Our Company ResCare Community Living Work in conjunction with diverse clinical teams and utilize community resources to meet the needs of individuals receiving care management services. Provide services in accordance with care management service requirements set by the state and company. Responsible for developing and monitoring Tailored Care Management care plans and Individual Support Plans (ISPs) built from comprehensive assessments to an assigned caseload.
Responsibilities
* Develops positive relationships among and between members, family/guardians, Extenders, clinical and care team members and other community stakeholders to create an environment of compassion and professionalism, driving toward positive health and quality of life outcomes.

* Connects members with medical, mental, developmental, psychosocial, housing, transportation, home health, and community support services/systems to achieve a comprehensive, holistic, preventive approach.
* Empowers the member/family/guardian and other team members with knowledge that aids in implementing the care plan, treatment plan, medication regimen, and appointment keeping.
* Identifies barriers, gaps, and unmet health-related needs are addresses them proactively, expanding relationships and linkages to aid in meeting member's needs.
* Supervises up to two FTEs of care management extenders.
* Provides services that meet national, state, and local healthcare standards at the highest level.
* Reports issues of concern, general departmental activities and staffing needs to the Care Management Supervisor.
* Completes all required training and participates in educational sessions to improve overall skills.
* Attends industry meetings, training, and functions to promote positive relationships with stakeholders.

Qualifications

Education:

For care managers serving members with LTSS needs: two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience, in addition to the requirements cited above. Responsibilities
As a Home Health PTA, You will:
* Provides physical therapy services planned, delegated and supervised by the qualified Physical Therapist in accordance with the patient's Plan of Treatment
* Confers with Physical Therapist regularly regarding patient's plan of treatment, status of progress toward goals, questions and concerns
* Communicates patient's needs and progress to the Physical Therapist of record and assists in securing special adaptive equipment for patient to improve function. Incorporates the use of adaptive equipment to enhance the rehabilitation program

Responsibilities
* Assesses all patients assigned and establishes and implements the Plan of Care
* Adheres to policies and procedures addressing the scope of services outlined in both Infusion nursing services, pharmacy services and state laws and regulations
* Adheres to INS and IgNS standards of practice, CDC guidelines, scope and standards of practice, and code of ethics as defined by American Nurses Association
* Collaborates with pharmacy staff, sales partners, ordering provider and patient to develop and monitor goals of patients' therapies
* Completes patient care documentation in place of service including patients' home or AIS
* Manages vascular access devices, to include insertion of peripheral infusion devices and ability to draw labs via peripheral venipuncture
* Educates patients and caregivers
* Evaluates progress toward goals and makes recommendations to physicians for changes in the Plan of Care based on patients' needs
* Coordinates nursing care with home health and other community agencies as indicated
* Participate in on-call rotation as scheduled per branch protocol
* Complete expense reports monthly tracking mileage and other miscellaneous incidental expenses
* Complete annual competencies and mandatory training within assigned due dates
* Supervisory Responsibility: No
Qualifications
* Graduate of an accredited school of nursing; Bachelor of Science in Nursing preferred
* Valid nursing license in state(s) of practice
* Minimum of one (1) year of experience as a Registered Nurse
* Previous infusion and home health care experience preferred
* Extensive experience in IV therapy preferred
* Current CPR certification
* Flexible schedule with the availability to work a variety of shift models
* Ability to work effectively within a multidisciplinary team
* Effective verbal and written communication skills
* Valid driver's license and auto insurance
* Must meet company standards for driving record as well as driver age requirement
* Percentage of Travel: 100%-Regular driving to patient's homes for visits is required as part of this position
To perform this role will require constantly sitting, standing, walking, bending, reaching, crouching, stooping and typing on a keyboard with fingers. The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs

Responsibilities
As a Home Health Clinical Manager, You will:
* Conducts/participates in quarterly quality improvement meetings, reviews pertinent available data, identifies trends, and oversees the implementation of improvement plans
* Collaborates with the Branch Director in the implementation of quality improvement activities and corrective action plans
* Responsible for the oversight of and response to external and internal surveys and audits within the assigned area relating to the quality of care and clinically related KPIs in collaboration with operations leadership
* Completes documentation and chart reviews to identify variances in standardized care and care processes and to identify areas for improved performance related to patient care and the organization
* Holds case conferences to ensure oversight of care, coordination of services and that standards are met
* Facilitates the development of performance improvement action plans, based on available data (e.g., survey results, QAPI and clinical record reviews). Accomplishes staff results by communicating job expectations; planning, monitoring, and appraising job results; coaching, counseling, and disciplining employees; developing, coordinating, and enforcing systems, policies, procedures, and productivity standards
* Ensures assigned staff are evaluated fairly and timely in accordance with company performance evaluation procedures
Qualifications
* Bachelor's Degree preferred in Nursing from an accredited school of nursing
* Licensed Registered Nurse (RN) in good standing and currently licensed by the State
* Less than two years of clinical experience, and at least one year in a related management/supervisory role in home health care
* Current CPR certification
* Must meet all agency requirements for pre-employment as required by the company and/or State regulations
* Ability to use company documentation systems
* Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
* Ability to work independently as well as part of a team
* Capable of working responsibly with highly confidential information

Our Company Adoration Home Health and Hospice The Care Transition Coordinator (CTC) plays a pivotal role in facilitating seamless transitions for patients from healthcare facilities to home health or hospice care. This position is responsible for evaluating patient eligibility, coordinating care plans, and ensuring all services-including ancillary needs such as DME and infusion-are arranged in alignment with agency protocols and patient needs. The CTC serves as a liaison between the agency, referral sources, and healthcare providers, ensuring timely communication, documentation, and patient education. By executing strategic outreach plans and managing sales-related administrative functions, the CTC supports market growth, maintains compliance with financial stewardship, and enhances patient satisfaction through personalized, informed care transitions.
Responsibilities
* Achieve monthly personal production goals and Medicare-certified (MC) admission targets for assigned locations. Manage sales and marketing expenses to ensure financial stewardship and return on investment.• Implement weekly, monthly, and quarterly strategies to increase market share within assigned facilities.• Evaluate patients and physician orders for home care eligibility in accordance with Right of Choice guidelines.• Conduct face-to-face patient transitions to provide agency education and identify the primary care physician responsible for the plan of care.• Present identified patient needs to the Executive Director to obtain branch approval and acceptance. Complete Care Transition Coordinator (CTC) encounter documentation in Home Care Home Base.• Upon patient acceptance, coordinate transfer orders and ancillary services (e.g., DME, infusion). Educate patients on home care or hospice orders and related services received from the referral source.• Ensure all patient needs identified by the referral source are documented and addressed by the agency upon acceptance.• Collaborate with the Executive Director and Clinical Director to promote growth by aligning team efforts with the needs and expectations of referral sources and patients.
* Perform sales administration duties including BOA expense entry, adherence to BOA policies and procedures, payroll timesheet submission, participation in weekly 3LS meetings, submission of PTO requests, and attendance at required sales calls and company-provided in-services. Maintain timely communication via phone and email.• Educate patients on the importance of post-discharge physician appointments, obtaining necessary prescriptions prior to discharge, and understanding medication regimens, pharmacy use, and delivery methods.• Act as liaison between the agency and healthcare providers for newly referred patients and existing patients transferred to hospitals from home health services.• Notify discharge planning of active patients transferred from home health to a facility. Coordinate resumption of care with patients prior to discharge when applicable orders are obtained.• Provide follow-up feedback to the case management team on readmission status and non-admitdecisions based on agency-provided information.• Maintain patient confidentiality in accordance with applicable laws and agency policies.• Demonstrate knowledge of agency services, competitive advantages, specialty programs, and Medicare guidelines. Educate medical professionals using appropriate tools and literature.
Qualifications
* Required: Minimum of one (1) year of experience in home health or hospital-based case management.• Preferred: One (1) to three (3) years of experience in medical marketing or healthcare business development.• Current and active licensure in the state of practice as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Social Worker (SW), or Physical Therapist (PT) is required.• Respiratory Therapist (RT) certification and/or completion of a technical clinical program demonstrating strong clinical knowledge is preferred.• Must possess a valid driver's license, reliable transportation, and current auto insurance.• Demonstrated understanding of home health eligibility criteria and Medicare/insurance coverage guidelines is required.
