WORK FOR US! PrimaryOne Health is currently seeking a Family Nurse Practitioner or Physician Assistant for our West Broad St. health center. Essential Job Responsibilities: To develop and implement a standard care arrangement with each supervising physician, updated annually. In compliance with Section 472343 of Ohio Revised Code. Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care. Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral. Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issue. Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatment and/or medications. Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations consistent with organization's regulatory standards. Facilitates evaluation of records by physician(s), peers, and Quality and Standards according to protocols, and receives and implements constructive directives. Occasional travel required. Other duties or special projects as assigned. Successful candidates must possess an Ohio State certification as a Nurse Practitioner or Physician Assistant in the appropriate specialty-pediatric, family, adult, and women's health, and shall maintain such certification or eligibility during the term of their employment. Bilingual in Spanish required.
