Success Look Like In This Role?

Respectful, Person-Centered Care: Create a safe, comforting, and empowering space during each session-adapting your techniques to each individual's needs.
Supportive Education: Teach participants and caregivers how to safely use assistive devices, practice mobility techniques, and follow home exercise programs with confidence.
Collaborative Spirit: Work closely with the PhysicalTherapist and other members of the Interdisciplinary Team to ensure coordinated, holistic care for every participant.
Continuity of Care: Support transitions between home, hospital, or skilled nursing settings by helping ensure therapy services stay on track and goals are maintained.
Welcoming Environment: Keep therapy spaces clean, organized, and inviting-creating a place where participants feel safe and supported.
Reliable Documentation: Accurately record interventions and participant responses in the Electronic Medical Record (EMR) in accordance with PACE, state, and federal requirements.
Team Engagement: Actively participate in daily huddles, team meetings, and training sessions to stay informed, aligned, and inspired.
Flexible Support: Step in to assist with other participant care or team needs as they arise-bringing a helpful attitude and a willingness to contribute wherever you're most needed.
What Does An Ideal Candidate Look Like?
Qualified & Ready to Care: Graduate of an accredited PTA program with an active California license (or ability to obtain one).
Prepared for the Unexpected: CPR certification (or willingness to obtain within 30 days of hire).
Experienced in Geriatric Care: At least 2 years of PTA experience, preferably with older adults or medically complex participants.
Rooted in Purpose: A genuine passion for supporting frail older adults through compassionate, person-centered care.
Strong Communicator: Ability to engage participants, caregivers, and teammates with empathy, clarity, and positivity.
Adaptable Clinician: Comfortable adjusting interventions based on participant needs, goals, and physical capacity.

Travel Between Locations: Some travel between program sites or participant homes may be required. Flexibility and reliable transportation are important to support care continuity across settings.
Team-Driven Culture: You'll be part of a warm, collaborative interdisciplinary team-including nurses, physicians, social workers, activity coordinators, and fellow therapists-working together to provide whole-person, mission-driven care.
Participant-Centered: Every day, you'll build meaningful relationships with older adults and their caregivers, empowering them to regain strength, confidence, and independence.
