Provides speech and language services to clients/patients/residents; monitors disease progression and adjusts treatments accordingly. Instructs clients/patients/residents in practicing speech and language therapeutic exercises; provides consultation for clients/patients/residents working through speech and language treatment processes. Ensures all speech and language related standards, policies, protocols and quality requirements are met. May treat individuals of all ages with varying diagnosis and disorders. Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Demonstrates level of treatment associated with their specialty provided to their clients/patients/residents. Follows and adheres to accepted clinical practice guidelines of professional organization. Exhibits flexibility with acceptance of variable work schedules/assignments. Will be expected to provide ongoing education and training to peers in the area(s) of expertise as determined by the department leadership. Performs other related duties as requested by manager. Based on assigned area, work may focus on those receiving intervention in the post-acute, long term care and home health care environments when applicable. **Qualifications** Appropriate education level required in accordance with state licensure. Must be licensed in the state(s) of practice as Speech-Language Pathologist. When working at a Sanford Health Facility (not required but preferred by GSS Facilities): Holds a Certificate of Clinical Competence issued by the American Speech-Language Hearing Association (ASHA) or currently working on Clinical Fellowship Year (CFY). When applicable, may require valid driver's license. Obtains and subsequently maintains required department specific competencies and certifications. S/he directs patient participation in selected tasks to restore, reinforce, and enhance performance. The Speech-Language Pathologist facilitates the learning of skills and functions essential for communication and swallowing, to diminish or remediate disorders or deficits. S/he directs activities to promote and maintain health, within the scope of practice and consistent with the Code of Ethics of the profession. RESPONSIBILITIES: 1. Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures, synthesis information and interprets examination findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care. Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the speech language pathology treatment plan, and completes all related documentation and record keeping regarding these services. Services application of therapeutic techniques in areas such as: cognitive-communication; speech intelligibility; oral motor skills; swallowing disorders/dysphagia; perceptual abilities; orientation; memory; pragmatics; psychosocial expression; and/or functional communication. 2. Conducts inservices and training for facility staff on an ongoing basis. Essential Duties * Evaluate patients within twenty-four (24) hours, or 1 business day of physician referral. If unable to complete evaluation in 24 hours, documentation in medical record clearly defines cause for delay. * Develop effective treatment plan and obtain approval for services from referring physician. * Treat patients per the physician treatment plan. * Supervise Speech-Language Pathologist Assistants in direct patient care and patient related activities, following state practice act. * Communicate with supervisor and other health team members regarding patient progress, problem and plans. * Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences as needed. * Participate in in-services training program for other staff in the facility. * Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per Speech and Language Pathologist in the Board State Practice Act and governmental and third party payer requirements. * Record treatment changes per policy and procedures. * Instruct patient's families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services. * Participate in discharge planning. * Act as a clinical preceptor for affiliating CFY/RFP Speech-Language Pathologist and student programs. * Comply with the Speech Language Pathology and Audiology Bureau State Practice Act and the facility policy and procedures. * Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues. Supervisory Requirements Supervises CFY - Speech Clinical Fellows, if applicable, and assists nursing with training of Restorative Aides. Qualification Education and/or Experience A master's or doctoral degree in SLP and meet either of these requirements: * Is licensed as an SLP by the state in which they provide services; OR * In the case of a person who practices in a state that doesn't license SLPs, they must have: o Successfully completed 350 clock hours of supervised clinical practicum (or be in the process of accumulating supervised clinical experience); o Performed at least 9 months of supervised full-time SLP services after obtaining a master's or doctoral degree in SLP or a related field; and o Successfully completed a national examination in SLP approved by the HHS Secretary. Proficient in computer skills such as inputting data into programs such as Casamba and Point Click Care. Certificates, Licenses, Registrations Licensed as an Speech Therapist in the state. Employee must meet continue education requirements per state practices. Must maintain a license in good standing at all time with the state board. Demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns. Follow GO PT policies and procedures, including safety processes. Job Requirements Must be licensed as a Registered Speech Pathologist in the state of Nebraska.
