    Job Description The FIT Care Coordinator provides intensive, community-based behavioral health support to children and youth (under 21) with Serious Emotional Disturbance (SED) and their families. Working closely with FIT Clinicians, Family Partners, and community resources, this role delivers skills coaching, care coordination support, and real-world practice to help families achieve treatment goals. Care is culturally responsive, trauma-informed, family-driven, and youth-guided. KEY RESPONSIBILITIES • Facilitate and participate in care coordination (calls, in-person, virtual) with schools, providers, state agencies, crisis teams, and community partners. • Provide skills coaching and support to youth and caregivers in home, school, and community settings. • Reinforce coping, communication, and problem-solving strategies identified in the treatment plan. • Partner with the FIT Clinician and Family Partner to ensure goals are reinforced consistently across environments. • Participate in family sessions, treatment planning meetings, and team check-ins. • Provide crisis support and assist with implementing safety plans developed by the FIT Clinician. • Monitor and document services, progress toward goals, and contacts in the youth's service record. • Serve as a positive role model for healthy routines, social skills, and resilience-building strategies. • Maintain timely, accurate documentation per agency, CBHI, and MassHealth standards. • Perform other related duties as assigned. REQUIREMENTS Must-Haves • Bachelor's or associate's degree in a human services field with a minimum of 3 years of experience working with children, youth, or families. • OR interns enrolled in a Master's degree program. • Ability to engage and build trust with youth and families from diverse backgrounds. • Skills in coaching/mentoring and reinforcing positive behaviors. • Strong communication, teamwork, problem-solving, organization, and documentation skills. • Knowledge of trauma-informed and culturally responsive practices. • Reliable transportation, valid driver's license, and insurance; ability to travel locally. • Ability to work some evenings/weekends and participate in on-call rotation. OVERVIEW: CSR believes that everyone deserves peace of mind. Mental health is the key to personal well-being, family and interpersonal relationships, and contributing to society. Our Behavioral Health Department (BHD) offers a comprehensive array of high quality, culturally-sensitive mental health services that provide children and families with the services they need for success at home, in school, and in the community. SUMMARY Intensive Care Coordination (ICC) is a service that facilitates care planning and coordination of services for Mass Health youth, with serious emotional disturbances, under the age of 21, and enrolled in Mass Health Standard or CommonHealth who meet the medical necessity criteria. Care planning is driven by the needs of the youth and developed through a Wraparound planning process consistent with Systems of Care philosophy. RESPONSIBILITIES • Oversee the daily activities of Intensive Care Coordinators (ICC), who: (1) conduct clinical and risk assessments, inclusive of Child/Adolescent Needs and Strengths (CANS) tool; (2) facilitate the development of a wraparound team; (3) convene team meetings; (4) coordinate and communicate with the members of the team to ensure the implementation of the individualized care plan; (5) work directly with the youth/family to implement elements of the care plan; (6) make referrals for services to meet their clinical needs; (7) monitor and review progress toward care plan goals. • Ensure that the ICCs conduct all necessary assessments in a timely manner and that they are reviewed and approved as required by the mandate. • Ensure ICCs update electronic health records in an orderly and timely manner • Conduct weekly supervision with ICCs and ensure all supervisory notes are maintained and readily available for review by auditors during site visits. • Meet weekly for supervision with the director of ICC; ensure all cases are discussed and supervisory meetings are documented according to guidelines. • Ensure Medical case consultations occur as needed. • Ensure ICCs work closely with Family Partners and the Senior Family Partner is kept abreast of active cases. • Ensure ICCs follow-through with on-call policies and procedures. • Work with Director to ensure the necessary trainings are conducted upon employment and annually. • Ensure all fidelity monitoring activities required by EOHHS, payers and CSR are being conducted. • Ensure all services are provided in a professional manner, ensuring privacy and safety. Educational/ Experience Requirements • Must be licensed at the Independent practice level to support programs requirements - LICSW, LMHC, or LMFT required. • Must be a master's level (or above) clinician with at least 3 years' experience in providing outpatient behavioral health services to youth and families. Care planning is driven by the needs of the youth and developed through a Wraparound planning process consistent with Systems of Care philosophy. Intensive Care Coordinator is the first level in the ICC series. Essential Duties & Responsibilities: The Director of CSA may designate various other activities.
