Supports and provides educational material to patient/families for support groups as wellas community resources Provides education and training to patients, families, and caregivers regarding diagnoses,progress, and long-term functional outcomes Teaches, counsels, and communicates with the patient, family or caregiver educationalinformation related to the patient's diagnosis/condition Complies with departmental policies for documentation.
