Provide assessment, diagnosis, and treatment of speech, language, cognitive-communication, and swallowing (dysphagia) disorders in a geriatric, medically complex population in a subacute/long term SNF setting. Collaborate with interdisciplinary team and support quality patient outcomes and regulatory compliance. Key Responsibilities Conduct comprehensive evaluations within 24 hours of physician referral to assess speech, language, cognition, and swallowing. Develop a functional therapy diagnosis and prognosis. Design and implement individualized treatment plans, use evidence-based interventions, adjust treatment based on progress, and establish clear, measurable goals. Provide direct therapy addressing communication and swallowing disorders in residents, including compensatory strategies, cognitive-communication interventions, and group therapy when appropriate. Document accurately and timely: evaluation reports, daily treatment notes, weekly progress updates, 14 day physician progress reports, discharge summaries, billing documentation, and any POC (plan of care) logs. Educate residents, families, and nursing/CNA staff on communication strategies, safe swallowing techniques, and home exercise programs. Coordinate care with the interdisciplinary team nurses, physicians, PT/OT, dietary, social work to optimize patient outcomes and support discharge planning. Participate in quality improvement and facility initiatives; advocate for patient centered care, ethical practice, and compliance with Medicare/Medicaid documentation standards (PDPM, Title 22, CMS). If supervising SLPAs or therapy aides, ensure compliance with California regulations: supervisor must have 2 years experience (or 3,120 hours), provide direct supervision for 20% of duties for first 90 days, and complete CEUs in supervision. Qualifications Master s degree in Speech Language Pathology from an accredited institution. Current California SLP license. Temporary RPE license accepted if in process. Certificate of Clinical Competence (CCC SLP) from ASHA preferred. Experience in skilled nursing facility or geriatric/rehabilitation settings preferred. Strong skills in clinical assessment and treatment planning, plus excellent communication, organization, and interpersonal abilities. Knowledge of Medicare/Medicaid billing, PDPM requirements, and California regulations (Title 22, CMS, etc.). Current BLS/CPR certification required. Work Environment & Expectations Typical workload includes both subacute rehabilitation stays (often up to 100 days under Medicare Part A) and long term care residents. Therapy intensity may reach up to ~3 hours/day depending on tolerance. Facilities often expect high productivity standards usually around 80 90% of available work time billed to therapy so proficiency in documentation and patient scheduling is essential. Work hours may fluctuate: quiet periods followed by days with multiple new admissions and intensive therapy sessions. Flexibility and adaptability are key. Building strong relationships with nursing and CNA staff is vital collaborate respectfully, avoid interrupting med passes, and support staff when possible. Professional License Requirements:

ASHA recognized Masters or Doctorate degree from CAA approved program
Active SLP license, authored in the state you're applying to
Have experience working with children or in schools is a plus but not required
What to expect from this role:
Implementing?SLP therapy treatment for school-aged children with cognitive communication disorders,?speech and/or language impairments or delays
Serving as a member of the Individualized Education Plan (IEP) Team for students with speech and/or language concerns
Maintain appropriate records?through Invo's proprietary software
Coordinate with school personnel and parents to develop appropriate programs for eligible students
Ensuring all services are performed in accordance with state laws and regulations
