What You'll Do

Partner with program leadership to eliminate silos and drive operational excellence
Play a key role in regulatory surveys, audits, and continuous improvement initiatives

Minimum Requirements:

Community Habilitation offers a wealth of valuable, one-on-one services to children and adults with disabilities. The program offers choices and increased independence while encouraging productivity and shared community involvement. Through a variety of activities geared toward enhancing daily life-skills development, our knowledgeable, creative staff provide families with much-needed assistance by spending time with program participants in their homes and out in the community.
Independent self-starters needed to work directly one-on-one in the community with individuals with intellectual disabilities. Responsibilities include training individuals in the performance of personal hygiene and self-care tasks (dressing, grooming, etc.) general household chores, mobility to maximize independence in personal health care and travel, financial management, appropriate social behaviors. Duties include: following the Staff Action Plan based a person - centered approach, recording and monitoring progress, maintaining appropriate documentation, and assume driving responsibilities.
The ComHab/DSP/CCSM works one-on-one with individuals with intellectual and developmental disabilities by using strategies and community-based activities designed to promote skill building in the areas of communication, socialization, behavior, independent living, movement, etc. CHDSP's are involved in the creation and assessment of habilitative and behavioral plans that they will implement.
The CHDSP demonstrates knowledge of Richmond's quality mission, vision, and values. In addition, this position actively participates in quality improvement projects (i.e. communicating issues, offering suggestions) to ensure ongoing excellence of Richmond residents, staff, and programs.

Role's Responsibilities
• Performs clinical/physical assessments and triage.
• Implements plan of care in collaboration with other disciplines. Evaluates plans of care using interdisciplinary approach and updates plans as needed.
• Arranges medical services for supported individuals.
• Provides care direction as needed to LPN and DSP staff based on the individual's plan of nursing care.
• Supports the interdisciplinary approach to individual care as well as the team process and participates on the team as directed by the Director of Nursing or designee.
• Ensures that all individuals' medical records are in compliance with all applicable regulations.
• Provides restorative and rehabilitative services as necessary, i.e., bowel and bladder training, ostomy care and delivery of oxygen therapy.
• Performs on-going surveillance and nursing intervention to rescue chronically ill individuals in the agency's care from the development of negative side effects and secondary results of treatment.
• Takes immediate action, upon discovery of a medical issue, including notifying the • Director of Nursing or designee and Residential Services management.
• Gives medications and treatments as indicated.
• Takes and records vital signs.
• Provides oversight to AMAP certified staff, which includes trainings and in-servicing pertaining to regulations/OPWDD requirements.
• Participates in and/or directs the rendering of first aid and emergency care as needed or directed.
• Maintains nursing office(s) with supplies as needed, including, but not limited to, OTC medications and durable medical equipment.
• Maintains nursing office/nursing practice in compliance with OSHA guidelines.
• Utilizes principles of infection control when caring for supported individuals who attend community programs.
• Provides coverage for other agency nurses, as directed by the Director of Nursing or designee.
• Assists in writing and reviewing agency nursing policies and procedures as directed.

What You'll Be Doing:
Managing Benefits: You will be responsible for all aspects of our employee benefit plans, including timely additions and terminations for medical, dental, life, and disability benefits. This includes calculating and processing monthly premiums, handling invoices, and generating reports for various health and welfare benefits. You'll make sure our employees have access to the benefits they need and that all records are maintained accurately.
COBRA & Disability Claims: You'll help employees maintain their benefits through COBRA and manage both Short-Term and Long-Term Disability claims. As the main point of contact, you'll resolve any claims or issues that arise with benefits.
Leave of Absence Support: You'll manage leave requests, ensure proper documentation is in place, and coordinate with managers to ensure smooth transitions for employees on medical, family, or personal leave.
Open Enrollment Coordination: You will assist with Open Enrollment for medical and dental benefits by creating informative materials, coordinating carrier reps, and ensuring employees are well-informed to make the best decisions for themselves and their families.
Claim Troubleshooting & Liaison: As a key resource, you will handle benefit-related inquiries, working directly with employees and benefit carriers to resolve any issues quickly and efficiently.
Administrative & Compliance Tasks: You will ensure all deadlines are met by processing invoices, managing claims, submitting reports, and handling any unemployment claims as necessary, all while ensuring compliance with applicable regulations.
What We're Looking For:
A Bachelor's degree in Human Resources or a related field (or equivalent experience)
At least 2+ years of experience in benefits administration, leave management, or a similar HR role (5+ years is a big plus!)
Strong attention to detail and organizational skills, particularly in managing all aspects of benefit plan additions, terminations, and eligibility
Excellent communication skills , with the ability to assist and guide employees through benefits processes
Reliable transportation for local travel and a valid driver's license are preferred
A team player who thrives in a collaborative environment and enjoys working toward a shared goal

Position Summary
As a Community Specialist III at The Arc Rockland, you will take a leading role in designing and delivering programs that foster independence, inclusion, and personal growth. This position combines hands-on support with program development and staff mentorship. You will lead instructional activities, conduct assessments, and connect individuals to opportunities that align with their goals - whether in skill-building, community engagement, or employment readiness. Your expertise ensures quality services, compliance with regulations, and a safe, supportive environment for all participants.
Role's Responsibilities

Program Development & Instruction
• Design and implement lesson plans based on individual program goals and assessments.
• Prepare instructional materials and lead hands-on activities in site-based and community settings.
• Teach functional skills, including work readiness, independent living, behavior management, and community integration.
Assessment & Planning
• Conduct and document assessments to identify interests, skills, and training needs.
• Match individuals with training, volunteer opportunities, internships, and employment aligned with their goals.
• Maintain and organize program documentation, including valued outcomes, behavioral data, monthly notes, and habilitation plans.
Direct Support
• Provide personal care assistance as needed (e.g., toileting, feeding, hygiene).
• Administer medications after AMAP certification.
• Ensure health, safety, and welfare of participants on-site and in the community.
Leadership & Mentorship
• Provide guidance and oversight to staff, including mentoring new team members and supporting orientation.
• Act as liaison with community organizations, volunteer sites, and agencies.
• Participate in planning meetings and contribute to team decision-making.
Compliance & Reporting
• Complete timely documentation and incident reports per regulatory requirements.
• Maintain organized program areas and replenish materials as needed.
• Perform bus duty functions, including traffic monitoring and attendance documentation.
Requirements
• Bachelor's degree in Special Education, Psychology, Human Services, or a related field (Five years of experience working in a direct support position outside of this agency, along with demonstrated competencies in completion of the required duties, may be substituted in lieu of a degree.)
• One (1) year of professional experience working directly with individuals with development disabilities preferred.
