Coordinate care with the interdisciplinary team nurses, physicians, PT/OT, dietary, social work to optimize patient outcomes and support discharge planning. Participate in quality improvement and facility initiatives; advocate for patient centered care, ethical practice, and compliance with Medicare/Medicaid documentation standards (PDPM, Title 22, CMS). If supervising SLPAs or therapy aides, ensure compliance with California regulations: supervisor must have 2 years experience (or 3,120 hours), provide direct supervision for 20% of duties for first 90 days, and complete CEUs in supervision. Qualifications Master s degree in Speech Language Pathology from an accredited institution. Current California SLP license. Temporary RPE license accepted if in process. Certificate of Clinical Competence (CCC SLP) from ASHA preferred. Experience in skilled nursing facility or geriatric/rehabilitation settings preferred. TLC Nursing Associates, Inc. is currently seeking a compassionate and skilled Speech Language Pathologist (SLP) to assess, diagnose, and treat individuals with speech, language, communication, and swallowing disorders. The ideal candidate will deliver evidence-based therapy, work collaboratively with interdisciplinary teams, and support patients in achieving meaningful communication outcomes. Job Responsibilities Conduct thorough evaluations of patients with speech, language, voice, cognitive-communication, and swallowing disorders. Develop and implement individualized treatment plans based on clinical assessments and patient goals. Educate patients, families, and caregivers on treatment techniques and strategies for home practice.
