Provides Case Managers with appropriate DME referrals in an adequate amount of time in which the Case Manager is able to secure the equipment for the patient prior to D/C from BEH. Provides information regarding follow-up services which may include a referral to other community resources. Participates in patient and family education process. Provides education and training to patients, families, and caregivers regarding diagnoses, progress, long-term functional outcomes. Documents patient/family education appropriately Supports and provides educational material to patients/families for support groups as well as communityresources. Strives to meet department productivity standards. Assists with coverage on other units when caseload permits and when multiple caseloads may be modified easily to permit one person to assist with coverage in another area. Complies with established standards of efficient patient scheduling to maintain appropriate levels of productivity when covering the inpatient unit. Attends staff meetings, team meetings, and mandatory in-services as scheduled. Completes mandatory hospital training annually and in a timely manner.
